On account of being an organ (or rather, a system of organs) whose functions are almost entirely autonomic, most of us don’t think about our gut much. We usually know there’s acid in the stomach, and we usually know there are “good and bad” gut bacteria. But what of the rest of what goes on?

For anyone who has a hazy half-remembered knowledge from school, this will serve as not only a reminder, but a distinct upgrade in knowledge.

Dr. Giuliua Enders talks us through not just the processes of what goes on, but, as a medical doctor, also many instances of what can go wrong, for example:

  • Why do some people’s bodies mistake nuts for a deadly threat (and consequently, accidentally elevate them to the status of actually becoming a deadly threat)?
  • Why are some people lactose-intolerant, and why do food intolerances often pop up later with age?
  • Why do constipation and diarrhoea happen?
  • Why is it that stress can cause stomach ulcers?

The style of writing is light and easy-reading, and the illustrations are clear too. This is a very accessible book that doesn’t assume prior knowledge, and also doesn’t skimp on the scientific explanations—there’s no dumbing down here.

Bottom line: knowing what goes on in our gut as akin to knowing what goes on under the hood of a car. A lot of the time we don’t need to know, but knowing can make a big difference from time to time, and that’s when you’ll wish you’d learned!

    The Sun Exposure Dilemma

    Yesterday, we asked you about your policy on sun exposure, and got the above-pictured, below-described, set of answers:

    • A little over a third of respondents chose “I recognize the risks, but I think the benefits outweigh them”
    • A quarter of respondents chose “I am a creature of the shadows and I avoid the sun at all costs‍”
    • A little over a fifth of respondents chose “I recognize the benefits, but I think the risks outweigh them”
    • A little under a fifth of respondents chose “I’m a sun-lover! Give me that vitamin D and other benefits!”

    All in all, this is perhaps the most even spread of answers we’ve had for Friday mythbuster polls—though the sample size was smaller than it often is.

    Of those who added comments, common themes were to mention your local climate, and the importance of sunscreen and/or taking vitamin D supplements.

    One subscriber mentioned having lupus and living in Florida, which is a particularly unfortunate combination:

    Another subscriber wrote:

    ❝Use a very good sunscreen with a high SPF all the time. Reapply after swimming or as needed! I also wear polarized sunglasses anytime I’m outside.❞

    …which are important things to note too, and a lot of people forget!

    But, onto today’s science for the topic at hand…

    We need to get plenty of sun to get plenty of vitamin D: True or False?

    True or False, depending on so many factors—to the point that many people get it wildly wrong in either direction.

    Whether we are getting enough vitamin D depends on many circumstances, including:

    • The climate (and depending on latitude, time of year) where we live
    • Our genes, and especially (but not only) our skintone
    • The clothes we wear (or don’t)
    • Our diet (and not just “how much vitamin D do we consume”)
    • Chronic diseases that affect vitamin D metabolism and/or requirements and/or sensitivity to the sun

    Notably, on the topic of whether you should stay in the sun for longer to get more vitamin D…

    ❝The body can only produce a certain amount of vitamin D at the time, so staying in the sun any longer than needed (which could be just a few minutes, in a sunny climate) is not going to help increase your vitamin D levels, while it will increase your risk of skin cancer.❞

    In contrast, she does also note:

    ❝During winter, catching enough sun can be difficult, especially if you spend your days confined indoors. Typically, the required exposure increases to two to three hours per week in winter. This is because sunlight exposure can only help produce vitamin D if the UVB rays reach us at the correct angle. So in winter we should regularly spend time outside in the middle of the day to get our dose of vitamin D.❞

    We can skip the sun and get our vitamin D from diet/supplements: True or False?

    True! However, vitamin D is not the only health benefit of sun exposure.

    Not only is sunlight-induced serotonin production important for many things ranging from mood to circadian rhythm (which in turn affects many other aspects of health), but also…

    While too much sun can cause skin cancer, too little sun could cause other kinds of cancer:

    Additionally, according to new research, the circadian rhythm benefits we mentioned above may also have an impact on type 2 diabetes:

    Which way to jump?

    A lot of it depends on who you are, ranging from the factors we mentioned earlier, to even such things as “having many moles” or “having blonde hair”.

    This latter item, blonde hair, is a dual thing: it’s a matter of genetic factors that align with being prone to being more sensitive to the sun, as well as being a lesser physical barrier to the sun’s rays than dark hair (that can block some UV rays).

    So for example, if two people have comparably gray hair now, but one of them used to have dark hair and the other blonde, there will still be a difference in how they suffer damage, or don’t—and yes, even if their skin is visually of the same approximate skintone.

    You probably already know for yourself whether you are more likely to burn or tan in the sun, and the former group are less resistant to the sun’s damage… But the latter group are more likely to spend longer in the sun, and accumulate more damage that way.

    …and skip to “At risk groups”, if you don’t want to read the whole thing; “Skin type” is also an important subsection, which also uses your hair and eye color as indicators.

    Writer’s note: genetics are complicated and not everyone will fall neatly into categories, which is why it’s important to know the individual factors.

    For example, I am quite light-skinned with slightly graying dark hair and gray-blue eyes, and/but also have an obscure Sámi gene that means my skin makes vitamin D easily, while simultaneously being unusually resistant to burning (I just tan). Basically: built for the midnight sun of the Arctic circle.

    And yet! My hobbies include not getting skin cancer, so I tend to still be quite mindful of UV levels in different weathers and times of day, and make choices (schedule, clothing, sunscreen or not) accordingly.

    Bottom line:

    That big self-perpetuating nuclear explosion in the sky is responsible for many things, good and bad for our health, so be aware of your own risk factors, especially for vitamin D deficiency, and skin cancer.

    • If you have a predisposition to both, that’s unfortunate, but diet and supplementation at least can help with the vitamin D while getting modest amounts of sun at most.
    • Remember that you can only make so much vitamin D at once, so sunbathing for health benefits need only take a few minutes
    • Remember that sunlight is important for our circadian rhythm, which is important for many things.
    • That’s governed by specific photoreceptor cells, though, so we don’t need our skin to be exposed for that; we just need to be able to see sunlight.
    • If you’re going to be out in the sun, and not covered up, sunscreen is your friend, and yes, that goes for clear cold days under the winter sun too.
    • Most phone weather apps these days have a UV index score as part of the data they give. Get used to checking it as often as you’d check for rain.

  • Overdone It? How To Speed Up Recovery After Exercise

    How To Speed Up Recovery After A Workout (According To Actual Science)

    Has your enthusiasm ever been greater than your ability, when it comes to exercise?

    Perhaps you leapt excitedly into a new kind of exercise, or maybe you made a reprise of something you used to do, and found out the hard way you’re not in the same condition you used to be?

    If you’ve ever done an exercise session and then spent the next three days recovering, this one’s for you. And if you’ve never done that? Well, prevention is better than cure!

    Post-exercise stretching probably won’t do much to help

    If you like to stretch after a workout, great, don’t let us stop you. Stretching is, generally speaking, good.

    But: don’t rely on it to hasten recovery. Here’s what scientists Afonso et al. had to say recently, after doing a big review of a lot of available data:

    ❝There wasn’t sufficient statistical evidence to reject the null hypothesis that stretching and passive recovery have equivalent influence on recovery.

    Data is scarce, heterogeneous, and confidence in cumulative evidence is very low. Future research should address the limitations highlighted in our review, to allow for more informed recommendations.

    For now, evidence-based recommendations on whether post-exercise stretching should be applied for the purposes of recovery should be avoided, as the (insufficient) data that is available does not support related claims.❞

    …and breath! What a title.

    Hot and Cold

    Contrast bath therapy (alternating hot and cold, which notwithstanding the name, can also be done in a shower) can help reduce muscle soreness after workout, because of how the change in temperature stimulates vasodilation and vasoconstriction, reducing inflammation while speeding up healing:

    If doing this in the shower isn’t practical for you, and you (like most people) have only one bathtub, then cold is the way to go for the most evidence-based benefits:

    Eat protein whenever, carbs after

    Eating protein before a workout can boost muscle protein synthesis. Be aware that even if you’re not bodybuilding, your body will still need to do cell replacement and repair, including in any muscle tissue that got damaged* during the workout

    If you don’t like eating before a workout, eating protein after is fine too:

    Pre- versus post-exercise protein intake has similar effects on muscular adaptations

    *Note: muscle tissue is supposed to get damaged (slightly!) during many kinds of workout.

    From lactic acid (that “burn” you feel when exercising) to microtears, the body’s post-workout job is to make the muscle stronger than before, and to do that, it needs you to have found the weak spots for it.

    That’s what exercise-to-exhaustion does.

    Eating carbs after a workout helps replace lost muscle glycogen.

    Alcohol is not the post-workout carb you want

    Shocking, right? But of course, it’s very common for casual sportspeople to hit the bar for a social drink after their activity of choice.

    However, consuming alcohol after exercise doesn’t merely fail to help, it actively inhibits glycogen replacement and protein synthesis:

    Also, if you’re tempted to take alcohol “to relax”, please be aware that alcohol only feels relaxing because of what it does to the brain; to the rest of the body, it is anything but, and also raises blood pressure and cortisol levels.

    As to what to drink instead…

    Hydrate, and consider creatine and tart cherry supplementation

    Hydration is a no-brainer, but when you’re dehydrated, it’s easy to forget!

    Creatine is a very well-studied supplement, that helps recovery from intense exercise:

    Tart cherry juice has been found to reduce muscle damage, soreness, and inflammation after exercise:

    Flossing Without Flossing?

    You almost certainly brush your teeth. You might use mouthwash. A lot of people floss for three weeks at a time, often in January.

    There are a lot of options for oral hygiene; variations of the above, and many alternatives too. This is a big topic, so rather than try to squeeze it all in one, this will be a several-part series.

    How important is flossing?

    Interdental cleaning is indeed pretty important, even though it may not have the heart health benefits that have been widely advertised:

    However! The health of our gums is very important in and of itself, especially as we get older:

    But! It helps to avoid periodontal (e.g. gum) disease, not dental caries:

    And! Most certainly it can help avoid a stack of other diseases:

    …so in short, if you’d like to have happy healthy teeth and gums, flossing is an important adjunct, and/but not a one-stop panacea.

    Is it better to floss before or after brushing?

    As you prefer. A team of scientists led by Dr. Claudia Silva studied this, and found that there was “no statistical difference between brush-floss and floss-brush”:

    Flossing is tedious. How do we floss without flossing?

    This is (mostly) about water-flossing! Which does for old-style floss what sonic toothbrushes to for old-style manual toothbrushes.

    If you’re unfamiliar, it means using a device that basically power-washes your teeth, but with a very narrow high-pressure jet of water.

    Do they work? Yes:

    As for how it stacks up against traditional flossing, Liang et al. found:

    ❝In our previous single-outcome analysis, we concluded that interdental brushes and water jet devices rank highest for reducing gingival inflammation while toothpick and flossing rank last.

    In this multioutcome Bayesian network meta-analysis with equal weight on gingival inflammation and bleeding-on-probing, the surface under the cumulative ranking curve was 0.87 for water jet devices and 0.85 for interdental brushes.

    Water jet devices and interdental brushes remained the two best devices across different sets of weightings for the gingival inflammation and bleeding-on-probing.

    Our Verdict

    When comparing chia to flax, we picked the chia.

    Why?

    Both are great! And it’s certainly close. Both are good sources of protein, fiber, and healthy fats.

    Flax seeds contain a little more fat (but it is healthy fat), while chia seeds contain a little more fiber.

    They’re both good sources of vitamins and minerals, but chia seeds contain more. In particular, chia seeds have about twice as much calcium and selenium, and notably more iron and phosphorous—though flax seeds do have more potassium.

    Of course the perfect solution is to enjoy both, but since for the purpose of this exercise we have to pick one, we’d say chia comes out on top—even if flax is not far behind.

    Our Verdict

    When comparing dates to prunes, we picked the prunes.

    Why?

    First let’s note: we’re listing the second fruit here as “prunes” rather than “plums”, since prunes are dehydrated plums, and it makes more sense to compare the dried fruit to dates which are invariably dried too. Otherwise, the water weight of plums would unfairly throw out the nutrient proportions per 100g (indeed, upon looking up numbers, dates would overwhelmingly beat plums easily in the category of pretty much every nutrient).

    So let’s look at the fairer comparison:

    In terms of macros, dates have a little more protein, carbohydrate, and fiber. This is because while both are dried, prunes are usually sold with more water remaining than dates; indeed, per 100g prunes still have 30g water weight to dates’ 20g water weight. This makes everything close, but we are going to call this category a nominal win for dates. Mind you, hydration is still good, but please do not rely on dried fruit for your hydration!

    When it comes to vitamins, dates have more of vitamins B5 and B9, while prunes have more of vitamins A, B2, B3, B6, C, E, K, and choline. A clear win for prunes here.

    In the category of minerals, it’s a similar story: dates have more iron, magnesium, and selenium, while prunes have more calcium, copper, manganese, phosphorus, potassium, and zinc. Another win for prunes.

    In short, enjoy either or both, but prunes win on overall nutritional density!
