How To Make Your Body Fat Heart-Healthier

It matters where and how fat is stored, and the good news is, you can influence that!

Where it goes

Firstly, there’s an important distinction between subcutaneous fat (the squishable stuff just underneath your skin) and visceral fat (you can’t squish this; it’s under your abdominal muscles, surrounding your organs).

Subcutaneous fat is good in moderation, with a fairly wide margin for error. The healthiest body fat percentages are (assuming normal hormones) generally considered to be in the range of 20–25% for women and 15–20% for men. Visceral fat is generally bad. We technically do need some, but almost everyone has either the right amount or too much, and its presence is very strongly associated with metabolic health problems, well beyond the kind of health risks that can be attributed to systemic failures in the healthcare system when it comes to those with merely more subcutaneous fat than most (see: Fat’s Real Barriers To Health). So whereas subcutaneous fat tends to get scapegoated a lot for largely unrelated things, excess visceral fat is genuinely an undeniable problem metabolically.

Recently (the paper was published two days ago, at time of writing) researchers (Dr. Vladimir Losev et al.) analysed UK Biobank data from 21,241 people, using whole body and heart imaging and AI to calculate a “heart age” compared with chronological age.

What they found: excess visceral fat around organs was linked to faster aging of the heart and blood vessels, even in people who appear fit and have a “healthy” BMI.

We put that “healthy” in quotation marks there, because BMI isn’t very reliable for anything, and in this study, BMI didn’t predict heart age well, showing that fat location is more important than overall weight. See also: When BMI Doesn’t Quite Measure Up

Why this happens: people think of fat as being “just there”, but in reality it’s metabolically active, releasing cytokines, hormones, and chemokines; visceral fat promotes insulin resistance, inflammation, and lipid problems, while subcutaneous fat differs developmentally and functionally

They also found: hormonally-driven sex differences, notably that women have less visceral fat (54% of men’s level) but more subcutaneous fat (38% higher), and as such:

  • men with “apple-shaped” fat distribution (belly fat) showed faster heart aging
  • women with “pear-shaped” fat (hips and thighs) had slower heart aging

… and, confirming that hypothesis further, higher estrogen levels were found to be protective against heart aging.

    But, what’s this about sharper tastes leading to a sharper brain?

    It’s about more than just antioxidants

    Polyphenols are well-known (to 10almonds readers, at least) as an excellent source of antioxidants. A good general rule of thumb for “does this food contain a lot of healthy polyphenols?” is: “is this (edible) plant bitter/astringent/pungent”?

    …which is why it’s no surprise that black coffee and bitter chocolate score highly, as do hot peppers and even garlic.

    Even fruits, generally considered something sweet to eat, often contain more polyphenols when they are bitter—many berries are great examples of this, for example: Goji Berries: Which Benefits Do They Really Have?

    Last year (at time of writing), it was hypothesized that the taste itself may actually make a difference (i.e., you won’t get the same benefits just from supplements without the taste, and you may get some benefits just from the taste alone):

    ❝As most of polyphenols have a bitter taste, migrate from the oral cavity to the gastrointestinal tract and interact with intestinal secretory cells, they likely regulate sugar metabolism or feeding via the T2Rs.

    On the other hand, if we consider the effects of astringent polyphenols on the circulatory system, metabolism and brain function, their effects have a great deal in common with the benefits of exercise. Astringency, a stressor, elicits a hormetic response in sympathetic nerve overactivity and is considered to have beneficial effects in moderate doses.

    Research on the bio-modulation of polyphenols with taste, which has not received much attention to date, may provide a solution to the polyphenol paradox, in which polyphenols exert bio-regulatory effects despite their extremely low bioavailability.❞

    Which brings us to the latest news…

    The same researchers (Dr. Naomi Osakabe et al.) have discovered there is indeed more to the story. Specifically, that the astringent taste of flavanols—plant polyphenols found in cocoa, red wine, and berries—can directly stimulate the brain and nervous system, improving alertness, memory, and stress regulation.

    This, Dr. Osakabe explains, is because the sensation of astringency itself acts as a neural signal: sensory nerves transmit this stimulus to the brain, activating corticotropin-releasing hormone (CRH) neurons in the hypothalamus and noradrenaline pathways in the locus coeruleus.

    Those are some big words, so let’s break down the process:

    1. Noradrenaline transmission from the locus coeruleus to the preoptic area (relevant parts of the brain) promotes wakefulness.
    2. Noradrenaline and dopamine activates the hippocampus and improves learning and memory.
    3. Noradrenaline signals to the brainstem and stimulates sympathetic nervous system activity, circulation, and metabolism.

    All in all, this makes for a very strong extra reason to make sure: Are You Getting The Right Kinds Of Flavonoids? ← flavonols are a subcategory of flavonoid, and they’re very important, and the source matters, and the research we shared today underlines how really, getting these things from your diet (including tea and coffee, as well as berries and other kinds of food) is much better than getting them in supplement form!

    What if I don’t like those tastes?

    If you do not have a medical condition that proscribes them (do check with your doctor if unsure), the best advice is to simply eat them anyway, and your tastes will adapt.

    It will also help if you avoid sweet foods (though this too is also a good general rule of thumb!), as this will move the balance of where your brain’s “set range” is for “good taste”.

    Bonus tip: dark chocolate (80%+ cocoa if possible, 95% if you can get it) and chilli peppers go great with each other. Here’s an example of a chilli chocolate product on Amazon; it’s 70% cocoa (which is not bad, but could be better). You might be able to get a higher percentage locally, especially if you ask your local chocolatière, or make it yourself!

  • Licorice, Digestion, & Hormones

    Let’s Take A Look At Licorice…

    Licorice, as a confectionary, is mostly sugar and is useless for medicinal purposes.

    Licorice (Glycyrrhiza sp., most often Glycyrrhiza glabra), in the form of either the root extract (which can be taken as a supplement, or used topically) or the whole root (which can be taken as a powder/capsule, or used to make tea), is a medicinal plant with a long history of use.

    How well-evidenced is it for its popular uses?

    Licorice for digestion

    In this case, it is more accurate to say that it combats indigestion, including acid reflux and ulcerative colitis:

    Network pharmacology mechanisms and experimental verification of licorice in the treatment of ulcerative colitis ← looking at the mechanism of action; ultimately they concluded that “licorice improves ulcerative colitis, which may be related to the activation of the Nrf2/PINK1 signaling pathway that regulates autophagy.“

    Licorice vs menopause symptoms

    This one, while a popular use, isn’t so clear. Here’s a study that examines the compounds in licorice (in this case, Glycyrrhiza uralensis) that interact with estrogen receptors, notes that the bioavailability is poor, and proposes, tests, and recommends a way to make it more bioavailable:

    On the other hand, it is established that it will lower serum testosterone levels, which may make it beneficial for menopause and/or PCOS:

    Licorice for men

    You may be wondering: what about for men? Well, the jury is out on whether it meaningfully reduces free testosterone levels:

    And finally, it may (notwithstanding its disputed effect on testosterone itself) be useful as a safer alternative to finasteride (an antiandrogen mostly commonly used to treat benign prostatic hyperplasia, also used to as a hair loss remedy), since it (like finasteride) modulates 5α-reductase activity (this enzyme converts testosterone to the more potent dihydrogen testosterone, DHT), without lowering sperm count:

    Licorice for the skin

    As well as its potentially estrogenic activity, its anti-inflammatory and antioxidant powers make it comparable to hydrocortisone cream for treating eczema, psoriasis, and other such skin conditions:

    Is it safe?

    It is “generally recognized as safe”, as the classification goes.

    However, consumed in excess it can cause/worsen hypertension, and other contraindications include if you’re on blood thinners, or have kidney problems.

  • Barley Malt Flour vs chickpea flour – Which is Healthier?

    Our Verdict

    When comparing barley malt flour to chickpea flour, we picked the chickpea.

    Why?

    First, some notes:

    About chickpea flour: this is also called besan flour, gram flour, and garbanzo bean flour; they are all literally the same thing by different names, and are all flour made from ground chickpeas.

    About barley malt flour: barley is a true grain, and does contain gluten. We’re not going to factor that into today’s decision, but if you are avoiding gluten, avoid barley. As for “malt”; malting grains means putting them in an environment (with appropriate temperature and humidity) that they can begin germination, and then drying them with hot air to stop the germination process from continuing, so that we still have grains to make flour out of, and not little green sprouting plants. It improves the nutritional qualities and, subjectively, the flavor.

    To avoid repetition, we’re just going to write “barley” instead of “barley malt” now, but it’s still malted.

    Now, let’s begin:

    Looking at the macros first, chickpea flour has 2x the protein and also more fiber, while barley flour has more carbs. An easy win for chickpea flour.

    In the category of vitamins, chickpea flour has more of vitamins A, B1, B5, B9, E, and K, while barley flour has more of vitamins B2, B3, B6, and C. A modest 6:4 victory for chickpea flour.

    When it comes to minerals, things are much more one-sided; chickpea flour has more calcium, copper, iron, magnesium, manganese, phosphorus, potassium, and zinc, while barley flour has more selenium. An overwhelming win for chickpea flour.

    Adding up these three wins for chickpea flour makes for a convincing story in favor of using that where reasonably possible as a flour! It has a slight nutty taste, so you might not want to use it in everything, but it is good for a lot of things.

    It’s not a narrative book, rather, it is a guide, a little in the style of “Lonely Planet”, with many “know before you go” tips, information about the best time to go, difficult level, alternative routes if you want to get most of the enjoyment while having an easier time of it (or, conversely, if you want to see some extra sights along the way), and what to expect at all points.

    Where the book really excels is in balancing inspiration with information. There are some books that make you imagine being in a place, but you’ll never actually go there. There are other books that are technical manuals but not very encouraging. This one does both; it provides the motivation and the “yes, you really can, here’s how” information that, between them, can actually get you packing and on your way.

    Bottom line: if you yearn for breathtaking views and time in the great outdoors, but aren’t sure where to start, this will give you an incredible menu to choose from, and give you the tools to go about doing it.

  • What is ‘double pneumonia’, the condition that’s put Pope Francis in hospital?

    Pope Francis has been in hospital for more than a week with what some media reports are now calling “double pneumonia”.

    The Vatican released a statement on Tuesday evening saying

    laboratory tests, chest X-rays, and the clinical condition of the Holy Father continue to present a complex picture.

    The 88-year-old Catholic leader has a long history of respiratory illness.

    So, what makes this bout of pneumonia – a severe lung infection – so “complex”? And how will it be treated?

    What is double pneumonia?

    Pneumonia is a serious infection that fills the lungs with liquid or pus and can make it difficult to breathe. People may also have chest pain, cough up green mucus and have a fever.

    “Double pneumonia” is not an official medical term. It may be being used to describe two different aspects of Pope Francis’s condition.

    1. A bilateral infection

    Pope Francis has pneumonia in both lungs. This is known as “bilateral pneumonia”.

    An infection in both lungs doesn’t necessarily mean it’s more severe, but location is important. It can make a difference which parts of the lung are affected.

    When just one part of the lung or one lung is affected, the person can continue to breathe using the other lung while their body fights the infection.

    However when both lungs are compromised, the person will be receiving very little oxygen.

    2. A polymicrobial infection

    The Vatican has also said the infection affecting Pope Francis’s lungs is “polymicrobial”.

    This means the infection is being caused by more than one kind of microorganism (or “pathogen”).

    So, the cause could be two (or more) different kinds of bacteria, or any combination of bacteria, virus and fungus. It’s vital to know what’s causing the infection to effectively treat it.

    How is it diagnosed?

    Usually, when someone presents with suspected pneumonia the hospital will sample their lungs with a sputum test or swab.

    They will often also undergo an X-ray, usually to confirm which parts of the lung are involved.

    Healthy lungs look “empty” on an X-ray, because they are filled with air. But pneumonia fills the lungs with fluid.

    This means it’s usually very easy to see where pneumonia is affecting them, because the infection shows up as solid white mass on the scan.

    How is it treated?

    The sputum or swab helps detect what is causing the infection and determine treatment. For example, a specific antibiotic will be used to target a certain bacterium.

    Usually this works well. But if the infection is polymicrobial, the normal treatment might not be effective.

    For example, the antibiotics may work on the bacteria. But if there’s also a virus – which can’t be treated with antibiotics – it may become the dominant pathogen driving the infection.

    As a result, the patient may initially respond well to medication and then begin deteriorating again.

    If the infection is caused by multiple bacteria, the patient might be given a broad-spectrum antibiotic rather than a single targeted drug.

    A viral infection is harder to treat, as the anti-viral drugs that are available aren’t very effective or targeted.

    In severe cases, a patient will also need to be in intensive care on a breathing machine because they can’t breathe alone. This helps make sure they receive enough oxygen while their body fights the infection.

    Who is most susceptible?

    It’s possible to recover, even from severe infections. However having pneumonia can damage the lungs, and this can make a repeat infection more likely.

    Most people will never have a severe infection from these same pathogens. They may only experience a minor cold or flu, because their immune system can adequately fight the infection.

    However, certain groups are much more vulnerable to developing a serious case of pneumonia.

    Risk factors include:

    • age: babies under two, whose immune systems are still developing, and adults over 65, who tend to have weakened immune systems
    • lung damage: previous infections can cause scarring
    • lung disease: for example, if you have emphysema or chronic obstructive pulmonary disease
    • being a smoker
    • immunosuppression: if your immune system is weakened, for example by medication you take after a transplant or during cancer treatment.

    Pope Francis has a number of these risk factors. The pontiff is 88 years old and has a history of respiratory illness.

    He also had pleurisy (a condition that inflames the lungs) as a young adult. As a result, he had part of one lung removed, making him susceptible to lung infections.

    On Tuesday, the Vatican said Pope Francis remains “in good spirits” while he receives medical care and is grateful for the support he has received.

    Evidence shows that people in ancient times typically had straight teeth set well into strong jaws, with even wisdom teeth fitting properly.

    So, what went wrong? Did evolution do us a disservice?

    Some information to chew on

    Transition from hard-to-chew diets to processed, refined foods over millennia has reduced jaw size while tooth size stayed constant. Smaller jaws lead to tooth crowding, crookedness, and impacted wisdom teeth, requiring braces or extractions in modern times.

    However, all is not lost!

    Studies on non-human animals show softer diets reduce jaw and facial growth, causing dental crowding. In other words: dental crowding is primarily attributed to dietary and lifestyle changes, though genetics may play a role.

    And notably, when it comes to humans, populations with less processed diets experience fewer dental problems, suggesting lifestyle modifications could help prevent tooth crowding.

    And no, it is not too late. Remember, you are rebuilding your body all the time, including your bones!
