This is especially important to know if you measure your own blood pressure at home.
Even if you don’t, it’s still good to know this as healthcare providers also can (and often will) do it wrong, especially if they are under time pressure (e.g. they need to get you out of their office and the next person in):
From the heart
Many things can change our blood pressure, and even gravity changes (considerably!) our blood pressure locally.
For example, even with good circulation, so long as we are in the Earth’s gravity under normal conditions (e.g. not skydiving, not riding a rollercoaster, etc), our blood pressure will always be higher below our heart, and lower above it, because gravity is pulling our blood downwards; this is also why if your circulation is not good, you may feel light-headed upon sitting up or standing up, as the bloodstream takes a moment to win a battle against gravity. This is also why blood rushes to your head if you are hanging upside down—increasing the local blood pressure in your head, which unlike your feet, isn’t used to it, so you feel it, and the effect may be visible from the outside, too.
When it comes to having your arm above or below your heart, the difference is less pronounced as it’s only a small change, but that small change can make a big difference:
- If the cuff is above heart level → Lower blood pressure reading.
- If the cuff is below heart level → Higher blood pressure reading.
- Every 1-inch difference causes a 2 mmHg change in readings.
For the reading to be accurate, the blood pressure cuff therefore needs to be at the same height as your heart.
You may be thinking: “my heart is bigger than an inch; do I aim for the middle?”
And the answer is: ideally the cuff should be at the same height as the right atrium of the heart, which is under the midpoint of the sternum.
However, your arm needs to be supported at that height, because if you have to keep it there using your own power, that will mean a tensing of your muscles, and increase in both heart rate and blood pressure. In fact, studies cited in the video found:
- Unsupported arm, in healthy patients → Systolic +8 mmHg, Diastolic +7 mmHg.
- Unsupported arm, in high blood pressure patients → Systolic +23 mmHg, Diastolic +10 mmHg.
Some other considerations; firstly, correct sitting posture:
- Sit upright with back support
- Feet flat on the floor, legs uncrossed
- Arm should be outward from the body and, as per the above explanation, supported (armrest, table, etc.)
And finally, you should be relaxed and at rest.
For example, your writer here is due for a regular checkup in a couple of weeks, and usually when I go there, I will have walked a couple of miles to get there, then bounced cheerfully up 6 flights of stairs. However, for this appointment, I will need to make sure to arrive early, so that I have time for my (so far as I know, happy and healthy) heart to return to its resting pulse and blood pressure.
Also, if you are anything like this writer, the blood pressure cuff activating is not a relaxing experience (and so invites a higher pulse and blood pressure), so it’s better to take three readings and then discard the first one, and record the average of the second two (I do it this way at home).
Similarly, if a medical environment in general is stressful for you, then taking two minutes to do a little mindfulness meditation, or even just breathing exercises, can be good.

Most books on the topic of longevity focus on such things as diet and exercise, and indeed, those are of course important things. But what of psychological and sociological factors?
Dr. Friedman and Dr. Martin look at a landmark longitudinal study, following a large group of subjects from childhood into old age. Looking at many lifestyle factors and life events, they crunched the numbers to see what things really made the biggest impact on healthy longevity.
A strength of the book is that this study had a huge amount of data—a limitation of the book is that it often avoids giving that concrete data, preferring to say “many”, “a majority”, “a large minority”, “some”, and so forth.
However, the conclusions from the data seem clear, and include many observations such as:
- conscientiousness is a characteristic that not only promotes healthy long life, but also can be acquired as time goes by (some “carefree” children became “conscientious” adults)
- resilience is a characteristic that promotes healthy long life—but tends to only be “unlocked” by adversity
- men tend to live longer if married—women, not so much
- religion and spirituality are not big factors in healthy longevity—but social connections (that may or may not come with such) do make a big difference
Bottom line: if you’d like to know which of your decisions are affecting your healthy longevity (beyond the obvious diet, exercise, etc), this is a great book for collating that information and presenting, in essence, a guideline for a long healthy life.

What’s the difference between physical and chemical sunscreens? And which one should you choose?

Sun exposure can accelerate ageing, cause skin burns, erythema (a skin reaction), skin cancer, melasmas (or sun spots) and other forms of hyperpigmentation – all triggered by solar ultraviolet radiation.
Approximately 80% of skin cancer cases in people engaged in outdoor activities are preventable by decreasing sun exposure. This can be done in lots of ways including wearing protective clothing or sunscreens.
But not all sunscreens work in the same way. You might have heard of “physical” and “chemical” sunscreens. What’s the difference and which one is right for you?
How sunscreens are classified
Sunscreens are grouped by their use of active inorganic and organic ultraviolet (UV) filters. Chemical sunscreens use organic filters such as cinnamates (chemically related to cinnamon oil) and benzophenones. Physical sunscreens (sometimes called mineral sunscreens) use inorganic filters such as titanium and zinc oxide.
These filters prevent the effects of UV radiation on the skin.
Organic UV filters are known as chemical filters because the molecules in them change to stop UV radiation reaching the skin. Inorganic UV filters are known as physical filters, because they work through physical means, such as blocking, scattering and reflection of UV radiation to prevent skin damage.
Nano versus micro
The effectiveness of the filters in physical sunscreen depends on factors including the size of the particle, how it’s mixed into the cream or lotion, the amount used and the refraction index (the speed light travels through a substance) of each filter.
When the particle size in physical sunscreens is large, it causes the light to be scattered and reflected more. That means physical sunscreens can be more obvious on the skin, which can reduce their cosmetic appeal.
Nanoparticulate forms of physical sunscreens (with tiny particles smaller than 100 nanometers) can improve the cosmetic appearance of creams on the skin and UV protection, because the particles in this size range absorb more radiation than they reflect. These are sometimes labelled as “invisible” zinc or mineral formulations and are considered safe.
So how do chemical sunscreens work?
Chemical UV filters work by absorbing high-energy UV rays. This leads to the filter molecules interacting with sunlight and changing chemically.
When molecules return to their ground (or lower energy) state, they release energy as heat, distributed all over the skin. This may lead to uncomfortable reactions for people with skin sensitivity.
Generally, UV filters are meant to stay on the epidermis (the first skin layer) surface to protect it from UV radiation. When they enter into the dermis (the connective tissue layer) and bloodstream, this can lead to skin sensitivity and increase the risk of toxicity. The safety profile of chemical UV filters may depend on whether their small molecular size allows them to penetrate the skin.
Chemical sunscreens, compared to physical ones, cause more adverse reactions in the skin because of chemical changes in their molecules. In addition, some chemical filters, such as dibenzoylmethane tend to break down after UV exposure. These degraded products can no longer protect the skin against UV and, if they penetrate the skin, can cause cell damage.
Due to their stability – that is, how well they retain product integrity and effectiveness when exposed to sunlight – physical sunscreens may be more suitable for children and people with skin allergies.
Although sunscreen filter ingredients can rarely cause true allergic dermatitis, patients with photodermatoses (where the skin reacts to light) and eczema have higher risk and should take care and seek advice.
What to look for
The best way to check if you’ll have a reaction to a physical or chemical sunscreen is to patch test it on a small area of skin.
And the best sunscreen to choose is one that provides broad-spectrum protection, is water and sweat-resistant, has a high sun protection factor (SPF), is easy to apply and has a low allergy risk.
Health authorities recommend sunscreen to prevent sun damage and cancer. Chemical sunscreens have the potential to penetrate the skin and may cause irritation for some people. Physical sunscreens are considered safe and effective and nanoparticulate formulations can increase their appeal and ease of use.

Let’s get straight to the point

However, after a while, it became less painful and more comfortable—except for trying standing on it, which still hurt (this writer has one too, and I often stand on it at my desk, whenever I feel my feet need a little excitement—it’s probably good for the circulation, but that is just a hypothesis)
- Soon, it became relaxing. Writer’s note: that raised hemicylindrical pillow she’s using? Try putting it under your neck instead, to stimulate the vagus nerve.
- While it is best use on bare skin, the effect can be softened by wearing a thin later of clothing between you and the mat.
- She got hers for £71 GBP (this writer got hers for a fraction of that price from Aldi—and here’s an example product on Amazon, at a more mid-range price)

- Any mutual-support oriented, we-have-this-shared-experience community, could be anything from AA to the VA.
Find your people, and surround yourself with them. There are more than 8,000,000,000 people on this planet, you will not find all the most compatible ones with you on your street.
Grow & nurture your community
Chances are, you have a lot to contribute. Your life experiences are valuable.
Being of service to other people is strongly associated “flourishing”, per the science.
Indeed, one of the questions on the subjective wellness scale test is to ask how much one agrees with the statement “I actively contribute to the happiness and wellbeing of others”.

So, help people, share your insights, create whatever is relevant to your community and fits your skills (it could be anything from art to tutorials to call-to-action posts or whatever works for you and your community)
As a bonus: when people notice you are there for them, they’ll probably be there for you, too. Not always, sadly, but there is undeniable strength in numbers.
Remember it’s not the boss of you
Whatever social media platform(s) you use, the companies in question will want you to use it in the way that is most profitable for them.
Usually that means creating a lot of shallow content, clicking on as many things as possible, and never logging off.
Good ways to guard against that include:
- Use the social media from a computer rather than a handheld device
- Disable “infinite scroll” in the settings, if possible
- Set a timer and stick to it
- Try to keep your interactions to only those that are relevant and kind (for the good of your own health, let alone anyone else’s)
On that latter note…
Before posting, ask “what am I trying to achieve here?” and ensure your action is aligned with your actual desires, and not just reactivity. See also:

The hit list
Dr. Li bids us avoid:
High carb, low fiber foods: consuming a diet high in carbohydrates, particularly refined carbs like cakes, white bread, pizza, and sugary syrups, can significantly harm brain health. Over time, imbalanced (i.e. not balanced with fiber) carbohydrate consumption leads to the growth of visceral fat (not the same as subcutaneous fat, which is the squishy bits just under your skin; visceral fat is further underneath, around your viscera), , which triggers systemic inflammation and oxidative stress. These processes disrupt communication between brain cells, impair memory, and increase the risk of diseases like Alzheimer’s and Parkinson’s. High carb diets can also contribute to metabolic syndrome—a cluster of conditions, including diabetes, obesity and high blood pressure—that damage blood vessels, leading to strokes and vascular dementia.
Trans fats: these are region-bound, as they’re banned in some places and not others—check your local regulations. Found in processed foods such as fried items, baked goods, packaged snacks, and margarine, trans fats are created through hydrogenation, which makes fats more stable at room temperature. These artificial fats raise bad cholesterol, lower good cholesterol, and promote atherosclerosis. This damages the brain by reducing oxygen supply, triggering chronic inflammation, and increasing the risk of Alzheimer’s and dementia.
Sodas: regular consumption of sodas, whether sugary or artificially sweetened, is harmful to brain health. A single can of soda contains around 9 teaspoons of sugar, which overwhelms metabolism, contributes to insulin resistance, and leads to inflammation. These effects damage blood vessels and brain tissue, disrupt neuron function, and increase the risk of type 2 diabetes and dementia. Furthermore, insulin resistance caused by excessive sugar intake can impair neuronal survival, activate immune responses, and exacerbate cognitive decline. As for the artificial sweeteners, the mechanism of harm depends on the sweetener (and some can also mess up insulin response, for reasons that are not entirely clear yet, but they measurably do), but even picking the healthiest artificial sweetener, training your palate to enjoy hyper-sweetened things will tend to lead to more sugar-laden food choices in other parts of one’s diet.
Processed foods: arguably a broad category that encompasses some of the above, but it’s important to consider it separately for catch-all purposes: these convenience foods, laden with artificial preservatives, colors, and sweeteners, harm brain health through chronic inflammation and usually a lack of essential nutrients. Processed foods are also a significant source of microplastics, which have been found to accumulate in the arteries, contributing to plaque build-up, atherosclerosis, and reduced blood flow to the brain. This combination of inflammation and oxidative stress from microplastics damages brain cells, paving the way for cognitive decline and dementia.
Seafood with high mercury levels: large fish such as tuna, swordfish, sharks, and tilefish accumulate high amounts of mercury, a potent neurotoxin. Fish that are larger, older, and/or higher up the food chain will have the most mercury (and other cumulative contaminants, for that matter, but we’re considering mercury here). Mercury disrupts essential brain chemicals like dopamine and serotonin, triggering oxidative stress and damaging brain cells. Chronic exposure to mercury leads to inflammation and neuroinflammation, both of which increase the risk of Alzheimer’s and dementia.
Alcohol: contrary to popular belief, any amount of alcohol is detrimental to brain health. While red wine is often promoted for its health benefits, the purported positive effects come from polyphenols, not the alcohol itself, and (for example) resveratrol from red wine cannot be delivered in meaningful doses without drinking an impossibly large quantity. Alcohol is a neurotoxin that can damage or kill brain cells, impair neuronal communication, and lead to cognitive decline. Excessive drinking results in hangover symptoms like headaches and brain fog, which are indicators of its harmful impact on the brain. Chronic alcohol consumption exacerbates neuron death, increases inflammation, and raises the risk of dementia.
As for what to eat instead?
Dr. Li recommends including foods such as:
- foods rich in omega-3s that aren’t mercury-laden fish, e.g. flaxseeds, chia seeds, walnuts, and hemp seeds, as they reduce inflammation, protect blood vessel linings, and prevent vascular dementia.
- berries, and in particular he recommends organic strawberries, which are rich in ellagic acid and anthocyanins, which improve memory, reduce depressive symptoms, and enhance cognitive function.

Rebalancing Dopamine (Without “Dopamine Fasting”)
Credit Steve Fisch This is Dr. Anna Lembke. She’s a professor of psychiatry at Stanford, and chief of the Stanford Addiction Medicine Dual Diagnosis Clinic—as well as running her own clinical practice, and serving on the board of an array of state and national addiction-focused organizations.
Today we’re going to look at her work on dopamine management…
Getting off the hedonic treadmill
For any unfamiliar with the term, the “hedonic treadmill” is what happens when we seek pleasure, enjoy the pleasure, the pleasure becomes normalized, and now we need to seek a stronger pleasure to get above our new baseline.
In other words, much like running on a reciprocal treadmill that just gets faster the faster we run.
What Dr. Lembke wants us to know here: pleasure invariably leads to pain
This is not because of some sort of extrinsic moral mandate, nor even in the Buddhist sense. Rather, it is biology.
Pleasure and pain are processed by the same part of the brain, and if we up one, the other will be upped accordingly, to try to keep a balance.
Consequently, if we recklessly seek “highs”, we’re going to hit “lows” soon enough. Whether that’s by drugs, sex, or just dopaminergic habits like social media overuse.
Dr. Lembke’s own poison of choice was trashy romance novels, by the way. But she soon found she needed more, and more, and the same level wasn’t “doing it” for her anymore.
So, should we just give up our pleasures, and do a “dopamine fast”?
Not so fast!
It depends on what they are. Dopamine fasting, per se, does not work. We wrote about this previously:

However, when it comes to our dopaminergic habits, a short period (say, a couple of weeks) of absence of that particular thing can help us re-find our balance, and also, find insight.
Lest that latter sound wishy-washy: this is about realizing how bad an overuse of some dopaminergic activity had become, the better to appreciate it responsibly, going forwards.
So in other words, if your poison is, as in Dr. Lembke’s case, trashy romance novels, you would abstain from them for a couple of weeks, while continuing to enjoy the other pleasures in life uninterrupted.
Substances that create a dependency are a special case
There’s often a popular differentiation between physical addictions (e.g. alcohol) and behavioral addictions (e.g. video games). And that’s fair; physiologically speaking, those may both involve dopamine responses, but are otherwise quite different.
However, there are some substances that are physical addictions that do not create a physical dependence (e.g. sugar), and there are substances that create a physical dependence without being addictive (e.g. many antidepressants)

In the case of anything that has created a physical dependence, Dr. Lembke does not recommend trying to go “cold turkey” on that without medical advice and supervision.
Going on the counterattack
Remember what we said about pleasure and pain being processed in the same part of the brain, and each rising to meet the other?
While this mean that seeking pleasure will bring us pain, the inverse is also true.
Don’t worry, she’s not advising us to take up masochism (unless that’s your thing!). But there are very safe healthy ways that we can tip the scales towards pain, ultimately leading to greater happiness.
Cold showers are an example she cites as particularly meritorious.
