Losing Sleep = Gaining Pounds

When it comes to gaining or losing fat, then generally people focus a lot more on movement than on sleep.

And movement is great! See for example: The Doctor Who Wants Us To Exercise Less, & Move More

But it’s nothing without adequate sleep, because…

You snooze, you lose (pounds)

Researchers (Dr. Marie-Pierre St-Onge et al.) pooled randomized trials and found that reducing nightly sleep by about 80 minutes for 6 weeks led to an increase in weight and increased sedentary behavior.

That lost sleep is bad for the health is not, in and of itself, breaking news. See for example: What Harm Can One Sleepless Night Do? and Why You Probably Need More Sleep ← yes, including if you are in the older age range; we bust that myth in the article too!*

*Unless you have a (rare!) mutated ADRB1 gene, which reduces that. But we also cover that in the article, and how to know whether you have it.

However, previous research mainly examined extreme sleep deprivation of around 4 hours per night for only a few days, whereas this study investigated the milder, chronic sleep loss that is much more common in everyday life.

What they did: 95 adults who normally slept 7–8 hours per night completed two 6-week phases; one with their usual sleep schedule, and another in which they delayed their bedtime by 90 minutes (in principle; in fact it was more like 78.4 minutes on average) while wearing wrist monitors to measure sleep and activity (which is where we got the 78.4 minutes figure from). Dr. St-Onge and her team then also tracked body weight, waist circumference, body composition, fasting appetite-related hormones, sleep duration, and physical activity.

The results were clear:

❝Body weight (0.45 kg [CI, 0.33 to 0.57 kg]), waist circumference (0.52 cm [CI, 0.25 to 0.79 cm]), and whole-body volume (0.56 L [CI, 0.19 to 0.93 L]) were increased with SR relative to AS. Leptin levels were elevated with SR versus AS (2.03 ng/mL [CI, 0.38 to 3.68 ng/mL]). Sedentary time was increased by 17.2 minutes (CI, 11.7 to 22.7 minutes) per day with SR versus AS.❞

AS = adequate sleep
SR = sleep restriction

In words: participants in general spent an average of 17 additional minutes per day being sedentary during the sleep-restriction phase, while men and postmenopausal women increased their sedentary time by nearly 30 minutes per day. To be clear, the increase in sedentary time wasn’t simply a case of participants being awake longer, as they were proportionally less active when they slept less.

This does mean that because prolonged sedentary behavior is associated with chronic disease, the increased inactivity (resultant from tiredness) itself may (and almost certainly does) contribute to the health risks famously linked with insufficient sleep.

Indeed, a previous study involving a subset of the same participants found that similar mild sleep restriction increased insulin resistance in women with elevated cardiometabolic risk, particularly postmenopausal women, and increased inflammatory cells in the hearts of both men and women with elevated heart risk.

    Our Verdict

    When comparing currants to grapes, we picked the currants.

    Why?

    First, a note on nomenclature: when we say “currants”, we are talking about actual currants, of the Ribes genus, and in this case (as per the image) red ones. We are not talking about “currants” that are secretly tiny grapes that also get called currants in the US. So, there are important botanical differences here, beyond how they have been cultivated; they are literally entirely different plants.

    So, about those differences…

    In terms of macros, currants have nearly 5x the fiber, while grapes are slightly higher in carbs. So there’s an easy choice here in terms of fiber and on the glycemic index front; currants win easily.

    In the category of vitamins, currants have more of vitamins B5, B9, C, and choline, while grapes have more of vitamins A, B1, B2, B3, B6, E, and K. So, a win for grapes in this round.

    When it comes to minerals, currants have more calcium, copper, iron, magnesium, phosphorus, potassium, selenium, and zinc, while grapes have more manganese. A win, therefore, for currants again this time.

    In terms of polyphenols, currants have a lot more in terms of total polyphenols, including (as a matter of interest) approximately 5x the resveratrol content compared to grapes—and that’s compared to black grapes, which are the “best” kind of grapes for such. Grapes really aren’t a very good source of resveratrol; people just really like the idea of red wine being a health food, so it has been talked up a lot and got a popular reputation despite its extreme paucity of nutritional value.

    In any case, adding up the sections makes for a clear overall win for currants, but by all means enjoy either or both; diversity is good!

  • An Addiction Expert’s Insights On Festive Drinking

    This is Dr. Christopher Kahler. He’s Professor of Behavioral and Social Sciences, Director of Alcohol and Addiction Studies, Professor of Psychiatry and Human Behavior, all at Brown University.

    What does he want us to know?

    It’s the trickiest time of the year

    Per stats, alcohol sales peak in December, with the heaviest drinking being from mid-December (getting an early start on the Christmas cheer) to New Year’s Eve. As for why, there’s a collection of reasons, as he notes:

    ❝The main challenge is there’s an extra layer of stress, with a lot of obligations and expectations from friends and family. We’re around people who maybe we’re not usually around, and in larger groups. It’s also a time of heightened emotion and, for some people, loneliness.

    On top of that, alcohol use is built into a lot of our winter holiday traditions. It’s often marketed as part of the “good life.” We’re expected to have alcohol when we celebrate.❞

    As for how much alcohol is safe to drink… According to the World Health Organization, the only safe amount of alcohol is zero:

    Dr. Kahler acknowledges, however, that many people will wish to imbibe anyway, and indeed, he himself does drink a little, but endeavours to do so mindfully, and as such, he recommends that we…

    HALT!

    Dr. Kahler counsels us against making decisions (including the decision to drink alcohol), on occasions when we are one or more of the following:

    • Hungry
    • Angry
    • Lonely
    • Tired

    He also notes that around this time of year, often our normal schedules and habits are disrupted, which introduces more microdecisions to our daily lives, which in turn means more “decision fatigue”, and the greater chance of making bad decisions.

    Set your intentions now

    He bids us figure out what our goal is, and really think it through, including not just “how many drinks to have” if we’re drinking, but also such things as “what feelings are likely to come up”. Because, if we’ve historically used alcohol as a maladaptive coping mechanism, we’re going to need a different, better, healthier coping mechanism (we talked more about that in our above-linked article about reducing or quitting alcohol, too, with some examples).

    He also suggests that we memorize our social responses—exactly what we’re going to say if offered a drink, for example:

    ❝It’s important to know what you’re going to say about your alcohol use. If someone asks if they can get you a drink, good responses could be: “A glass of water would be great” or “Do you have any non-alcoholic cider?” You don’t have to explain yourself. Just ask for what you want, because saying no to someone can be difficult.❞

    Menopause is something that very few people are adequately prepared for despite its predictability, and also something that very many people then neglect to take seriously enough.

    Dr. Shepherd encourages a more proactive approach throughout all stages of menopause and beyond; she discusses “the preseason, the main event, and the after-party” (perimenopause, menopause, and postmenopause), which is important, because typically people take up an interest in perimenopause, are treating it like a marathon by menopause, and when it comes to postmenopause, it’s easy to think “well, that’s behind me now”, and it’s not, because untreated menopause will continue to have (mostly deleterious) cumulative effects until death.

    As for HRT, there’s a chapter on that of course, going into quite some detail. There is also plenty of attention given to popular concerns such as managing weight changes and libido changes, as well as oft-neglected topics such as brain changes, as well as things considered more cosmetic but that can have a big impact on mental health, such as skin and hair.

    The style throughout is pop-science; friendly without skimping on detail and including plenty of good science.

    Bottom line: if you’d like a fairly comprehensive overview of the changes that occur from perimenopause all the way to menopause and well beyond, then this is a great book for that.

    First, what this book is mostly not about: bubble baths and scented candles. We say “mostly”, because stress management is an important aspect given worthy treatment in this book, but there is more emphasis on evidence-based interventions and thus Dr. Kumar is readier to prescribe nature walks and meditation, than product-based pampering sessions.

    As is made clear in the subtitle “Transforming Heart Health with Lifestyle Medicine”, the focus is on heart health throughout, but as 10almonds readers know, “what’s good for your heart is good for your brain” is a truism that indeed holds true here too.

    Dr. Kumar also gives nutritional tweaks to optimize heart health, and includes a selection of heart-healthy recipes, too. And exercise? Yes, customizable exercise plans, even. And a plan for getting sleep into order if perchance it has got a bit out of hand (most people get less sleep than necessary for maintenance of good health), and he even delves into “social prescribing”, that is to say, making sure that one’s social connectedness does not get neglected—without letting it, conversely, take over too much of one’s life (done badly, social connectedness can be a big source of unmanaged stress).

    Perhaps the most value of this book comes from its 10-week self-care plan (again, with a focus on heart health), basically taking the reader by the hand for long enough that, after those 10 weeks, habits should be quite well-ingrained.

    A strong idea throughout is that the things we take up should be sustainable, because well, a heart is for life, not just for a weekend retreat.

    Bottom line: if you’d like to improve your heart health in a way that feels like self-care rather than an undue amount of work, then this is the book for you.

    In an age when doctors are increasingly pressed to get you out of their office quickly and not take the time to do thorough tests, having a good basic knowledge of signs and symptoms of disease has become more important than ever for all of us:

    The eyes have it:

    Dr. Siobhan Deshauer is back, this time working with Dr. Maria Howard, a Canadian optometrist, who advised behind-the-scenes to ensure the best information about these signs and symptoms and what they tell us:

    1. Color blindness test: Ishihara color test identifies color blindness; in the version in the video, seeing “74” is normal, “12” indicates red-green color blindness, and no numbers suggest complete color blindness due to genetics or retinal/optic nerve issues.
    2. Yellow sclera (scleral icterus): yellow sclera indicates high bilirubin from excessive red blood cell breakdown, liver damage, bile duct blockage, or Gilbert syndrome.
    3. Blue sclera: indicates thin collagen in the sclera, which can be linked to osteogenesis imperfecta, Ehlers-Danlos syndrome, and Marfan syndrome.
    4. Pink eye: caused by infections, autoimmune diseases, or trauma; persistent symptoms or associated pain/vision changes need medical evaluation.
    5. Physiologic diplopia (double vision): normal test where fingers appear doubled when focusing on different planes; absence may indicate amblyopia.
    6. Pinhole test (visual acuity): looking through a small pinhole can determine if glasses are needed for clearer vision.
    7. Nearsighted vs farsighted: nearsightedness risks retinal tears and night vision issues, while farsightedness increases the risk of glaucoma.
    8. Eye color and health: brown eyes lower cancer risk but higher cataract risk; light eyes higher cancer risk but lower cataract risk; sudden changes may indicate a condition.
    9. Kayser-Fleischer rings: golden-brown rings around the iris suggest copper buildup from Wilson disease, treatable with chelation therapy.
    10. Corneal arcus: gray/white ring around the iris indicates cholesterol buildup, normal with aging but concerning in younger individuals, signaling hypercholesterolemia or artery narrowing.
    11. Limbal rings: dark rings around the iris are generally aesthetic and not health-related.
    12. Red desaturation test: a difference in red color perception between eyes may indicate optic nerve or retinal issues.
    13. Eye twitching: often linked to stress, sleep deprivation, or caffeine; persistent twitching or muscle involvement requires medical attention.
    14. Pupillary reflex: pupil constriction in light; abnormal responses suggest trauma, overdose, or poisoning.
    15. Cataracts: lens cloudiness due to age, UV exposure, smoking, diabetes, or prednisone; also occurs sometimes in youth due to conditions like diabetes.
    16. Yellow spots (pinguecula and pterygium): sun damage, wind, and dust exposure cause yellow spots; protect with sunglasses to prevent progression impacting vision.
    17. Dark spots in the eye: includes freckles, moles (nevi), and melanoma; changes require medical evaluation.
    18. Hypnotic induction profile: eye roll test assesses susceptibility to hypnosis.
    19. Floaters: normal clumps in the eye; sudden increases, flashes, or curtain-like effects may signal retinal detachment.
    20. Retinal detachment: caused by aging-related vitreous shrinkage; treated with lasers, gas bubbles, or retinal buckles.
    21. Macular degeneration (Amsler grid test): wavy, fuzzy lines or missing vision spots may indicate this condition.
    22. Giant cell arteritis: no, that’s not a typo: rather it is about blood vessel inflammation that can cause blindness; treated with prednisone, symptoms include headaches and vision changes.
    23. Near point of convergence: focus test to detect convergence issues common with excessive screen time.
    24. Blepharitis: eyelid inflammation causing itchiness, burning, or flaky skin; treated with hygiene, antibiotics, or tea tree oil.
    25. Proptosis (Graves’ disease): bulging eyes due to hyperthyroidism; treatable with medications, radiation, or surgery.
    26. Ptosis (droopy eyelids): indicates myasthenia gravis, temporarily improved with the ice pack test.
    27. Night vision issues: caused by retinal problems or high myopia, not typically vitamin A deficiency in developed countries.
    28. Dry eyes: caused by screen time, smoking, medications, or autoimmune diseases; managed with lubricating drops, reduced screen time, and adjustments.
    29. Watery eyes: caused by irritation or blocked tear ducts; treated with lubricating drops or surgery.
    30. Retinoblastoma: rare childhood cancer detectable through flash photography showing one white pupil; early detection enables treatment.

    The subtitle here claims “a non-diet approach”, but doesn’t everything, nowadays? Even books titled “The such-and-such Diet” tend to also assure us “it’s not actually a diet; it’s just a way of eating”, as if a diet is not—by definition—a way of eating. Usually what they want to communicate is that it’s not a restrictive diet, usually meaning not restrictive in quantity, or not restrictive in food type (rarely both).

    This book is about intuitive eating, which is about as non-restrictive as any dietary approach can be, since it doesn’t restrict food type at at all, and it doesn’t restrict quantity in advance—rather, we learn to pay closer attention to our full signals.

    No wait, we don’t. This time, it’s not about “full”, it’s about “satisfied”. This comes in two forms:

    1. A principle somewhat akin to the “eat until 80% full” idea
    2. A principle of ensuring the good is culinarily satisfying

    This latter is important, if we want to have a good relationship with eating, and it also helps reduce portion sizes, when we truly take the time to mindfully savor a tasty morsel, rather than wolf down a plate of mediocre food.

    The style is one that balance being encouraging with delivering science to back up that encouragement. This not only means encouragement to take up this dietary approach, but also, encouragement to let go of things like calorie-counting and BMI.

    The recipes arranged per meal type, and indeed include things not found in many healthy eating books, such as gyoza dumplings, gnocchi, wontons, and shortbread. The recipes are mostly not, by default, vegan, vegetarian, gluten-free, dairy-free, or such. So if you have your own food restriction(s), the number of usable recipes will be diminished, barring any substitutions you can make yourself.

    Bottom line: this is more about about how to go about intuitive eating, than it is a book with a lot of nutritional information (though there is some of that too). If you’d like to get going with intuitive eating, then this book can help.
