In an age when doctors are increasingly pressed to get you out of their office quickly and not take the time to do thorough tests, having a good basic knowledge of signs and symptoms of disease has become more important than ever for all of us:

The eyes have it:

Dr. Siobhan Deshauer is back, this time working with Dr. Maria Howard, a Canadian optometrist, who advised behind-the-scenes to ensure the best information about these signs and symptoms and what they tell us:

  1. Color blindness test: Ishihara color test identifies color blindness; in the version in the video, seeing “74” is normal, “12” indicates red-green color blindness, and no numbers suggest complete color blindness due to genetics or retinal/optic nerve issues.
  2. Yellow sclera (scleral icterus): yellow sclera indicates high bilirubin from excessive red blood cell breakdown, liver damage, bile duct blockage, or Gilbert syndrome.
  3. Blue sclera: indicates thin collagen in the sclera, which can be linked to osteogenesis imperfecta, Ehlers-Danlos syndrome, and Marfan syndrome.
  4. Pink eye: caused by infections, autoimmune diseases, or trauma; persistent symptoms or associated pain/vision changes need medical evaluation.
  5. Physiologic diplopia (double vision): normal test where fingers appear doubled when focusing on different planes; absence may indicate amblyopia.
  6. Pinhole test (visual acuity): looking through a small pinhole can determine if glasses are needed for clearer vision.
  7. Nearsighted vs farsighted: nearsightedness risks retinal tears and night vision issues, while farsightedness increases the risk of glaucoma.
  8. Eye color and health: brown eyes lower cancer risk but higher cataract risk; light eyes higher cancer risk but lower cataract risk; sudden changes may indicate a condition.
  9. Kayser-Fleischer rings: golden-brown rings around the iris suggest copper buildup from Wilson disease, treatable with chelation therapy.
  10. Corneal arcus: gray/white ring around the iris indicates cholesterol buildup, normal with aging but concerning in younger individuals, signaling hypercholesterolemia or artery narrowing.
  11. Limbal rings: dark rings around the iris are generally aesthetic and not health-related.
  12. Red desaturation test: a difference in red color perception between eyes may indicate optic nerve or retinal issues.
  13. Eye twitching: often linked to stress, sleep deprivation, or caffeine; persistent twitching or muscle involvement requires medical attention.
  14. Pupillary reflex: pupil constriction in light; abnormal responses suggest trauma, overdose, or poisoning.
  15. Cataracts: lens cloudiness due to age, UV exposure, smoking, diabetes, or prednisone; also occurs sometimes in youth due to conditions like diabetes.
  16. Yellow spots (pinguecula and pterygium): sun damage, wind, and dust exposure cause yellow spots; protect with sunglasses to prevent progression impacting vision.
  17. Dark spots in the eye: includes freckles, moles (nevi), and melanoma; changes require medical evaluation.
  18. Hypnotic induction profile: eye roll test assesses susceptibility to hypnosis.
  19. Floaters: normal clumps in the eye; sudden increases, flashes, or curtain-like effects may signal retinal detachment.
  20. Retinal detachment: caused by aging-related vitreous shrinkage; treated with lasers, gas bubbles, or retinal buckles.
  21. Macular degeneration (Amsler grid test): wavy, fuzzy lines or missing vision spots may indicate this condition.
  22. Giant cell arteritis: no, that’s not a typo: rather it is about blood vessel inflammation that can cause blindness; treated with prednisone, symptoms include headaches and vision changes.
  23. Near point of convergence: focus test to detect convergence issues common with excessive screen time.
  24. Blepharitis: eyelid inflammation causing itchiness, burning, or flaky skin; treated with hygiene, antibiotics, or tea tree oil.
  25. Proptosis (Graves’ disease): bulging eyes due to hyperthyroidism; treatable with medications, radiation, or surgery.
  26. Ptosis (droopy eyelids): indicates myasthenia gravis, temporarily improved with the ice pack test.
  27. Night vision issues: caused by retinal problems or high myopia, not typically vitamin A deficiency in developed countries.
  28. Dry eyes: caused by screen time, smoking, medications, or autoimmune diseases; managed with lubricating drops, reduced screen time, and adjustments.
  29. Watery eyes: caused by irritation or blocked tear ducts; treated with lubricating drops or surgery.
  30. Retinoblastoma: rare childhood cancer detectable through flash photography showing one white pupil; early detection enables treatment.

    What’s this, a philosophy book in a health and productivity newsletter? Well, look at it this way: Aristotle basically wrote the “How To Win Friends And Influence People” of his day, and Plato before him wrote a book about management.

    In this (chiefly modern!) book, we see what the later Stoic philosophers had to say about getting the most out of life—which is also what we’re about, here at 10almonds!

    We tend to use the word “stoic” in modern English to refer to a person who is resolute in the face of hardship. The traditional meaning does encompass that, but also means a lot more: a whole, rounded, philosophy of life.

    Philosophy in general is not an easy thing into which to “dip one’s toe”. No matter where we try to start, it seems, it turns out there were a thousand other things we needed to read first!

    This book really gets around that. The format is:

    • There’s a theme for each month
    • Each month has one lesson per day
    • Each daily lesson starts with some words from a renowned stoic philosopher, and then provides commentary on such
    • The commentary provides a jumping-off point and serves as a prompt to actually, genuinely, reflect and apply the ideas.

    Unlike a lot of “a year of…” day-by-day books, this is not light reading, by the way, and you are getting a weighty tome for your money.

    But, the page-length daily lessons are indeed digestible—which, again, is what we like at 10almonds!

  • Could the shingles vaccine lower your risk of dementia?

    A recent study has suggested Shingrix, a relatively new vaccine given to protect older adults against shingles, may delay the onset of dementia.

    This might seem like a bizarre link, but actually, research has previously shown an older version of the shingles vaccine, Zostavax, reduced the risk of dementia.

    In this new study, published last week in the journal Nature Medicine, researchers from the United Kingdom found Shingrix delayed dementia onset by 17% compared with Zostavax.

    So how did the researchers work this out, and how could a shingles vaccine affect dementia risk?

    From Zostavax to Shingrix

    Shingles is a viral infection caused by the varicella-zoster virus. It causes painful rashes, and affects older people in particular.

    Previously, Zostavax was used to vaccinate against shingles. It was administered as a single shot and provided good protection for about five years.

    Shingrix has been developed based on a newer vaccine technology, and is thought to offer stronger and longer-lasting protection. Given in two doses, it’s now the preferred option for shingles vaccination in Australia and elsewhere.

    In November 2023, Shingrix replaced Zostavax on the National Immunisation Program, making it available for free to those at highest risk of complications from shingles. This includes all adults aged 65 and over, First Nations people aged 50 and older, and younger adults with certain medical conditions that affect their immune systems.

    What the study found

    Shingrix was approved by the US Food and Drugs Administration in October 2017. The researchers in the new study used the transition from Zostavax to Shingrix in the United States as an opportunity for research.

    They selected 103,837 people who received Zostavax (between October 2014 and September 2017) and compared them with 103,837 people who received Shingrix (between November 2017 and October 2020).

    By analysing data from electronic health records, they found people who received Shingrix had a 17% increase in “diagnosis-free time” during the follow-up period (up to six years after vaccination) compared with those who received Zostavax. This was equivalent to an average of 164 extra days without a dementia diagnosis.

    The researchers also compared the shingles vaccines to other vaccines: influenza, and a combined vaccine for tetanus, diphtheria and pertussis. Shingrix and Zostavax performed around 14–27% better in lowering the risk of a dementia diagnosis, with Shingrix associated with a greater improvement.

    The benefits of Shingrix in terms of dementia risk were significant for both sexes, but more pronounced for women. This is not entirely surprising, because we know women have a higher risk of developing dementia due to interplay of biological factors. These include being more sensitive to certain genetic mutations associated with dementia and hormonal differences.

    Why the link?

    The idea that vaccination against viral infection can lower the risk of dementia has been around for more than two decades. Associations have been observed between vaccines, such as those for diphtheria, tetanus, polio and influenza, and subsequent dementia risk.

    Research has shown Zostavax vaccination can reduce the risk of developing dementia by 20% compared with people who are unvaccinated.

    But it may not be that the vaccines themselves protect against dementia. Rather, it may be the resulting lack of viral infection creating this effect. Research indicates bacterial infections in the gut, as well as viral infections, are associated with a higher risk of dementia.

    Notably, untreated infections with herpes simplex (herpes) virus – closely related to the varicella-zoster virus that causes shingles – can significantly increase the risk of developing dementia. Research has also shown shingles increases the risk of a later dementia diagnosis.

    The mechanism is not entirely clear. But there are two potential pathways which may help us understand why infections could increase the risk of dementia.

    First, certain molecules are produced when a baby is developing in the womb to help with the body’s development. These molecules have the potential to cause inflammation and accelerate ageing, so the production of these molecules is silenced around birth. However, viral infections such as shingles can reactivate the production of these molecules in adult life which could hypothetically lead to dementia.

    Second, in Alzheimer’s disease, a specific protein called Amyloid-β go rogue and kill brain cells. Certain proteins produced by viruses such as COVID and bad gut bacteria have the potential to support Amyloid-β in its toxic form. In laboratory conditions, these proteins have been shown to accelerate the onset of dementia.

    What does this all mean?

    With an ageing population, the burden of dementia is only likely to become greater in the years to come. There’s a lot more we have to learn about the causes of the disease and what we can potentially do to prevent and treat it.

    This new study has some limitations. For example, time without a diagnosis doesn’t necessarily mean time without disease. Some people may have underlying disease with delayed diagnosis.

    This research indicates Shingrix could have a silent benefit, but it’s too early to suggest we can use antiviral vaccines to prevent dementia.

    Overall, we need more research exploring in greater detail how infections are linked with dementia. This will help us understand the root causes of dementia and design potential therapies.

  • Kiwi Fruit vs Pineapple – Which is Healthier?

    Our Verdict

    When comparing kiwi fruit to pineapple, we picked the kiwi.

    Why?

    In terms of macros, they’re mostly quite comparable, being fruits made of mostly water, and a similar carb count (slightly different proportions of sugar types, but nothing that throws out the end result, and the GI is low for both). Technically kiwi has twice the protein, but they are fruits and “twice the protein” means “0.5g difference per 100g”. Aside from that, and more meaningfully, kiwi also has twice the fiber.

    When it comes to vitamins, kiwi has more of vitamins A, B9, C, E, K, and choline, while pineapple has more of vitamins B1, B2, B3, B5, and B6. This would be a marginal (6:5) win for kiwi, but kiwi’s margins of difference are greater per vitamin, including 72x more vitamin E (with a cupful giving 29% of the RDA, vs a cupful of pineapple giving 0.4% of the RDA) and 57x more vitamin K (with a cupful giving a day’s RDA, vs a cupful of pineapple giving a little under 2% of the RDA). So, this is a fair win for kiwi.

    In the category of minerals, things are clear: kiwi has more calcium, copper, iron, magnesium, manganese, phosphorus, potassium, selenium, and zinc, while pineapple has more manganese. An overwhelming win for kiwi.

    Looking at their respective anti-inflammatory powers, pineapple has its special bromelain enzymes, which is a point in its favour, but when it comes to actual polyphenols, the two fruits are quite balanced, with kiwi’s flavonoids vs pineapple’s lignans.

    Adding up the sections, it’s a clear win for kiwi—but pineapple is a very respectable fruit too (especially because of its bromelain content), so do enjoy both!

    Updated COVID-19 vaccines are now available: They’re meant to give you the best protection against the strain of the virus that is making people severely sick and also causing deaths. 

    Many people were infected during the persistent summer wave, which may leave you wondering when you should get the updated vaccine. The short answer is that it depends on when you last got infected or vaccinated and on your particular level of risk. 

    We heard from six experts—including medical doctors and epidemiologists—about when they recommend getting an updated vaccine. Read on to learn what they said. And to make it easy, check out the flowchart below.

    If I was infected with COVID-19 this summer, when should I get the updated vaccine?

    All the experts we spoke to agreed that if you were infected this summer, you should wait at least three months since you were infected to get vaccinated. 

    “Generally, an infection may be protective for about three months,” Dr. Ziyad Al-Aly, chief of research and development at the Veterans Affairs St. Louis Health Care System, tells PGN. “If they got infected three or more months ago, it is a good idea to get vaccinated sooner than later.”

    This three-month rule applies if you got vaccinated over the summer, which may be the case for some immunocompromised people, adds Dr. Peter Chin-Hong, professor of medicine at the University of California, San Francisco. 

    If I didn’t get infected with COVID-19 this summer, when should I get vaccinated?

    Most of the experts we talked to say that if you didn’t get infected with COVID-19 this summer, you should get the vaccine as soon as possible. Dr. Peter Hotez, dean of the National School of Tropical Medicine at Baylor College of Medicine, emphasizes that if this applies to you, you should get vaccinated as soon as possible, especially given the current COVID-19 surge.

    Al-Aly agrees. “Vaccine-derived immunity lasts for several months, and it should cover the winter season. Plus, the current vaccine is a KP.2-adapted vaccine, so it will work most optimally against KP.2 and related subvariants [such as] KP.3 that are circulating now,” Al-Aly says. “We don’t know when the virus will mutate to a variant that is not compatible with the KP.2 vaccine.” 

    Al-Aly adds that if you’d rather take the protection you can get right now, “It may make more sense to get vaccinated sooner than later.”

    This especially applies if you’re over 65 or immunocompromised and you haven’t received a COVID-19 vaccine in a year or more because, as Chin-Hong adds, “that is the group that is being hospitalized and disproportionately dying now.”

    Some experts—including epidemiologist Katelyn Jetelina, author of newsletter Your Local Epidemiologist—also say that if you’re younger than 65 and not immunocompromised, you can consider waiting and aiming to get vaccinated before Halloween to get the best protection in the winter, when we’re likely to experience another wave because of the colder weather, gathering indoors, and the holidays. 

    “I am more worried about the winter than the summer, so I would think of October (some time before Halloween) as the ‘Goldilocks moment’—not too early, not too late, but just right,” Chin-Hong adds. Time it “such that your antibodies peak during the winter when COVID-19 cases are expected to exceed what we are seeing this summer.”

    My children are starting school—should I get them vaccinated now? 

    According to most experts we spoke to, now is a good time to get your children vaccinated. 

    Jennifer Nuzzo, professor of epidemiology and director of the Pandemic Center at the Brown University School of Public Health, adds that “with COVID-19 infection levels as high as they are and increased exposures in school,” now is a particularly good time to get an updated vaccine if people haven’t gotten COVID-19 recently. 

    Additionally, respiratory viruses spike when kids are back in school, so “doing everything you can to reduce your child’s risk of infection can help protect families and communities,” says epidemiologist Jessica Malaty Rivera, science communications advisor at the de Beaumont Foundation.

    Our Verdict

    When comparing pineapple to passion fruit, we picked the passion fruit.

    Why?

    Both are certainly great, and both have won their respective previous comparisons! And this one’s close:

    In terms of macros, passion fruit has about 4x the protein, nearly 2x the carbs, and more than 7x the fiber. So, this one’s a clear and overwhelming win for passion fruit.

    Vitamins are quite close; pineapple has more of vitamins B1, B5, B6, B9, and C, while passion fruit has more of vitamins A, B2, B3, and choline. So, a 5:4 marginal win for pineapple.

    When it comes to minerals, pineapple has more calcium, copper, manganese, and zinc, while passion fruit has more iron, manganese, phosphorus, potassium, and selenium. Superficially, this would be a 5:5 tie, but looking at the numbers, passion fruit’s margins of difference are much greater, which means it gives the better overall mineral coverage, and thus wins the category.

    Looking at polyphenols, pineapple wins this category with its variety of lignans, while passion fruit has just secoisolariciresinol, of which pineapple has more anyway. Plus, not a polyphenol but doing much of the same job of same, pineapple has bromelain, which is unique to it. So pineapple wins on the phytochemicals reckoning.

    Adding up the sections and weighting them for importance (e.g. what a difference it makes to health) and statistical relevance (e.g. greater or smaller margins of difference) makes for a nominal passion fruit win, but like we say, both of these fruits are great, so enjoy both!

  • Cheeky diet soft drink getting you through the work day? Here’s what that may mean for your health

    Many people are drinking less sugary soft drink than in the past. This is a great win for public health, given the recognised risks of diets high in sugar-sweetened drinks.

    But over time, intake of diet soft drinks has grown. In fact, it’s so high that these products are now regularly detected in wastewater.

    So what does the research say about how your health is affected in the long term if you drink them often?

    What makes diet soft drinks sweet?

    The World Health Organization (WHO) advises people “reduce their daily intake of free sugars to less than 10% of their total energy intake. A further reduction to below 5% or roughly 25 grams (six teaspoons) per day would provide additional health benefits.”

    But most regular soft drinks contain a lot of sugar. A regular 335 millilitre can of original Coca-Cola contains at least seven teaspoons of added sugar.

    Diet soft drinks are designed to taste similar to regular soft drinks but without the sugar. Instead of sugar, diet soft drinks contain artificial or natural sweeteners. The artificial sweeteners include aspartame, saccharin and sucralose. The natural sweeteners include stevia and monk fruit extract, which come from plant sources.

    Many artificial sweeteners are much sweeter than sugar so less is needed to provide the same burst of sweetness.

    Diet soft drinks are marketed as healthier alternatives to regular soft drinks, particularly for people who want to reduce their sugar intake or manage their weight.

    But while surveys of Australian adults and adolescents show most people understand the benefits of reducing their sugar intake, they often aren’t as aware about how diet drinks may affect health more broadly.

    What does the research say about aspartame?

    The artificial sweeteners in soft drinks are considered safe for consumption by food authorities, including in the US and Australia. However, some researchers have raised concern about the long-term risks of consumption.

    People who drink diet soft drinks regularly and often are more likely to develop certain metabolic conditions (such as diabetes and heart disease) than those who don’t drink diet soft drinks.

    The link was found even after accounting for other dietary and lifestyle factors (such as physical activity).

    In 2023, the WHO announced reports had found aspartame – the main sweetener used in diet soft drinks – was “possibly carcinogenic to humans” (carcinogenic means cancer-causing).

    Importantly though, the report noted there is not enough current scientific evidence to be truly confident aspartame may increase the risk of cancer and emphasised it’s safe to consume occasionally.

    Will diet soft drinks help manage weight?

    Despite the word “diet” in the name, diet soft drinks are not strongly linked with weight management.

    In 2022, the WHO conducted a systematic review (where researchers look at all available evidence on a topic) on whether the use of artificial sweeteners is beneficial for weight management.

    Overall, the randomised controlled trials they looked at suggested slightly more weight loss in people who used artificial sweeteners.

    But the observational studies (where no intervention occurs and participants are monitored over time) found people who consume high amounts of artificial sweeteners tended to have an increased risk of higher body mass index and a 76% increased likelihood of having obesity.

    In other words, artificial sweeteners may not directly help manage weight over the long term. This resulted in the WHO advising artificial sweeteners should not be used to manage weight.

    Studies in animals have suggested consuming high levels of artificial sweeteners can signal to the brain it is being starved of fuel, which can lead to more eating. However, the evidence for this happening in humans is still unproven.

    What about inflammation and dental issues?

    There is some early evidence artificial sweeteners may irritate the lining of the digestive system, causing inflammation and increasing the likelihood of diarrhoea, constipation, bloating and other symptoms often associated with irritable bowel syndrome. However, this study noted more research is needed.

    High amounts of diet soft drinks have also been linked with liver disease, which is based on inflammation.

    The consumption of diet soft drinks is also associated with dental erosion.

    Many soft drinks contain phosphoric and citric acid, which can damage your tooth enamel and contribute to dental erosion.

    Moderation is key

    As with many aspects of nutrition, moderation is key with diet soft drinks.

    Drinking diet soft drinks occasionally is unlikely to harm your health, but frequent or excessive intake may increase health risks in the longer term.

    Plain water, infused water, sparkling water, herbal teas or milks remain the best options for hydration.
