Yesterday’s book review (Counterclockwise) was about psychological factors affecting physical aging (progression or reversal thereof); today we have a book about the physiological factors affecting physical aging (progression or reversal thereof).

Dr. Levine is first and foremost a gerontological epigeneticist, and a lot of this book touches on her research in that field and that of her colleagues.

She does also discuss direct environmental factors also though, and—as you might well expect—lifestyle factors.

Regular readers of 10almonds are unlikely to gain anything new in the category of lifestyle matters, but a lot of the other material will be enlightening, especially with regard to the things that might at first glance seem set in stone, but we can in fact modify, and thus “choose our own adventure” when it comes to how the rest of our life plays out, healthwise (so: choose wisely!).

The book is mostly an overview on the (at time of writing: 2022) current state of affairs in the world of longevity research, and although it’s not a “how to” manual, there is plenty in the category of practical takeaways to be gleaned too.

The style is is mostly light pop science, but with a lot of hard science woven in—she is a good explainer, and has clearly made a notable effort to explain complex concepts in simple ways, while still delivering the complex concepts too (i.e. not overly “dumbing down”).

Bottom line: if you’d like to know about what can be done to increase your healthspan and general longevity, this book has a lot of answers!

    Key Triggers for Tinnitus

    Several everyday habits can make your tinnitus louder. Caffeine and nicotine increase blood pressure, restricting blood flow to the cochlea and worsening tinnitus. Common medications, such as pain relievers, high-dose antibiotics, and antidepressants, can also exacerbate tinnitus, especially with higher or long-term dosages.

    Impact of Diet and Sleep

    Dietary choices significantly impact tinnitus. Alcohol and salt alter the fluid balance in the cochlea, increasing tinnitus perception. Alcohol changes blood flow patterns and neurotransmitter production, while high salt intake has similar effects. Poor sleep quality elevates stress levels, making it harder to ignore tinnitus signals. Addressing sleep disorders like sleep apnea and insomnia can help manage tinnitus symptoms.

    Importance of Treating Hearing Loss

    Untreated hearing loss worsens tinnitus. Nearly 90% of individuals with tinnitus have some hearing loss. Hearing aids can reduce tinnitus perception by restoring missing sounds and reducing the brain’s internal compensatory signals. Combining hearing aids with sound therapy is said to provide even greater relief.

    Our Verdict

    When comparing peas to green beans, we picked the peas.

    Why?

    Looking at macros first, peas have nearly 6x the protein, nearly 2x the fiber, and nearly 2x the carbs, making them the “more food per food” choice.

    In terms of vitamins, peas have more of vitamins A, B1, B2, B3, B5, B6, B7, B9, C, and choline, while green beans have more of vitamins E and K. An easy win for peas.

    In the category of minerals, peas have more copper, iron, magnesium, manganese, phosphorus, potassium, selenium, and zinc, while green beans have more calcium. Another overwhelming win for peas.

    In short, enjoy both (diversity is good), but there’s a clear winner here and it’s peas.

    Why we eat too much (and how we can fix that)

    This is Dr. Andrew Jenkinson. He’s a Consultant Surgeon specializing in the treatment of obesity, gallstones, hernias, heartburn and abdominal pain. He runs regular clinics in both London and Dubai. What he has to offer us today, though, is insight as to what’s on our table that puts us on his table, and how we can quite easily change that up.

    So, why do we eat too much?

    First things first: some metabolic calculations. No, we’re not going to require you to grab a calculator here… Your body does it for you!

    Our body’s amazing homeostatic system (the system that does its best to keep us in the “Goldilocks Zone” of all our bodily systems; not too hot or too cold, not dehydrated or overhydrated, not hyperglycemic or hypoglycemic, blood pressure not too high or too low, etc, etc) keeps track of our metabolic input and output.

    What this means: if we increase or decrease our caloric consumption, our body will do its best to increase or decrease our metabolism accordingly:

    • If we don’t give it enough energy, it will try to conserve energy (first by slowing our activities; eventually by shutting down organs in a last-ditch attempt to save the rest of us)
    • If we give it too much energy, it will try to burn it off, and what it can’t burn, it will store

    In short: if we eat 10% or 20% more or less than usual, our body will try to use 10% to 20% more or less than usual, accordingly.

    So… How does this get out of balance?

    The problem is in how our system does that, and how we inadvertently trick it, to our detriment.

    For a system to function, it needs at its most base level two things—a sensor and a switch:

    • A sensor: to know what’s going on
    • A switch: to change what it’s doing accordingly

    Now, if we eat the way we’re evolved to—as hunter-gatherers, eating mostly fruit and vegetables, supplemented by animal products when we can get them—then our body knows exactly what it’s eating, and how to respond accordingly.

    Furthermore, that kind of food takes some eating! Most fruit these days is mostly water and fiber; in those days it often had denser fiber (before agricultural science made things easier to eat), but either way, our body knows when we are eating fruit and how to handle that. Vegetables, similarly. Unprocessed animal products, again, the gut goes “we know what this is” and responds accordingly.

    But modern ultra-processed foods with trans-fatty acids, processed sugar and flour?

    These foods zip calories straight into our bloodstream like greased lightning. We get them so quickly so easily and in such great caloric density, that our body doesn’t have the chance to count them on the way in!

    What this means is: the body has no idea what it’s just consumed or how much or what to do with it, and doesn’t adjust our metabolism accordingly.

    Bottom line:

    Evolutionarily speaking, your body has no idea what ultra-processed food is. If you skip it and go for whole foods, you can, within the bounds of reason, eat what you like and your body will handle it by adjusting your metabolism accordingly.

    Now, advising you “avoid ultra-processed foods and eat whole foods” was probably not a revelation in and of itself.

    But: sometimes knowing a little more about the “why” makes the difference when it comes to motivation.

    Vitamin D is essential for bone health, immune function and overall wellbeing. And it becomes even more crucial as we age.

    New guidelines from the international Endocrine Society recommend people aged 75 and over should consider taking vitamin D supplements.

    But why is vitamin D so important for older adults? And how much should they take?

    Young people get most vitamin D from the sun

    In Australia, it is possible for most people under 75 to get enough vitamin D from the sun throughout the year. For those who live in the top half of Australia – and for all of us during summer – we only need to have skin exposed to the sun for a few minutes on most days.

    The body can only produce a certain amount of vitamin D at a time. So staying in the sun any longer than needed is not going to help increase your vitamin D levels, while it will increase your risk of skin cancer.

    But it’s difficult for people aged over 75 to get enough vitamin D from a few minutes of sunshine, so the Endocrine Society recommends people get 800 IU (international units) of vitamin D a day from food or supplements.

    Why you need more as you age

    This is higher than the recommendation for younger adults, reflecting the increased needs and reduced ability of older bodies to produce and absorb vitamin D.

    Overall, older adults also tend to have less exposure to sunlight, which is the primary source of natural vitamin D production. Older adults may spend more time indoors and wear more clothing when outdoors.

    As we age, our skin also becomes less efficient at synthesising vitamin D from sunlight.

    The kidneys and the liver, which help convert vitamin D into its active form, also lose some of their efficiency with age. This makes it harder for the body to maintain adequate levels of the vitamin.

    All of this combined means older adults need more vitamin D.

    Deficiency is common in older adults

    Despite their higher needs for vitamin D, people over 75 may not get enough of it.

    Studies have shown one in five older adults in Australia have vitamin D deficiency.

    In higher-latitude parts of the world, such as the United Kingdom, almost half don’t reach sufficient levels.

    This increased risk of deficiency is partly due to lifestyle factors, such as spending less time outdoors and insufficient dietary intakes of vitamin D.

    It’s difficult to get enough vitamin D from food alone. Oily fish, eggs and some mushrooms are good sources of vitamin D, but few other foods contain much of the vitamin. While foods can be fortified with the vitamin D (margarine, some milk and cereals), these may not be readily available or be consumed in sufficient amounts to make a difference.

    In some countries such as the United States, most of the dietary vitamin D comes from fortified products. However, in Australia, dietary intakes of vitamin D are typically very low because only a few foods are fortified with it.

    Why vitamin D is so important as we age

    Vitamin D helps the body absorb calcium, which is essential for maintaining bone density and strength. As we age, our bones become more fragile, increasing the risk of fractures and conditions like osteoporosis.

    Keeping bones healthy is crucial. Studies have shown older people hospitalised with hip fractures are 3.5 times more likely to die in the next 12 months compared to people who aren’t injured.

    Vitamin D may also help lower the risk of respiratory infections, which can be more serious in this age group.

    There is also emerging evidence for other potential benefits, including better brain health. However, this requires more research.

    According to the society’s systematic review, which summarises evidence from randomised controlled trials of vitamin D supplementation in humans, there is moderate evidence to suggest vitamin D supplementation can lower the risk of premature death.

    The society estimates supplements can prevent six deaths per 1,000 people. When considering the uncertainty in the available evidence, the actual number could range from as many as 11 fewer deaths to no benefit at all.

    Should we get our vitamin D levels tested?

    The Endocrine Society’s guidelines suggest routine blood tests to measure vitamin D levels are not necessary for most healthy people over 75.

    There is no clear evidence that regular testing provides significant benefits, unless the person has a specific medical condition that affects vitamin D metabolism, such as kidney disease or certain bone disorders.

    Routine testing can also be expensive and inconvenient.

    In most cases, the recommended approach to over-75s is to consider a daily supplement, without the need for testing.

    You can also try to boost your vitamin D by adding fortified foods to your diet, which might lower the dose you need from supplementation.

    Even if you’re getting a few minutes of sunlight a day, a daily vitamin D is still recommended.

    Our Verdict

    When comparing trout to carp, we picked the trout.

    Why?

    Both have their strong points!

    In terms of macros, trout has slightly more protein and fat, and/but also has less cholesterol than carp. So, we pick the trout in the macros category.

    In the category of vitamins, trout has much more of vitamins A, B1, B2, B3, B5, B6, B7, B12, C, D, E, K, and choline, while carp has slightly more vitamin B9. In other words, an easy win for trout here.

    When it comes to minerals, however, trout has more potassium and selenium, while carp has more calcium, copper, iron, magnesium, manganese, phosphorus, and zinc. A fair win for carp this time.

    You may be wondering about heavy metals: this will vary depending on location, as well as the age of the fish (younger fish have had less time to accumulate heavy metals than old ones, so if you’re visiting the fishmonger, choose the smaller ones) and the lives they have led (e.g. wild vs farmed), however, as a general rule of thumb, trout will generally have lower heavy metals levels than carp, all other things (e.g. location, age, etc) being equal.

    In short, enjoy either or both in moderation, but trout wins on 3/4 categories today.

    When it comes to dealing with hunger, a common-sense way of dealing with it is “eat something”. However, many people find that they then eat the wrong things, in the wrong quantities, and end up in a cycle of overeating and being hungry.

    If this gets to the extreme, it can turn into a full-blown eating disorder:

    …and even in more moderate presentations, the cycle of hunger and overeating is not great for the health. So, how to avoid that?

    Listen to your body (but: actually listen)

    Your body says: we’re running a little low on glycogen reserves so our energy’s going to start suffering in a few hours if we don’t eat some fruit, kill something and eat its fatty organs, or perhaps find some oily nuts.

    You hear: eat something bright and sugary, shout at the dog, eat some fried food, got it!

    Your body says: our water balance is a little off, we could do with some sodium, potassium, and perhaps some phosphorus to correct it.

    You hear: eat something salty, got it, potato chips coming right up!

    …and so on. Now, we know 10almonds readers are quite a health-conscious readership, so perhaps your responses are not quite like that. But the take-away point is still important: we need to listen to the whole message, and give the body what it actually needs, not what will just shut the message off the most quickly.

    About those cravings…

    As illustrated a little above, a lot of cravings might not be what they first appear, and in evolutionary terms, our body is centuries behind industrialization, in terms of adaptations, which means that even if we try to take the above into account, our responses can sometimes be inappropriate in the age of supermarkets.

    Natural appetite suppressants

    Eating more is not always the answer, not even if it’s more healthy food. And hunger pangs can be especially inconvenient if, for example, we are fasting at present, which is by the way a very healthful thing for most people:

    One way to suppress hunger is simply to trigger the stomach into sending “full” signals, which involves filling it. Since you do not want to overeat, the trick here is imply to use high-volume food.

    Consider for example: 30 grapes and 30 raisins have approximately the same calorie count (what with raisins being dried grapes, and the calories didn’t evaporate), but the bowl of fresh fruit is going to physically fill your stomach a lot more quickly than the tiny amount of dried fruit.

    Protein is of course also an appetite suppressant, but it takes about 20 minutes for the signal to kick in. So a “hack” here is to snack on something proteinous at least 15 minutes before your main meal (for example, a portion of nuts while cooking, unless you’re allergic, or some dried fish unless you’re vegetarian/vegan; you get it, pick something high in protein and good for snacking, and have a small portion before your main meal).

    Nor is protein the only option!

    Scale it down

    Related to the above, there is a feedback loop that occurs here. The more you eat, the more your stomach slowly grows to accommodate it; the less you eat, the more your stomach slowly shrinks because the body tries hard to be an efficient organism, and will not maintain something that isn’t being used.

    So, there’s a bit of a catch-22; sate your hunger by filling your stomach with high volume foods, but filling it will cause it to grow?

    The trick is: do the “eat until 80% full” thing. That’s full enough that you have had a nice meal and are not suffering, without stretching the stomach.

    Enjoy your food

    Seriously! Actually enjoy it. Which means paying full attention to it. Eating can and should be a wonderful experience, so it’s best savored rather than inhaling a bowl of something in 30 seconds.

    Have you seen those dog bowls that have obstructions to slow down how quickly a dog eats? We can leverage that kind of trick too! While you might not want to eat from a dog bowl, how about having a little bowl of pistachio nuts rather than ready-to-eat peanuts? Or any shelled nuts that we must shell as we go. If you’re allergic to nuts, there are plenty of other foods with a high work-to-food ratio. Take some time and enjoy that pomegranate, for instance!

    Not that we necessarily have to make things difficult for ourselves either; we can just take appropriate care to ensure a good dining experience. Life is for living, so why not enjoy it?
