What’s Your Vascular Dementia Risk?

We often say that “what’s good for your heart is good for your brain”, and this is because the former feeds the latter, with oxygen and nutrients, and also clears away detritus like beta-amyloid (associated with Alzheimer’s) and alpha-synuclein (associated with Parkinson’s).

For this reason, there are many risk factors that apply equally cardiovascular disease (CVD), and neurodegenerative diseases like Alzheimer’s and other vascular dementias, as well as stroke risk.

The link between the two has also been studied; recently a team of scienists led by Dr. Anisa Dhana asked the question:

❝What is the association between cardiovascular health (CVH) and biomarkers of neurodegeneration, including neurofilament light chain and total tau?❞

To answer this, they looked at data from more than 10,000 Americans aged 65+; of these, they were able to get serum samples from 5,470 of them, and tested those samples for the biomarkers of neurodegeneration mentioned above.

They then tabulated the results with cardiovascular health scores based on the American Heart Association (AHA)’s “Life’s Simple 7” tool, and found, amongst other things:

  • 34.6% of participants carried the APOE e4 allele, a genetic risk factor for Alzheimer’s.
  • Higher CVH scores were associated with lower NfL levels, but not with t-tau concentrations.
  • APOE e4 carriers with high CVH had significantly lower NfL levels.
  • Race did not influence the CVH-NfL relationship.
  • Higher CVH was linked to a slower annual increase in NfL levels but did not affect t-tau changes.
  • Over 10 years, participants with the lowest CVH scores saw a 7.1% annual increase in NfL levels, while those with the highest CVH scores had a 5.2% annual increase.
  • Better CVH is linked to lower serum NfL levels, regardless of age, sex, or race.
  • CVH is particularly crucial for APOE e4 carriers

In other words: higher cardiovascular health meant lower markers of neurodegeneration, and this not only still held true for APOE e4 carriers, but also, the benefits actually even more pronounced in those participants.

You may be wondering: “but it said it helped with NfL levels, not t-tau concentrations?” And, indeed, it is so. But this means that the overall neurodegeneration risk is still inversely proportional to cardiovascular health; it just means it’s not a magical panacea and we must still do other things too.

Life’s Simple 7

We mentioned that they used the AHA’s “Life’s Simple 7” tool to assess cardiovascular health; it is indeed simple, but important. Here it is:

As the AHA notes,

❝Unfortunately, 99% of the U.S. adult population has at least one of seven cardiovascular health risks: tobacco use,
poor diet, physical inactivity, unhealthy weight, high blood pressure, high cholesterol or high blood glucose.❞

It then goes on to talk about the financial burden of this on employers, but this was taken from a workplace health resource, and we recognize the rest of it won’t be of pressing concern for most of our readers. In case you are interested though, here it is:

    If you’re thinking about buying plant-based foods, a trip to the supermarket can leave you bewildered.

    There are plant-based burgers, sausages and mince. The fridges are loaded with non-dairy milk, cheese and yoghurt. Then there are the tins of beans and packets of tofu.

    But how much is actually healthy?

    Our nutritional audit of more than 700 plant-based foods for sale in Australian supermarkets has just been published. We found some products are so high in salt or saturated fat, we’d struggle to call them “healthy”.

    We took (several) trips to the supermarket

    In 2022, we visited two of each of four major supermarket retailers across Melbourne to collect information on the available range of plant-based alternatives to meat and dairy products.

    We took pictures of the products and their nutrition labels.

    We then analysed the nutrition information on the packaging of more than 700 of these products. This included 236 meat substitutes, 169 legumes and pulses, 50 baked beans, 157 dairy milk substitutes, 52 cheese substitutes and 40 non-dairy yoghurts.

    Plant-based meats were surprisingly salty

    We found a wide range of plant-based meats for sale. So, it’s not surprising we found large variations in their nutrition content.

    Sodium, found in added salt and which contributes to high blood pressure, was our greatest concern.

    The sodium content varied from 1 milligram per 100 grams in products such as tofu, to 2,000mg per 100g in items such as plant-based mince products.

    This means we could eat our entire daily recommended sodium intake in just one bowl of plant-based mince.

    An audit of 66 plant-based meat products in Australian supermarkets conducted in 2014 found sodium ranged from 316mg in legume-based products to 640mg in tofu products, per 100g. In a 2019 audit of 137 products, the range was up to 1,200mg per 100g.

    In other words, the results of our audit seems to show a consistent trend of plant-based meats getting saltier.

    What about plant-based milks?

    Some 70% of the plant-based milks we audited were fortified with calcium, a nutrient important for bone health.

    This is good news as a 2019-2020 audit of 115 plant-based milks from Melbourne and Sydney found only 43% of plant-based milks were fortified with calcium.

    Of the fortified milks in our audit, almost three-quarters (73%) contained the recommended amount of calcium – at least 100mg per 100mL.

    We also looked at the saturated fat content of plant-based milks.

    Coconut-based milks had on average up to six times higher saturated fat content than almond, oat or soy milks.

    Previous audits also found coconut-based milks were much higher in saturated fat than all other categories of milks.

    A first look at cheese and yoghurt alternatives

    Our audit is the first study to identify the range of cheese and yoghurt alternatives available in Australian supermarkets.

    Calcium was only labelled on a third of plant-based yoghurts, and only 20% of supermarket options met the recommended 100mg of calcium per 100g.

    For plant-based cheeses, most (92%) were not fortified with calcium. Their sodium content varied from 390mg to 1,400mg per 100g, and saturated fat ranged from 0g to 28g per 100g.

    So, what should we consider when shopping?

    As a general principle, try to choose whole plant foods, such as unprocessed legumes, beans or tofu. These foods are packed with vitamins and minerals. They’re also high in dietary fibre, which is good for your gut health and keeps you fuller for longer.

    If opting for a processed plant-based food, here are five tips for choosing a healthier option.

    1. Watch the sodium

    Plant-based meat alternatives can be high in sodium, so look for products that have around 150-250mg sodium per 100g.

    2. Pick canned beans and legumes

    Canned chickpeas, lentils and beans can be healthy and low-cost additions to many meals. Where you can, choose canned varieties with no added salt, especially when buying baked beans.

    3. Add herbs and spices to your tofu

    Tofu can be a great alternative to meat. Check the label and pick the option with the highest calcium content. We found flavoured tofu was higher in salt and sugar content than minimally processed tofu. So it’s best to pick an unflavoured option and add your own flavours with spices and herbs.

    4. Check the calcium

    When choosing a non-dairy alternative to milk, such as those made from soy, oat, or rice, check it is fortified with calcium. A good alternative to traditional dairy will have at least 100mg of calcium per 100g.

    5. Watch for saturated fat

    If looking for a lower saturated fat option, almond, soy, rice and oat varieties of milk and yoghurt alternatives have much lower saturated fat content than coconut options. Pick those with less than 3g per 100g.The Conversation

    We see a lot of books that exhort us to get a six-pack in a month, change our life in 7 days, learn Japanese in 24 hours. The reality is, things take time!

    Brett Blumenthal is more realistic while being just as motivational:

  • Cottage Cheese vs Ricotta – Which is Healthier?

    Our Verdict

    When comparing cottage cheese to ricotta, we picked the ricotta.

    Why?

    Cottage cheese is a famous health food, mostly for being a low-fat, low-carb, source of protein. And yet, ricotta beats it in most respects.

    Looking at the macros first, cottage cheese has more carbs, while ricotta has more protein and fat. The fat profile is pretty much the same, and in both cases it’s two thirds saturated fat, which isn’t good in either case, but cottage cheese has less overall fat which means less saturated fat in total even if the percentage is the same. Because the difference in carbs and protein is not large, while ricotta has considerably more fat, we’ll call this category a win for cottage cheese.

    In terms of vitamins, cottage cheese has more of vitamins B1, B5, and B12, while ricotta has more of vitamins A, B2, B3, B9, D, E, and K, so this one’s a win for ricotta.

    In the category of minerals, cottage cheese has slightly more copper, while ricotta has much more calcium, iron, magnesium, manganese, potassium, selenium, and zinc. In particular, 2.5x more calcium, and 5x more iron! An easy and clear win for ricotta here.

    Taking everything into account: yes, cottage cheese has less fat (and thus, in total, less saturated fat, although the percentage is the same), but that doesn’t make up for ricotta winning in pretty much every other respect. Still, enjoy either or both (in moderation!) if you be so inclined.

    Our Verdict

    When comparing sweet cinnamon to regular cinnamon, we picked the sweet.

    Why?

    In this case, it’s not close. One of them is health-giving and the other is poisonous (but still widely sold in supermarkets, especially in the US and Canada, because it is cheaper).

    It’s worth noting that “regular cinnamon” is a bit of a misnomer, since sweet cinnamon is also called “true cinnamon”. The other cinnamon’s name is formally “cassia cinnamon”, but marketers don’t tend to call it that, preferring to calling it simply “cinnamon” and hope consumers won’t ask questions about what kind, because it’s cheaper.

    Note: this too is especially true in the US and Canada, where for whatever reason sweet cinnamon seems to be more difficult to obtain than in the rest of the world.

    In short, both cinnamons contain cinnamaldehyde and coumarin, but:

    • Sweet/True cinnamon contains only trace amounts of coumarin
    • Regular/Cassia cinnamon contains about 250x more coumarin

    Coumarin is heptatotoxic, meaning it poisons the liver, and the recommended safe amount is 0.1mg/kg, so it’s easy to go over that with just a couple of teaspoons of cassia cinnamon.

    You might be wondering: how can they get away with selling something that poisons the liver? In which case, see also: the alcohol aisle. Selling toxic things is very common; it just gets normalized a lot.

    Cinnamaldehyde is responsible for cinnamon’s healthier properties, and is found in reasonable amounts in both cinnamons. There is about 50% more of it in the regular/cassia than in the sweet/true, but that doesn’t come close to offsetting the potential harm of its higher coumarin content.

    Well, it certainly sounds wonderful! Granted, it may depend on what you do in bed :p

    Descartes did a lot of his work from bed (and also a surprising amount of it while hiding in an oven, but that’s another story), which was probably not so good for the health.

    As for Darwin, his health was terrible in quite a lot of ways, so he may not be a great model.
