How To Be 7.5x More Likely To Develop Chronic Fatigue Syndrome

First, what is it?

Many more people have chronic fatigue, which is the symptom of being exhausted all the time, than have chronic fatigue syndrome (CFS) which is the illness of myalgic encephalomyelitis (ME).

This is because fatigue can be a symptom of many, many other conditions, and can be heavily influenced by lifestyle factors too.

A lot of the advice for dealing with chronic fatigue is often the same in both cases, but some will be different, because for example:

  • If your fatigue is from some other condition, that condition probably impacts what lifestyle factors you are (and are not) able to change, too
  • If your fatigue is from lifestyle factors, that hopefully means you can change those and enjoy less fatigue…
    • But if it’s not from lifestyle factors, as in ME/CFS, then advice to “exercise more” etc is not going to help so much.

The chronic disease pipeline

While it had been strongly suspected that COVID infection could lead to CFS, with long COVID having chronic fatigue as one of its characteristic symptoms, a research team led by Dr. Suzanne Vernon has now established the nature of the relationship.

It was a large (n=13,224) longitudinal observational cohort study of people with no pre-existing ME/CFS, grouped according to their COVID infection status:

  1. acute infected, enrolled within 30 days of infection or enrolled as uninfected who became infected (n=4,515)
  2. post-acute infected, enrolled greater than 30 days after infection (n=7,270)
  3. uninfected (n=1,439).

(to be clear, that last means “never infected”, or else they would be in group 2)

Note: people who had COVID and were hospitalized for it were excluded from the study, so this risk is the risk represented by even just more “moderate” infections.

What they found:

❝The proportion of all RECOVER-Adult participants that met criteria for ME/CFS following SARS-CoV-2 infection was 4.5% (531 of 11,785) compared to 0.6% (9 of 1439) in uninfected participants.❞

There are then different numbers if we look per 100 person-years, as the study also did—in which case, we get a re-modelled increase in risk of 5x instead of 7.5x, but a) that’s still not good b) the “here-and-now” figures of 4.5% vs 0.6% are also relevant.

The killer nobody wants to talk about anymore

Of course, as we all know the pandemic is over, because politicians declared it so, which is very reassuring.

Nevertheless, COVID is currently the still 4th leading cause of death in the US, placing it higher than stroke, Alzheimer’s, diabetes, and others.

So, while it’s very good to take care of our hearts, brains, blood sugars, and so forth, let’s at the very least continue to keep on top of our vaccinations, avoid enclosed crowded spaces where possible, etc.

    After all, many things are easily treatable today that mere decades ago were death sentences, and science is progressing all the time. We just have to stay alive, and in as good a condition as we reasonably can, to benefit from those advances!

    Bottom line: if you have a chronic disease, or if a loved one does, then this is an immensely valuable book to read.

    Hair today; gone tomorrow

    While a little peach fuzz is pretty ubiquitous, coarser hairs are less common in women especially earlier in life. However, even before menopause, such hair can be caused by main things, ranging from PCOS to genetics and more. In most cases, the underlying issue is excess androgen production, for one reason or another (i.e. there are many possible reasons, beyond the scope of this article).

    Options for dealing with this include…

    • Topical, such as eflornithine (e.g. Vaniqa) thins terminal hairs (those are the coarse kind); a course of 6–8 weeks continued use is needed.
    • Hormonal, such as estrogen (opposes testosterone and suppresses it), progesterone (downregulates 5α-reductase, which means less serum testosterone is converted to the more powerful dihydrogen testosterone (DHT) form), and spironolactone or other testosterone-blockers; not hormones themselves, but they do what it says on the tin (block testosterone).
    • Non-medical, such as electrolysis, laser, and IPL. Electrolysis works on all hair colors but takes longer; laser needs to be darker hair against paler skin* (because it works by superheating the pigment of the hair while not doing the same to the skin) but takes more treatments, and IPL is a less-effective more-convenient at-home option, that works on the same principles as laser (and so has the same color-based requirements), and simply takes even longer than laser.

    *so for example:

    • Black hair on white skin? Yes
    • Red hair on white skin? Potentially; it depends on the level of pigmentation. But it’s probably not the best option.
    • Gray/blonde hair on white skin? No
    • Black hair on mid-tone skin? Yes, but a slower pace may be needed for safety
    • Anything else on mid-tone skin? No
    • Anything on dark skin? No

    Lucid Dreaming: Methods & Uses

    We’ve written about dreaming more generally before:

    Today we’re going to be talking more about a subject we’ve only touched on previously: lucid dreaming

    What it is: lucid dreaming is the practice of being mentally awake while dreaming, with awareness that it is a dream, and control over the dream.

    Why is it useful? Beyond simply being fun, it can banish nightmares, it can improve one’s relationship with sleep (always something to look forward to, and sleep doesn’t feel like a waste of time at all!), and it can allow for exploring a lot of things that can’t easily be explored otherwise—which can be quite therapeutic.

    How to do it

    There are various ways to induce lucid dreaming, but the most common and “entry-level” method is called Mnemonic-Induced Lucid Dreaming (MILD).

    MILD involves having some means of remembering what one has forgotten, i.e., that one is dreaming. To break it down further, first we’ll need to learn how to perform a reality check. Again, there are many of these, but one of the simplest is to ask yourself:

    How did I get here?

    • If you can retrace your steps with relative ease and the story of how you got here does not sound too much like a dream sequence, you are probably not dreaming.
    • If you are dreaming, however, chances are that nothing actually led to where you are now; you just appeared here.

    Other reality checks include checking whether books, clocks, and/or lightswitches work as they should—all are notorious for often being broken in dreams; books have gibberish or missing or repeated text; clocks do not tell the correct time and often do not even tell a time that could be real (e.g: 07:72), and lightswitches may turn a light on/off without actually changing the level of illumination in the room.

    Now, a reality check is only useful if you actually perform it, so this is where MILD comes in.

    You need to make a habit of doing a reality check frequently. Whenever you remember, it’s a good time to do a reality check, but you should also try tying it to something. Many people use a red light, because then they can also use a timed red light during the night to subconsciously cue them that they are dreaming. But it could be as simple as “whenever I go to the bathroom, I do a reality check”.

    With this in mind, a fun method that has extra benefits is to try to use a magical power, such as psychokinesis. If (while fully awake) whenever you go to pick up some object you imagine it just wooshing magically to meet your hand halfway, then at some point you’ll instinctively do that while dreaming, and it’ll stand a good chance of working—and thus cluing you in that you are dreaming.

    How to stay lucid

    When you awaken within a dream (i.e. become lucid), there’s a good chance of one of two things happening quickly:

    • you forget again
    • you wake up

    So when you realize you are dreaming, do two things at once:

    • verbally repeat to yourself “I am dreaming now”. This will help stretch your awareness from one second to the next.
    • look at your hands, and touch things, especially the floor and/or walls. This will help to ground you within the dream.

    Things to do while lucid

    Flying is a good fun entry-level activity; it’s very common to initially find it difficult though, and only be able to lift up very slightly before gently falling down, or things like that. A good tip is: instead of trying to move yourself, you stay still and move the dream around you, as though you are rotating a 3D model (because guess what: you are).

    Confronting your nightmares and/or general fears is a good thing for many. Think, while you’re still awake during the day, about what you would do about the source/trigger of your fear if you had magical powers. Whatever you choose, keep it consistent for now, because this is about habit-forming.

    Example: let’s say there’s a person from your past who appears in your nightmares. Let’s say your chosen magic would be “I would cause the ground to open up, swallow them, and close again behind them”. Vividly imagine that whenever they come to mind while you are awake, and when you encounter them next in a nightmare, you’ll remember to do exactly that, and it’ll work.

    Learning about your own subconscious is a more advanced activity, but once you’re used to lucid dreaming, you can remember that everything in there is an internal projection of your own mind, so you can literally talk to parts of your subconscious, including past versions of yourself, or singular parts of your greater-whole personality, as per IFS:

    Moderation remains key

    Eggs have come under the spotlight for their protective potential against dementia, largely due to their content of omega-3 fatty acids, choline, and other nutrients.

    Nevertheless, the study had some limitations (including not measuring the quantity of eggs consumed, just the frequency), and while eating eggs daily showed the lowest rates of dementia, not eating them at all did not significantly alter the risk.

    Eating more than 2 eggs per day is still not recommended, however, for reasons of increasing the risk of other health issues, such as heart disease.

    More than suitable

    It’s common for a lot of things to come with the warning “not suitable for those who are pregnant or nursing”, with such frequency that it can be hard to know what one can safely do/take while pregnant or nursing.

    In the case of COVID vaccines, though, nearly 90% of babies who had to be hospitalized with COVID-19 had mothers who didn’t get the vaccine while they were pregnant.

    And as for how common that is: babies too young to be vaccinated (so, under 6 months) had the highest covid hospitalization rate of any age group except people over 75.

    Positive dieting

    Adding things into one’s diet is a lot more fun than taking things out, is generally easier to sustain, and (as a general rule of thumb; there are exceptions of course) give the greatest differences in health outcomes.

    This is perhaps most true of beans and pulses, which add many valuable vitamins, minerals, protein, and perhaps most importantly of all (single biggest factor in reducing heart disease risk), fiber.

    Clearing out disordered thinking

    Hoarding is largely driven by fear of loss, and this radical therapy tackles that at the root, by such means as rehearsing alternative outcomes of discarding through imagery rescripting, and examining the barriers to throwing things away—to break down those barriers one at a time.

    Superfluous

    Fluoridated water may not be as helpful for the teeth as it used to be prior to about 1975. Not because it became any less effective per se, but because of the modern prevalence of fluoride-containing toothpastes, mouthwashes, etc rendering it redundant in more recent decades.

    Off-label?

    With rising costs of living including rising healthcare costs, and increasing barriers to accessing in-person healthcare, it’s little wonder that many are turning to the gray market online to get their medications.

    These websites typically use legal loopholes to sell prescription drugs to the public, by employing morally flexible doctors who are content to expediently rubber-stamp prescriptions upon request, on the basis of the patient having filled out a web form and checked boxes for their symptoms (and of course also having waived all rights of complaint or legal recourse).

    However, some less scrupulous sorts are exploiting this market, to sell outright fake medications, using a setup that looks like a “legitimate” gray market website. Caveat emptor indeed.

    A rising threat

    In 2021 (we promise the paper was published only a few days ago!), the leading causes of death were:

    1. COVID-19
    2. Heart disease
    3. Stroke

    …which latter represented a rising threat, likely in part due to the increase in the aging population.

  • A person in Texas caught bird flu after mixing with dairy cattle. Should we be worried?

    The United States’ Centers for Disease Control and Prevention (CDC) has issued a health alert after the first case of H5N1 avian influenza, or bird flu, seemingly spread from a cow to a human.

    A farm worker in Texas contracted the virus amid an outbreak in dairy cattle. This is the second human case in the US; a poultry worker tested positive in Colorado in 2022.

    The virus strain identified in the Texan farm worker is not readily transmissible between humans and therefore not a pandemic threat. But it’s a significant development nonetheless.

    Some background on bird flu

    There are two types of avian influenza: highly pathogenic or low pathogenic, based on the level of disease the strain causes in birds. H5N1 is a highly pathogenic avian influenza.

    H5N1 first emerged in 1997 in Hong Kong and then China in 2003, spreading through wild bird migration and poultry trading. It has caused periodic epidemics in poultry farms, with occasional human cases.

    Influenza A viruses such as H5N1 are further divided into variants, called clades. The unique variant causing the current epidemic is H5N1 clade 2.3.4.4b, which emerged in late 2020 and is now widespread globally, especially in the Americas.

    In the past, outbreaks could be controlled by culling of infected birds, and H5N1 would die down for a while. But this has become increasingly difficult due to escalating outbreaks since 2021.

    Wild animals are now in the mix

    Waterfowl (ducks, swans and geese) are the main global spreaders of avian flu, as they migrate across the world via specific routes that bypass Australia. The main hub for waterfowl to migrate around the world is Quinghai lake in China.

    But there’s been an increasing number of infected non-waterfowl birds, such as true thrushes and raptors, which use different flyways. Worryingly, the infection has spread to Antarctica too, which means Australia is now at risk from different bird species which fly here.

    H5N1 has escalated in an unprecedented fashion since 2021, and an increasing number of mammals including sea lions, goats, red foxes, coyotes, even domestic dogs and cats have become infected around the world.

    Wild animals like red foxes which live in peri-urban areas are a possible new route of spread to farms, domestic pets and humans.

    Dairy cows and goats have now become infected with H5N1 in at least 17 farms across seven US states.

    What are the symptoms?

    Globally, there have been 14 cases of H5N1 clade 2.3.4.4b virus in humans, and 889 H5N1 human cases overall since 2003.

    Previous human cases have presented with a severe respiratory illness, but H5N1 2.3.4.4b is causing illness affecting other organs too, like the brain, eyes and liver.

    For example, more recent cases have developed neurological complications including seizures, organ failure and stroke. It’s been estimated that around half of people infected with H5N1 will die.

    The case in the Texan farm worker appears to be mild. This person presented with conjunctivitis, which is unusual.

    Food safety

    Contact with sick poultry is a key risk factor for human infection. Likewise, the farm worker in Texas was likely in close contact with the infected cattle.

    The CDC advises pasteurised milk and well cooked eggs are safe. However, handling of infected meat or eggs in the process of cooking, or drinking unpasteurised milk, may pose a risk.

    Although there’s no H5N1 in Australian poultry or cattle, hygienic food practices are always a good idea, as raw milk or poorly cooked meat, eggs or poultry can be contaminated with microbes such as salmonella and E Coli.

    If it’s not a pandemic, why are we worried?

    Scientists have feared avian influenza may cause a pandemic since about 2005. Avian flu viruses don’t easily spread in humans. But if an avian virus mutates to spread in humans, it can cause a pandemic.

    One concern is if birds were to infect an animal like a pig, this acts as a genetic mixing vessel. In areas where humans and livestock exist in close proximity, for example farms, markets or even in homes with backyard poultry, the probability of bird and human flu strains mixing and mutating to cause a new pandemic strain is higher.

    The cows infected in Texas were tested because farmers noticed they were producing less milk. If beef cattle are similarly affected, it may not be as easily identified, and the economic loss to farmers may be a disincentive to test or report infections.

    How can we prevent a pandemic?

    For now there is no spread of H5N1 between humans, so there’s no immediate risk of a pandemic.

    However, we now have unprecedented and persistent infection with H5N1 clade 2.3.4.4b in farms, wild animals and a wider range of wild birds than ever before, creating more chances for H5N1 to mutate and cause a pandemic.

    Unlike the previous epidemiology of avian flu, where hot spots were in Asia, the new hot spots (and likely sites of emergence of a pandemic) are in the Americas, Europe or in Africa.

    Pandemics grow exponentially, so early warnings for animal and human outbreaks are crucial. We can monitor infections using surveillance tools such as our EPIWATCH platform.

    The earlier epidemics can be detected, the better the chance of stamping them out and rapidly developing vaccines.

    Although there is a vaccine for birds, it has been largely avoided until recently because it’s only partially effective and can mask outbreaks. But it’s no longer feasible to control an outbreak by culling infected birds, so some countries like France began vaccinating poultry in 2023.

    For humans, seasonal flu vaccines may provide a small amount of cross-protection, but for the best protection, vaccines need to be matched exactly to the pandemic strain, and this takes time. The 2009 flu pandemic started in May in Australia, but the vaccines were available in September, after the pandemic peak.

    To reduce the risk of a pandemic, we must identify how H5N1 is spreading to so many mammalian species, what new wild bird pathways pose a risk, and monitor for early signs of outbreaks and illness in animals, birds and humans. Economic compensation for farmers is also crucial to ensure we detect all outbreaks and avoid compromising the food supply.

    Are You Ready For Allergy Season?

    For those of us in the Northern Hemisphere, fall will be upon us soon, and we have a few weeks to be ready for it. A common seasonal ailment is of course seasonal allergies—it’s not serious for most of us, but it can be very annoying, and can disrupt a lot of our normal activities.

    Suddenly, a thing that notionally does us no real harm, is making driving dangerous, cooking take three times as long, sex laughable if not off-the-table (so to speak), and the lightest tasks exhausting.

    So, what to do about it?

    Antihistamines: first generation

    Ye olde antihistamines such as diphenhydramine and chlorpheniramine are probably not what to do about it.

    They are small molecules that cross the blood-brain barrier and affect histamine receptors in the central nervous system. This will generally get the job done, but there’s a fair bit of neurological friendly-fire going on, and while they will produce drowsiness, the sleep will usually be of poor quality. They also tax the liver rather.

    If you are using them and not experiencing unwanted side effects, then don’t let us stop you, but do be aware of the risks.

    Antihistamines: later generations

    We’re going to aggregate 2nd gen, 3rd gen, and 4th gen antihistamines here, because otherwise we’ll be writing a history article and we don’t have room for that. But suffice it to say, later generations of antihistamines do not come with the same problems.

    Instead of going in all-guns-blazing to the CNS like first-gens, they are more specific in their receptor-targetting, resulting in negligible collateral damage:

    Special shout-out to cetirizine and loratadine, which are the drugs behind half the brand names you’ll see on pharmacy shelves around most of the world these days (including many in the US and Canada).

    Note that these two are very often discussed in the same sentence, sit next to each other on the shelf, and often have identical price and near-identical packaging. Their effectiveness (usually: moderate) and side effects (usually: low) are similar and comparable, but they are genuinely different drugs that just happen to do more or less the same thing.

    This is relevant because if one of them isn’t working for you (and/or is creating an unwanted side effect), you might want to try the other one.

    Another honorable mention goes to fexofenadine, for which pretty much all the same as the above goes, though it gets talked about less (and when it does get mentioned, it’s usually by its most popular brand name, Allegra).

    Finally, one that’s a little different and also deserving of a special mention is azelastine. It was recently (ish, 2021) moved from being prescription-only to being non-prescription (OTC), and it’s a nasal spray.

    It can cause drowsiness, but it’s considered safe and effective for most people. Its main benefit is not really the difference in drug, so much as the difference in the route of administration (nasal rather than oral). Because the drug is in liquid spray form, it can be absorbed through the mucus lining of the nose and get straight to work on blocking the symptoms—in contrast, oral antihistamines usually have to go into your stomach and take their chances there (we say “usually”, because there are some sublingual antihistamines that dissolve under the tongue, but they are less common.)

    Better than antihistamines?

    Writer’s note: at this point, I was given to wonder: “wait, what was I squirting up my nose last time anyway?”—because, dear readers, at the time I got it I just bought one of every different drug on the shelf, desperate to find something that worked. What worked for me, like magic, when nothing else had, was beclometasone dipropionate, which a) smelled delightfully of flowers, which might just be the brand I got, b) needs replacing now because I got it in March 2023 and it expired July 2024, and c) is not an antihistamine at all.

    But, that brings us to the final chapter for today: systemic corticosteroids

    They’re not ok for everyone (check with your doctor if unsure), and definitely should not be taken if immunocompromised and/or currently suffering from an infection (including colds, flu, COVID, etc) unless your doctor tells you otherwise (and even then, honestly, double-check).

    But! They can work like magic when other things don’t. Unlike antihistamines, which only block the symptoms, systemic corticosteroids tackle the underlying inflammation, which can stop the whole thing in its tracks.
