The Vagus Nerve: The Brain-Gut Highway
The longest cranial nerve is the vagus nerve; it runs all the way from your brain to your colon. It’s very important, and (amongst other tasks) it largely regulates your parasympathetic nervous system, and autonomous functions like:
- Breathing
- Heart rate
- Vasodilation & vasoconstriction
- Blood pressure
- Reflex actions (e.g. coughing, sneezing, swallowing, vomiting, hiccuping)
That’s great, but how does knowing about it help us?
Because of vagal maneuvers! This means taking an action to stimulate the vagus nerve, and prompt it to calm down various bodily functions that need calming down. This can take the form of:
- Massage
- Electrostimulation
- Diaphragmatic breathing
Massage is perhaps the simplest; “vagus” means “wandering”, and the nerve is accessible in various places, including behind the ears. That’s the kind of thing that’ easier to show than tell, though, so we’ll include a video at the end.
Electrostimulation is the fanciest, and has been used to treat migraines and cluster headaches. Check out, for example:

Diaphragmatic breathing means breathing from the diaphragm—the big muscular tissue that sits under your lungs. You might know it as “abdominal breathing”, and refers to breathing “to the abdomen” rather than merely to the chest.
Even though your lungs are obviously in your chest not your abdomen, breathing with a focus on expanding the abdomen (rather than the chest) when breathing in, will result in much deeper breathing as the diaphragm allows the lungs to fill downwards as well as outwards.
Why this helps when it comes to the vagus nerve is simply that the vagus nerve passes by the diaphragm, such that diaphragmatic breathing will massage the vagus nerve deep inside your body.
More than just treating migraines
Vagus nerve stimulation has also been researched and found potentially helpful for managing:
- Depression, inflammation, and heart disease
- Diabetes and glycemic issues in general
- Multiple sclerosis and autoimmune disease in general
- Alzheimer’s disease and dementia in general
- Rheumatoid arthritis (we already mentioned inflammation and autoimmune diseases, but this is an interesting paper so we included it)
All this is particularly important as we get older, because vagal response reduces with age, and vagus nerve stimulation, which improves vagal tone, makes it easier not just to manage the aforementioned maladies, but also simply to relax more easily and more deeply.

Let’s start with a “why”. If happiness doesn’t strike you as a worthwhile goal in and of itself, Dr. Chatterjee discusses the health implications of happiness/unhappiness.
And, yes, including in studies where other factors were controlled for, so he shows how happiness/unhappiness does really have a causal role in health—it’s not just a matter of “breaking news: sick people are less happy”.
The author, a British GP (General Practitioner, the equivalent of what the US calls a “family doctor”) with decades of experience, has found a lot of value in the practice of holistic medicine. For this reason, it’s what he recommends to his patients at work, in his books, his blog, and his regular spot on a popular BBC breakfast show.
The writing style is relaxed and personable, without skimping on information density. Indeed, Dr. Chatterjee offers many pieces of holistic health advice, and dozens of practical exercises to boost your happiness and proof you against adversity.
Because, whatever motivational speakers may say, we can’t purely “think ourselves happy”; sometimes we have real external threats and bad things in life. But, we can still improve our experience of even these things, not to mention suffer less, and get through it in better shape with a smile at the end of it.
Bottom line: if you’d like to be happier and healthier (who wouldn’t?), then this book is a sure-fire way to set you on that path.

What To Leave Off Your Table (To Stay Off This Surgeon’s)

Why we eat too much (and how we can fix that)
This is Dr. Andrew Jenkinson. He’s a Consultant Surgeon specializing in the treatment of obesity, gallstones, hernias, heartburn and abdominal pain. He runs regular clinics in both London and Dubai. What he has to offer us today, though, is insight as to what’s on our table that puts us on his table, and how we can quite easily change that up.
So, why do we eat too much?
First things first: some metabolic calculations. No, we’re not going to require you to grab a calculator here… Your body does it for you!
Our body’s amazing homeostatic system (the system that does its best to keep us in the “Goldilocks Zone” of all our bodily systems; not too hot or too cold, not dehydrated or overhydrated, not hyperglycemic or hypoglycemic, blood pressure not too high or too low, etc, etc) keeps track of our metabolic input and output.
What this means: if we increase or decrease our caloric consumption, our body will do its best to increase or decrease our metabolism accordingly:
- If we don’t give it enough energy, it will try to conserve energy (first by slowing our activities; eventually by shutting down organs in a last-ditch attempt to save the rest of us)
- If we give it too much energy, it will try to burn it off, and what it can’t burn, it will store
In short: if we eat 10% or 20% more or less than usual, our body will try to use 10% to 20% more or less than usual, accordingly.
So… How does this get out of balance?
The problem is in how our system does that, and how we inadvertently trick it, to our detriment.
For a system to function, it needs at its most base level two things—a sensor and a switch:
- A sensor: to know what’s going on
- A switch: to change what it’s doing accordingly
Now, if we eat the way we’re evolved to—as hunter-gatherers, eating mostly fruit and vegetables, supplemented by animal products when we can get them—then our body knows exactly what it’s eating, and how to respond accordingly.
Furthermore, that kind of food takes some eating! Most fruit these days is mostly water and fiber; in those days it often had denser fiber (before agricultural science made things easier to eat), but either way, our body knows when we are eating fruit and how to handle that. Vegetables, similarly. Unprocessed animal products, again, the gut goes “we know what this is” and responds accordingly.
But modern ultra-processed foods with trans-fatty acids, processed sugar and flour?
These foods zip calories straight into our bloodstream like greased lightning. We get them so quickly so easily and in such great caloric density, that our body doesn’t have the chance to count them on the way in!
What this means is: the body has no idea what it’s just consumed or how much or what to do with it, and doesn’t adjust our metabolism accordingly.
Bottom line:
Evolutionarily speaking, your body has no idea what ultra-processed food is. If you skip it and go for whole foods, you can, within the bounds of reason, eat what you like and your body will handle it by adjusting your metabolism accordingly.
Now, advising you “avoid ultra-processed foods and eat whole foods” was probably not a revelation in and of itself.
But: sometimes knowing a little more about the “why” makes the difference when it comes to motivation.

12 Questions For Better Brain Health

We usually preface our “Expert Insights” pieces with a nice banner that has a stylish tall cutout that allows us to put a photo of the expert in. Today we’re not doing that, because for today’s camera-shy expert, we could only find one photo, and it’s a small, grainy, square headshot that looks like it was taken some decades ago, and would not fit our template at all. You can see it here, though!
In any case, Dr. Linda Selwa is a neurologist and neurophysiologist with nearly 40 years of professional experience.
The right questions to ask
As a neurologist, she found that one of the problems that results in delayed interventions (and thus, lower efficacy of those interventions) is that people don’t know there’s anything to worry about until a degenerative brain condition has degenerated past a certain point. With that in mind, she bids us ask ourselves the following questions, and discuss them with our primary healthcare providers as appropriate:
- Sleep: Are you able to get sufficient sleep to feel rested?
- Affect, mood and mental health: Do you have concerns about your mood, anxiety, or stress?
- Food, diet and supplements: Do you have concerns about getting enough or healthy enough food, or have any questions about supplements or vitamins?
- Exercise: Do you find ways to fit physical exercise into your life?
- Supportive social interactions: Do you have regular contact with close friends or family, and do you have enough support from people?
- Trauma avoidance: Do you wear seatbelts and helmets, and use car seats for children?
- Blood pressure: Have you had problems with high blood pressure at home or at doctor visits, or do you have any concerns about blood pressure treatment or getting a blood pressure cuff at home?
- Risks, genetic and metabolic factors: Do you have trouble controlling blood sugar or cholesterol? Is there a neurological disease that runs in your family?
- Affordability and adherence: Do you have any trouble with the cost of your medicines?
- Infection: Are you up to date on vaccines, and do you have enough information about those vaccines?
- Negative exposures: Do you smoke, drink more than one to two drinks per day, or use non-prescription drugs? Do you drink well water, or live in an area with known air or water pollution?
- Social and structural determinants of health: Do you have concerns about keeping housing, having transportation, having access to care and medical insurance, or being physically or emotionally safe from harm?
You will note that some of these are well-known (to 10almonds readers, at least!) risk factors for cognitive decline, but others are more about systemic and/or environmental considerations, things that don’t directly pertain to brain health, but can have a big impact on it anyway.
About “concerns”: in the case of those questions that ask “do you have concerns about…?”, and you’re not sure, then yes, you do indeed have concerns.
About “trouble”: as for these kinds of health-related questionnaires in general, if a question asks you “do you have trouble with…?” and your answer is something like “no, because I have a special way of dealing with that problem” then the answer for the purposes of the questionnaire is yes, you do indeed have trouble.
Note that you can “have trouble with” something that you simultaneously “have under control”—just as a person can have no trouble at all with something that they leave very much out of control.

This health coach researched the science and crunched the numbers so that you don’t have to:
Body by the numbers
Let’s get mathematical:
Total Daily Energy Expenditure (TDEE) consists of:
- Basal Metabolic Rate (BMR): 70% of daily calorie burn (basic body functions, of which the brain is the single biggest calorie-burner)
- Non-Exercise Activity Thermogenesis (NEAT): 15% (the normal movements that occur as you go about your daily life)
- Exercise Activity: 5% (actual workouts, often overestimated)
- Thermic Effect of Food (TEF): 10% (energy needed for digestion)
Basic BMR estimate:
- Women: body weight (kg) × 0.9 × 24
- Men: body weight (kg) × 24
But yours may differ, so if you have a fitness tracker or other gadget that estimates it for you, go with that!
Note: muscle burns calories just to maintain it, making muscle mass crucial to increasing one’s BMR.
And now some notes about running a caloric deficit:
- Safe caloric deficit: no more than 500 calories/day.
- Absolute minimum daily intake: 1,200 calories (women), 1,500 calories (men) (not sustainable long-term).
- Tracking calories is useful but not always accurate.
- Extreme calorie restriction slows metabolism and can lead to binge-eating.
- Your body will adjust to calorie deficits over time, making long-term drastic deficits ineffective.
Diet for fat loss & muscle gain:
- Protein Intake: 1.5–2g per pound of body weight.
- Aim for 30g of protein per meal (supports muscle & satiety).
- Protein has a higher thermic effect (20-30%) than carbs (5-10%) & fats (2-4%), meaning more calories are burned digesting protein.
- Fats are essential for hormone health & satiety (0.5–1g per kg of body weight).
- Carbs should be complex (whole grains, vegetables, fruits, etc.).
- Avoid excessive simple carbs (sugar, white bread, white pasta, etc) to maintain stable hunger signals.
- Hydration is key for appetite control & metabolism (often mistaken for hunger).
Exercise for fat loss & muscle gain:
- Resistance training (3-5x per week) is essential for toning & metabolism.
- Cardio is NOT necessary for fat loss but good for overall health.
- NEAT (non-exercise movement) burns significant calories (walking, taking stairs, fidgeting, etc.).
- “Hot girl walks” & daily movement can significantly aid weight loss.
- Women won’t get “bulky” from weight training unless they eat like a bodybuilder (i.e. several times the daily caloric requirement).
Some closing words in addition:
Poor sleep reduces fat loss by 50% and increases hunger. High stress levels lead to fat retention and cravings for unhealthy foods. Thus, managing stress & sleep is as important as diet & exercise for body transformation!

“Osteoporosis” and “break” often don’t go well together, but here they do. So, what’s the breakthrough here?
There isn’t one, honestly. But if we overlook the marketing choices and focus on the book itself, the content here is genuinely good:
The book offers a comprehensive multivector approach to combatting osteoporosis, e.g:
- Diet
- Exercise
- Other lifestyle considerations
- Supplements
- Hormones
- Drugs
The author considers drugs a good and important tool for some people with osteoporosis, but not most. The majority of people, he considers, will do better without drugs—by tackling things more holistically.
The advice here is sound and covers all reasonable angles without getting hung up on the idea of there being a single magical solution for all.
Bottom line: if you’re looking for a book that’s a one-stop-shop for strategies against osteoporosis, this is a good option.

No, you don’t need the ‘Barbie drug’ to tan, whatever TikTok says. Here’s why melanotan-II is so risky

TikTok and Instagram influencers have been peddling the “Barbie drug” to help you tan.
But melanotan-II, as it’s called officially, is a solution that’s too good to be true. Just like tanning, this unapproved drug has a dark side.
Doctors, researchers and Australia’s drug regulator have been warning about its side effects – from nausea and vomiting to brain swelling and erection problems.
There are also safer ways of getting the tanned look, if that’s what you’re after.

No, it’s not a typo. Melanotan-II is very different from melatonin, which is a hormonal supplement used for insomnia and jet lag.
Melanotan-II is a synthetic version of the naturally ocurring hormone α-melanocyte stimulating hormone. This means the drug mimics the body’s hormone that stimulates production of the pigment melanin. This is what promotes skin darkening or tanning, even in people with little melanin.
Although the drug is promoted as a way of getting a “sunless tan”, it is usually promoted for use with UV exposure, to enhance the effect of UV and kickstart the tanning process.
Melanotan-II is related to, but different from, melanotan-I (afamelanotide), an approved drug used to treat the skin condition erythropoietic protoporphyria.
Melanotan-II is not registered for use with Australia’s Therapeutic Goods Administration (TGA). It is illegal to advertise it to the public or to provide it without a prescription.
However, social media has been driving unlicensed melanotan-II sales, a study published last year confirms.
There are many black market suppliers of melanotan-II injections, tablets and creams. More recently, nasal sprays have become more popular.
What are the risks?
Just like any drug, melanotan-II comes with the risk of side effects, many of which we’ve known about for more than a decade. These include changes in the size and pigmentation of moles, rapid appearance of new moles, flushing to the face, abdominal cramps, nausea, vomiting, chest pain and brain swelling.
It can also cause rhabdomyolysis, a dangerous syndrome where muscle breaks down and releases proteins into the bloodstream that damage the kidneys.
For men, the drug can cause priapism – a painful erection that does not go away and can damage the penis, requiring emergency treatment.
Its use has been linked with melanoma developing from existing moles either during or shortly after using the drug. This is thought to be due to stimulating pigment cells and causing the proliferation of abnormal cells.
Despite reports of melanoma, according to a study of social media posts the drug is often marketed as protecting against skin cancer. In fact, there’s no evidence to show it does this.
Social media posts about melanotan-II rarely mention health risks.
There are no studies on long-term safety of melanotan-II use.
Then there’s the issue of the drug not held to the high safety standards as TGA-approved products. This could result in variability in dose, undeclared ingredients and potential microbial contamination.

According to an ABC article published earlier this week, the TGA is cracking down on the illegal promotion of the drug on various websites. However, we know banned sellers can pop back up under a different name.
TikTok has banned the hashtags #tanningnasalspray, #melanotan and #melanotan2, but these products continue to be promoted with more generic hashtags, such as #tanning.
Part of a wider trend
Australia has some of the highest rates of skin cancer in the world. The “slip, slop, slap” campaign is a public health success story, with increased awareness of sun safety, a cultural shift and a decline in melanoma in young people.
However, the image of a bronzed beach body remains a beauty standard, especially among some young people.
Disturbingly, tan lines are trending on TikTok as a sought after summer accessory and the hashtag #sunburnttanlines has millions of views. We’ve also seen a backlash against sunscreen among some young people, again promoted on TikTok.
The Cancer Council is so concerned about the trend towards normalising tanning it has launched the campaign End the Trend.
You have other options
There are options beyond spraying an illegal, unregulated product up your nose, or risking unprotected sun exposure: fake tan.
Fake tan tends to be much safer than melanotan-II and there’s more long-term safety data. It also comes with potential side effects, albeit rare ones, including breathing issues (with spray products) and skin inflammation in some people.
Better still, you can embrace your natural skin tone.
