We’ve previously reviewed “It’s Not You, It’s Your Hormones” by nutritionist Nikki Williams, and noted at the time that it was very similar to the bestselling “The Galveston Diet”, not just in its content but all the way down its formatting. Some Amazon reviewers have even gone so far as to suggest that “It’s Not You, It’s Your Hormones” (2017) brazenly plagiarized “The Galveston Diet” (2023). However, after carefully examining the publication dates, we feel quite confident that the the earlier book did not plagiarize the later one.

Of course, we would not go so far as to make a counter-accusation of plagiarism the other way around; it was surely just a case of Dr. Haver having the same good ideas 6 years later.

Still, while the original book by Nikki Williams did not get too much international acclaim, the later one by Dr. Mary Claire Haver has had very good marketing and thus received a lot more attention, so let’s review it:

Dr. Haver’s basic principle is (again) that we can manage our hormonal fluctuations, by managing our diet. Specifically, in the same three main ways:

  • Intermittent fasting
  • Anti-inflammatory diet
  • Eating more protein and healthy fats

Why should these things matter to our hormones? The answer is to remember that our hormones aren’t just the sex hormones. We have hormones for hunger and satedness, hormones for stress and relaxation, hormones for blood sugar regulation, hormones for sleep and wakefulness, and more. These many hormones make up our endocrine system, and affecting one part of it will affect the others.

Will these things magically undo the effects of the menopause? Well, some things yes, other things no. No diet can do the job of HRT. But by tweaking endocrine system inputs, we can tweak endocrine system outputs, and that’s what this book is for.

The style is once again very accessible and just as clear, and Dr. Haver also walks us just as skilfully through the changes we may want to make, to avoid the changes we don’t want. The recipes are also very similar, so if you loved the recipes in the other book, you certainly won’t dislike this book’s menu.

In the category of criticism, there is (as with the other book by the other author) some extra support that’s paywalled, in the sense that she wants the reader to buy her personally-branded online plan, and it can feel a bit like she’s holding back in order to upsell to that.

Bottom line: this book is (again) aimed at peri-menopausal and post-menopausal women. It could also (again) definitely help a lot of people with PCOS too, and, when it comes down to it, pretty much anyone with an endocrine system. It’s (still) a well-evidenced, well-established, healthy way of eating regardless of age, sex, or (most) physical conditions.

    For many, cannabis use has taken the place that alcohol used to have when it comes to wanting a “downer”, that is to say, a drug that relaxes us as opposed to stimulating us.

    Indeed, it is generally considered safer than alcohol ← however this is not a strong claim, because alcohol is much more dangerous than one would think given its ubiquity and (in many places, at least) social acceptability.

    So, about cannabis and heart attacks

    Alcohol is a relaxant, and yet it can contribute to heart disease (amongst many other things, of course):

    Can We Drink To Good Health? ← this is mostly about red wine’s putative heart health benefits, how the idea got popularized, and how it doesn’t stand up to scrutiny when actually looking at the evidence.

    And cannabis, another relaxant? Not so good either!

    New research has shown that cannabis users have a higher risk of heart attacks, even among younger and otherwise healthy individuals. This is based on analyzing data from 4,636,628 relatively healthy adults.

    Specifically, the data showed that even young healthy cannabis users get:

    • Sixfold increased risk of heart attack
    • Fourfold increased risk of ischemic stroke
    • Threefold increased risk of cardiovascular death, heart attack, or stroke

    We mention the otherwise “relatively healthy” nature of the participants, because it’s important to note that less healthy people (who were perhaps using cannabis to self-medicate for some serious condition) were not included in the dataset, as it’d skew the data unhelpfully and it’d make the risk look a lot higher than the risk levels we mentioned above.

    The mechanisms by which cannabis affects heart health are not fully understood, but hypotheses include:

    • Disrupting heart rhythm regulation
    • Increasing oxygen demand in the heart muscle
    • Causing endothelial dysfunction, which affects blood vessel function

    Further, heart attack risk peaked one hour after cannabis use, and while this doesn’t prove causality, it certainly doesn’t make cannabis look safe.

  • 10 Tips for Better Sleep: Starting In The Morning

    You’ll probably have heard similar advice before (including from us), but it’s always good to do a quick rundown and check which ones you are actually doing, as opposed to merely know you should be doing:

    • Wake up at the same time every day, including weekends, to maintain a consistent sleep schedule and avoid “social jet lag.”
    • Expose yourself to bright light in the morning, either sunlight or light therapy, to regulate your circadian rhythm and melatonin production.
    • Avoid caffeine late in the day to maintain natural sleep pressure, experimenting with a cutoff time based on your sensitivity (e.g. 6–10 hours before bedtime)*.
    • Limit naps to under 30 minutes and take them early in the afternoon to avoid disrupting sleep pressure.
    • Exercise regularly but avoid strenuous activity 2 hours before bed. Optimal exercise time is 4–6 hours before bedtime.
    • Avoid alcohol, as it disrupts sleep quality and may worsen conditions like sleep apnea. If drinking, have your last drink early in the evening—but honestly, it’s better to not drink at all.
    • Establish a wind-down routine 1–2 hours before bed, including dimming lights and engaging in relaxing activities to signal your body to prepare for sleep.
    • Keep your bedroom cool (below 68°F/20°C) and ensure your hands and feet stay warm to aid in natural body temperature regulation.
    • Limit device use before bed. If unavoidable, reduce blue light exposure and avoid mentally stimulating content. Set boundaries, such as placing your phone out of reach.
    • Ensure complete darkness in your sleeping environment using blackout curtains, covering light-emitting devices, or wearing a sleep mask.

    *we imagine she picked 6–10 hours because, depending on whether you have the fast or slow caffeine metabolizer gene, the biological halflife of caffeine in your body will be around 4 or 8 hours (that’s not a range, that’s two distinct and non-overlapping options). However, if we use 4 or 8 hours depending on which gene version we have, then that will mean that 4 or 8 hours later, respectively, we’ll have half the caffeine in us that we did 4 or 8 hours ago (that’s what a halflife means). So for example if you had a double espresso that number of hours before bedtime, then congratulations, you have the caffeine of a single espresso in your body by bedtime. Which, for most people**, is not an ideal nightcap. Hence, adding on a few more hours. Again, earlier is better though, so consider limiting caffeine to the morning only.

    **we say “most people”, because if you have ADHD or a similar condition, your brain’s relationship with caffeine is a bit different, and—paradoxically—stimulants can help you to relax. Do speak with your doctor though, as individual cases vary widely, and it also may make a difference depending on what relevant meds (if any) you’re on, too.

    How strange! We tested it in several desktop browsers and several mobile browsers, and were unable to find any version that didn’t work. That includes switching between metric and imperial units, per preference; both appear to work fine. Do be aware that it’ll only take numerical imput, though.

    While there are a lot of conflicting dietary approaches out there, the science itself is actually fairly cohesive in most regards. This book does a lot of what we do here at 10almonds, and presents the science in a clear fashion without having any particular agenda to push.

    The author is a nutritionist (BSc, MSc, RNutr) and therefore provides an up-to-date evidence-based approach for eating.

    As a result, the only part of this book that brings it down in this reviewer’s opinion is the section on Intermittent Fasting. Being not strictly about nutrition, she has less expertise on that topic, and it shows.

    The information is largely presented in double-page spreads each answering a particular question. Because of this, and the fact there are colorful graphic representations of information too, we do recommend the print version over Kindle*.

    Bottom line: if you like the notion of real science being presented in a clear and simple fashion (we like to think our subscribers do!), then you’ll surely enjoy this book.

  • Increase in online ADHD diagnoses for kids poses ethical questions

    In 2020, in the midst of a pandemic, clinical protocols were altered for Ontario health clinics, allowing them to perform more types of care virtually. This included ADHD assessments and ADHD prescriptions for children – services that previously had been restricted to in-person appointments. But while other restrictions on virtual care are back, clinics are still allowed to virtually assess children for ADHD.

    This shift has allowed for more and quicker diagnoses – though not covered by provincial insurance (OHIP) – via a host of newly emerging private, for-profit clinics. However, it also has raised significant ethical questions.

    It solves an equity issue in terms of rural access to timely assessments, but does it also create new equity issues as a privatized service?

    Is it even feasible to diagnose a child for a condition like ADHD without meeting that child in person?

    And as rates of ADHD diagnosis continue to rise, should health regulators re-examine the virtual care approach?

    Ontario: More prescriptions, less regulation

    There are numerous for-profit clinics offering virtual diagnoses and prescriptions for childhood ADHD in Ontario. These include KixCare, which does not offer the option of an in-person assessment. Another clinic, Springboard, makes virtual appointments available within days, charging around $2,600 for assessments, which take three to four hours. The clinic offers coaching and therapy at an additional cost, also not covered by OHIP. Families can choose to continue to visit the clinic virtually during a trial stage with medications, prescribed by a doctor in the clinic who then sends prescribing information back to the child’s primary care provider.

    For-profit clinics like these are departing from Canada’s traditional single-payer health care model. By charging patients out-of-pocket fees for services, the clinics are able to generate more revenue because they are working outside of the billing standards for OHIP, standards that set limits on the maximum amount doctors can earn for providing specific services. Instead many services are provided by non-physician providers, who are not limited by OHIP in the same way.

    Need for safeguards

    ADHD prescriptions rose during the pandemic in Ontario, with women, people of higher income and those aged 20 to 24 receiving the most new diagnoses, according to research published in January 2024 by a team including researchers from the Centre for Addictions and Mental Health and Holland Bloorview Children’s Hospital. There may be numerous reasons for this increase but could the move to virtual care have been a factor?

    Ontario psychiatrist Javeed Sukhera, who treats both children and adults in Canada and the U.S., says virtual assessments can work for youth with ADHD, who may receive treatment quicker if they live in remote areas. However, he is concerned that as health care becomes more privatized, it will lead to exploitation and over-diagnosis of certain conditions.

    “There have been a lot of profiteers who have tried to capitalize on people’s needs and I think this is very dangerous,” he said. “In some settings, profiteering companies have set up systems to offer ADHD assessments that are almost always substandard. This is different from not-for-profit setups that adhere to quality standards and regulatory mechanisms.”

    Sukhera’s concerns recall the case of Cerebral Inc., a New York state-based virtual care company founded in 2020 that marketed on social media platforms including Instagram and TikTok. Cerebral offered online prescriptions for ADHD drugs among other services and boasted more than 200,000 patients. But as Dani Blum reported in the New York Times, Cerebral was accused in 2023 of pressuring doctors on staff to prescribe stimulants and faced an investigation by state prosecutors into whether it violated the U.S. Controlled Substances Act.

    “At the start of the pandemic, regulators relaxed rules around medical prescription of controlled substances,” wrote Blum. “Those changes opened the door for companies to prescribe and market drugs without the protocols that can accompany an in-person visit.”

    Access increased – but is it equitable?

    Virtual care has been a necessity in rural areas in Ontario since well before the pandemic, although ADHD assessments for children were restricted to in-person appointments prior to 2020.

    But ADHD assessment clinics that charge families out-of-pocket for services are only accessible to people with higher incomes. Rural families, many of whom are low income, are unable to afford thousands for private assessments, let alone the other services upsold by providers. If the private clinic/virtual care trend continues to grow unchecked, it may also attract doctors away from the public model of care since they can bill more for services. This could further aggravate the gap in care that lower income people already experience.

    Sukhera says some risks could be addressed by instituting OHIP coverage for services at private clinics (similar to private surgical facilities that offer mixed private/public coverage), but also with safeguards to ensure that profits are reinvested back into the health-care system.

    “This would be especially useful for folks who do not have the income, the means to pay out of pocket,” he said.

    Concerns of misdiagnosis and over-prescription

    Some for-profit companies also benefit financially from diagnosing and issuing prescriptions, as has been suggested in the Cerebral case. If it is cheaper for a clinic to do shorter, virtual appointments and they are also motivated to diagnose and prescribe more, then controls need to be put in place to prevent misdiagnosis.

    The problem of misdiagnosis may also be related to the nature of ADHD assessments themselves. University of Strathclyde professor Matthew Smith, author of Hyperactive: The Controversial History of ADHD, notes that since the publication of Diagnostic and Statistical Manual of Mental Disorders in 1980, assessment has typically involved a few hours of parents and patients providing their subjective perspectives on how they experience time, tasks and the world around them.

    “It’s often a box-ticking exercise, rather than really learning about the context in which these behaviours exist,” Smith said. “The tendency has been to use a list of yes/no questions which – if enough are answered in the affirmative – lead to a diagnosis. When this is done online or via Zoom, there is even less opportunity to understand the context surrounding behaviour.”

    Smith cited a 2023 BBC investigation in which reporter Rory Carson booked an in-person ADHD assessment at a clinic and was found not to have the condition, then had a private online assessment – from a provider on her couch in a tracksuit – and was diagnosed with ADHD after just 45 minutes, for a fee of £685.

    What do patients want?

    If Canadian regulators can effectively tackle the issue of privatization and the risk of misdiagnosis, there is still another hurdle: not every youth is willing to take part in virtual care.

    Jennifer Reesman, a therapist and Training Director for Neuropsychology at the Chesapeake Center for ADHD, Learning & Behavioural Health in Maryland, echoed Sukhera’s concerns about substandard care, cautioning that virtual care is not suitable for some of her young clients who had poor experiences with online education and resist online health care. It can be an emotional issue for pediatric patients who are managing their feelings about the pandemic experience.

    “We need to respect what their needs are, not just the needs of the provider,” says Reesman.

    In 2020, Ontario opted for virtual care based on the capacity of our health system in a pandemic. Today, with a shortage of doctors, we are still in a crisis of capacity. The success of virtual care may rest on how engaged regulators are with equity issues, such as waitlists and access to care for rural dwellers, and how they resolve ethical problems around standards of care.

    Children and youth are a distinct category, which is why we had restrictions on virtual ADHD diagnosis prior to the pandemic. A question remains, then: If we could snap our fingers and have the capacity to provide in-person ADHD care for all children, would we? If the answer to that question is yes, then how can we begin to build our capacity?

  • Want the health benefits of strength training but not keen on the gym? Try ‘exercise snacking’

    The science is clear: resistance training is crucial to ageing well. Lifting weights (or doing bodyweight exercises like lunges, squats or push-ups) can help you live independently for longer, make your bones stronger, reduce your risk of diseases such as diabetes, and may even improve your sleep and mental health.

    But not everyone loves the gym. Perhaps you feel you’re not a “gym person” and never will be, or you’re too old to start. Being a gym-goer can be expensive and time-consuming, and some people report feeling unwelcome or awkward at the gym.

    The good news is you don’t need the gym, or lots of free time, to get the health benefits resistance training can offer.

    You can try “exercise snacking” instead.

    What is exercise snacking?

    Exercise snacking involves doing multiple shorter bouts (as little as 20 seconds) of exercise throughout the day – often with minimal or no equipment. It’s OK to have several hours of rest between.

    You could do simple bodyweight exercises such as:

    • chair sit-to-stand (squats)
    • lunges
    • box step-ups
    • calf raises
    • push-ups.

    Exercise snacking like this can help improve muscle mass, strength and physical function.

    It’s OK to hold onto a nearby object for balance, if you need. And doing these exercises regularly will also improve your balance. That, in turn, reduces your risk of falls and fractures.

    OK I have done all those, now what?

    Great! You can also try using resistance bands or dumbbells to do the previously mentioned five exercises as well as some of the following exercises:

    When using resistance bands, make sure you hold them tightly and that they’re securely attached to an immovable object.

    Exercise snacking works well when you pair it with an activity you do often throughout the day. Perhaps you could:

    • do a few extra squats every time you get up from a bed or chair
    • do some lunges during a TV ad break
    • chuck in a few half squats while you’re waiting for your kettle to boil
    • do a couple of elevated push-ups (where you support your body with your hands on a chair or a bench while doing the push-up) before tucking into lunch
    • sneak in a couple of calf raises while you’re brushing your teeth.

    What does the evidence say about exercise snacking?

    One study had older adults without a history of resistance training do exercise snacks at home twice per day for four weeks.

    Each session involved five simple bodyweight exercises (chair sit-to-stand, seated knee extension, standing knee bends, marching on the spot, and standing calf raises). The participants did each exercise continuously for one minute, with a one-minute break between exercises.

    These short and simple exercise sessions, which lasted just nine minutes, were enough to improve a person’s ability to stand up from a chair by 31% after four weeks (compared to a control group who didn’t exercise). Leg power and thigh muscle size improved, too.

    Research involving one of us (Jackson Fyfe) has also shown older adults found “exercise snacking” feasible and enjoyable when done at home either once, twice, or three times per day for four weeks.

    Exercise snacking may be a more sustainable approach to improve muscle health in those who don’t want to – or can’t – lift heavier weights in a gym.

    A little can yield a lot

    We know from other research that the more you exercise, the more likely it is you will keep exercising in future.

    Very brief resistance training, albeit with heavier weights, may be more enjoyable than traditional approaches where people aim to do many, many sets.

    We also know brief-and-frequent exercise sessions can break up periods of sedentary behaviour (which usually means sitting too much). Too much sitting increases your risk of chronic diseases such as diabetes, whereas exercise snacking can help keep your blood sugar levels steady.

    Of course, longer-term studies are needed. But the evidence we do have suggests exercise snacking really helps.

    Why does any of this matter?

    As you age, you lose strength and mass in the muscles you use to walk, or stand up. Everyday tasks can become a struggle.

    All this contributes to disability, hospitalisation, chronic disease, and reliance on community and residential aged care support.

    By preserving your muscle mass and strength, you can:

    • reduce joint pain
    • get on with activities you enjoy
    • live independently in your own home
    • delay or even eliminate the need for expensive health care or residential aged care.

    What if I walk a lot – is that enough?

    Walking may maintain some level of lower body muscle mass, but it won’t preserve your upper body muscles.

    If you find it difficult to get out of a chair, or can only walk short distances without getting out of breath, resistance training is the best way to regain some of the independence and function you’ve lost.

    It’s even more important for women, as muscle mass and strength are typically lower in older women than men. And if you’ve been diagnosed with osteoporosis, which is more common in older women than men, resistance exercise snacking at home can improve your balance, strength, and bone mineral density. All of this reduces the risk of falls and fractures.

    You don’t need heavy weights or fancy equipment to benefit from resistance training.

    So, will you start exercise snacking today?
