The pressure is on (and it need not be!)

Hypertension often has no symptoms but causes continuous strain on your blood vessels, heart, kidneys, and brain, increasing your risk of heart attack, stroke, and organ failure.

So, how to keep on top of it? Here are 10 mistakes to avoid:

  1. Salt as a “be all and end all”: focusing only on salt ignores the bigger driver of high blood pressure, as metabolic dysfunction and insulin resistance raise pressure by increasing fluid retention, nervous system activity, and vessel stiffness
  2. Ultra-processed foods: these foods worsen insulin resistance, increase visceral fat, and contain hidden sodium, so reducing them improves multiple blood pressure pathways at once
  3. Ignoring visceral fat: fat around your organs acts as a hormone-producing tissue that drives inflammation and blood pressure regulation systems, while even modest weight loss can meaningfully lower your readings
  4. Measurement errors: incorrect technique like using the wrong cuff, crossing your legs, or not resting beforehand can skew readings by 5–15 points, making proper measurement essential, or else you may think you’re doing fine while actually your blood pressure is out of control
  5. Low dietary nitrates: foods like beetroot and leafy greens boost nitric oxide, which relaxes your blood vessels and can lower systolic pressure by 4–10 mmHg
  6. Low potassium intake: potassium helps your body excrete sodium and relax blood vessels, but most diets are deficient despite it being abundant in whole foods like beans, potatoes, and greens
  7. Sedentary lifestyle: regular aerobic exercise improves insulin sensitivity, reduces visceral fat, and lowers blood pressure by around 7–12 points, similar to some medications
  8. Poor sleep: lack of sleep or conditions like obstructive sleep apnea keep your body in a stressed state and are a common cause of treatment-resistant hypertension
  9. Alcohol consumption: alcohol raises blood pressure in a dose-dependent way, and reducing intake can lead to noticeable improvements within days to weeks
  10. Medication mindset: blood pressure medication is effective and essential for many people, but it doesn’t fix the underlying causes, so lifestyle changes are necessary for long-term control

    When comparing apricots to lychees, we picked the apricots.

    Why?

    In terms of macros, apricots have nearly 2x the fiber and nearly 2x the protein, while lychees have more carbs. An easy first-round win for apricots.

    In the category of vitamins, apricots have more of vitamins A, B1, B5, E, and K, while lychees have more of vitamins B2, B6, B9, and C, yielding a marginal 5:4 win to apricots.

    Looking at minerals, apricots have more calcium, iron, manganese, potassium, and zinc, while lychees have more copper, phosphorus, and selenium, thus a 5:3 win for apricots to win their third round in a row.

    In other considerations, apricots have more polyphenols, and also have specific anticancer properties that lychees don’t, which is another point or two in apricots’ favor.

    Adding up the sections makes for a clear overall win for apricots, but by all means enjoy either or both, as diversity is good!

    How Much Does It Hurt?

    Sometimes, a medical professional will ask us to “rate your pain on a scale of 1–10”.

    It can be tempting to avoid rating one’s pain too highly, because if we say “10” then where can we go from there? There is always a way to make pain worse, after all.

    But that kind of thinking, however logical, is folly—from a practical point of view. Instead of risking having to give an 11 later, you have now understated your level-10 pain as a “7” and the doctor thinks “ok, I’ll give Tylenol instead of morphine”.

    A more useful scale

    First, know this:

    Zero is not “this is the lowest level of pain I get to”.

    Zero is “no pain”.

    As for the rest…

    1. My pain is hardly noticeable.
    2. I have a low level of pain; I am aware of my pain only when I pay attention to it.
    3. My pain bothers me, but I can ignore it most of the time.
    4. I am constantly aware of my pain, but can continue most activities.
    5. I think about my pain most of the time; I cannot do some of the activities I need to do each day because of the pain.
    6. I think about my pain all of the time; I give up many activities because of my pain.
    7. I am in pain all of the time; It keeps me from doing most activities.
    8. My pain is so severe that it is difficult to think of anything else. Talking and listening are difficult.
    9. My pain is all that I can think about; I can barely move or talk because of my pain.
    10. I am in bed and I can’t move due to my pain; I need someone to take me to the emergency room because of my pain.

    One extra thing to bear in mind…

    Medical staff will be more likely to believe a pain is being overstated, on a like-for-like basis, if you are a woman, or not white, or both.

    The main content of this book is stories (true ones, but it would do them a disservice to call them anecdotes, and it would be unduly clinical to call them case studies) of people who by a certain definition died—in the sense that their hearts stopped—but timely medical intervention allowed them to “come back to life” and continue living. Each gets a chapter devoted to them.

    Others weren’t so lucky; some of the chapters also remember those who didn’t come back. For example: two teenagers struck by the same bolt of lightning; a passerby rushed to give CPR to the one he saw first, only afterwards seeing the other. Fate can be like that.

    While the randomness of life and death can be utterly dispassionate, those of us who live on are often not nearly so unfeeling, and this book is about that. Making sense of the senseless, finding meaning, and truly appreciating life in all its heights and depths.

    The style is very personal, and most of it comes in the form of retrospective narrative prose, with present-day quotations (the author, an ICU doctor who encounters many such cases, interviewed the survivors for this book, whence the stories) that really drive home the lasting impacts of the experiences (many of the interviews are years, sometimes decades, after the event), and the remarkable diversity of emotional responses on a person-by-person basis.

    Bottom line: this book is very engaging from start to finish, and is very thought-provoking. If you’re anything like this reviewer, you might take little crying breaks from time to time, but it’s all very much worth it.

  • What To Leave Off Your Table (To Stay Off This Surgeon’s)

    Why we eat too much (and how we can fix that)

    This is Dr. Andrew Jenkinson. He’s a Consultant Surgeon specializing in the treatment of obesity, gallstones, hernias, heartburn and abdominal pain. He runs regular clinics in both London and Dubai. What he has to offer us today, though, is insight as to what’s on our table that puts us on his table, and how we can quite easily change that up.

    So, why do we eat too much?

    First things first: some metabolic calculations. No, we’re not going to require you to grab a calculator here… Your body does it for you!

    Our body’s amazing homeostatic system (the system that does its best to keep us in the “Goldilocks Zone” of all our bodily systems; not too hot or too cold, not dehydrated or overhydrated, not hyperglycemic or hypoglycemic, blood pressure not too high or too low, etc, etc) keeps track of our metabolic input and output.

    What this means: if we increase or decrease our caloric consumption, our body will do its best to increase or decrease our metabolism accordingly:

    • If we don’t give it enough energy, it will try to conserve energy (first by slowing our activities; eventually by shutting down organs in a last-ditch attempt to save the rest of us)
    • If we give it too much energy, it will try to burn it off, and what it can’t burn, it will store

    In short: if we eat 10% or 20% more or less than usual, our body will try to use 10% to 20% more or less than usual, accordingly.

    So… How does this get out of balance?

    The problem is in how our system does that, and how we inadvertently trick it, to our detriment.

    For a system to function, it needs at its most base level two things—a sensor and a switch:

    • A sensor: to know what’s going on
    • A switch: to change what it’s doing accordingly

    Now, if we eat the way we’re evolved to—as hunter-gatherers, eating mostly fruit and vegetables, supplemented by animal products when we can get them—then our body knows exactly what it’s eating, and how to respond accordingly.

    Furthermore, that kind of food takes some eating! Most fruit these days is mostly water and fiber; in those days it often had denser fiber (before agricultural science made things easier to eat), but either way, our body knows when we are eating fruit and how to handle that. Vegetables, similarly. Unprocessed animal products, again, the gut goes “we know what this is” and responds accordingly.

    But modern ultra-processed foods with trans-fatty acids, processed sugar and flour?

    These foods zip calories straight into our bloodstream like greased lightning. We get them so quickly so easily and in such great caloric density, that our body doesn’t have the chance to count them on the way in!

    What this means is: the body has no idea what it’s just consumed or how much or what to do with it, and doesn’t adjust our metabolism accordingly.

    Bottom line:

    Evolutionarily speaking, your body has no idea what ultra-processed food is. If you skip it and go for whole foods, you can, within the bounds of reason, eat what you like and your body will handle it by adjusting your metabolism accordingly.

    Now, advising you “avoid ultra-processed foods and eat whole foods” was probably not a revelation in and of itself.

    But: sometimes knowing a little more about the “why” makes the difference when it comes to motivation.

  • Eating disorders don’t just affect teen girls. The risk may go up around pregnancy and menopause too

    Eating disorders impact more than 1.1 million people in Australia, representing 4.5% of the population. These disorders include binge eating disorder, bulimia nervosa, and anorexia nervosa.

    Meanwhile, more than 4.1 million people (18.9%) are affected by body dissatisfaction, a major risk factor for some types of eating disorders.

    But what image comes to mind first when you think of someone with an eating disorder or body image concerns? Is it a teenage girl? If so, you’re definitely not alone. This is often the image we see in popular media.

    Eating disorders and body image concerns are most common in teenage girls, but their prevalence in adults, particularly in women, aged in their 30s, 40s and 50s, is actually close behind.

    So what might be going on with girls and women in these particular age groups to create this heightened risk?

    The 3 ‘P’s

    We can consider women’s risk periods for body image issues and eating disorders as the three “P”s: puberty (teenagers), pregnancy (30s) and perimenopause and menopause (40s, 50s).

    A recent report from The Butterfly Foundation showed the three highest prevalence groups for body image concerns are teenage girls aged 15–17 (39.9%), women aged 55–64 (35.7%) and women aged 35–44 (32.6%).

    We acknowledge there’s a wide age range for when girls and women will go through these phases of life. For example, a small proportion of women will experience premature menopause before 40, and not all women will become pregnant.

    Variations in the way eating disorder symptoms are measured across different studies can make it difficult to draw direct comparisons, but here’s a snapshot of what the evidence tells us.

    Puberty

    In a review of studies looking at children aged six to adolescents aged 18, 30% of girls in this age group reported disordered eating, compared to 17% of boys. Rates of disordered eating were higher as children got older.

    Pregnancy

    During pregnancy, eating disorder prevalence is estimated at 7.5%. Almost 70% of women are dissatisfied with their body weight and figure in the post-partum period.

    Perimenopause

    It’s estimated more than 73% of midlife women aged 42–52 are unsatisfied with their body weight. However, only a portion of these women would have been going through the menopause transition at the time of this study.

    The prevalence of eating disorders is around 3.5% in women over 40 and 1–2% in men at the same stage.

    So what’s going on?

    Although we’re not sure of the exact mechanisms underlying eating disorder and body dissatisfaction risk during the three “P”s, it’s likely a combination of factors are at play.

    These life stages involve significant reproductive hormonal changes (for example, fluctuations in oestrogen and progesterone) which can lead to increases in appetite or binge eating and changes in body composition. These changes can result in concerns about body weight and shape.

    These stages can also represent a major change in identity and self-perception. A girl going through puberty may be concerned about turning into an “adult woman” and changes in attitudes of those around her, such as unwanted sexual attention.

    Pregnancy obviously comes with significant body size and shape changes. Pregnant women may also feel their body is no longer their own.

    While social pressures to be thin can stop during pregnancy, social expectations arguably return after birth, demanding women “bounce back” to their pre-pregnancy shape and size quickly.

    Women going through menopause commonly express concerns about a loss of identity. In combination with changes in body composition and a perception their appearance is departing from youthful beauty ideals, this can intensify body dissatisfaction and increase the risk of eating disorders.

    These periods of life can each also be incredibly stressful, both physically and psychologically.

    For example, a girl going through puberty may be facing more adult responsibilities and stress at school. A pregnant woman could be taking care of a family while balancing work and other demands. A woman going through menopause could potentially be taking care of multiple generations (teenage children, ageing parents) while navigating the complexities of mid-life.

    Research has shown interpersonal problems and stressors can increase the risk of eating disorders.

    We need to do better

    Unfortunately most of the policy and research attention currently seems to be focused on preventing and treating eating disorders in adolescents rather than adults. There also appears to be a lack of understanding among health professionals about these issues in older women.

    In research I (Gemma) led with women who had experienced an eating disorder during menopause, participants expressed frustration with the lack of services that catered to people facing an eating disorder during this life stage. Participants also commonly said health professionals lacked education and training about eating disorders during menopause.

    We need to increase awareness among health professionals and the general public about the fact eating disorders and body image concerns can affect women of any age – not just teenage girls. This will hopefully empower more women to seek help without stigma, and enable better support and treatment.

  • An unbroken night’s sleep is a myth. Here’s what good sleep looks like

    What do you imagine a good night’s sleep to be?

    Often when people come into our sleep clinic seeking treatment, they share ideas about healthy sleep.

    Many think when their head hits the pillow, they should fall into a deep and restorative sleep, and emerge after about eight hours feeling refreshed. They’re in good company – many Australians hold the same belief.

    In reality, healthy sleep is cyclic across the night, as you move in and out of the different stages of sleep, often waking up several times. Some people remember one or more of these awakenings, others do not. Let’s consider what a healthy night’s sleep looks like.

    Sleep cycles are a roller-coaster

    As an adult, our sleep moves through different cycles and brief awakenings during the night. Sleep cycles last roughly 90 minutes each.

    We typically start the night with lighter sleep, before moving into deeper sleep stages, and rising again into rapid eye movement (REM) sleep – the stage of sleep often linked to vivid dreaming.

    If sleeping well, we get most of our deep sleep in the first half of the night, with REM sleep more common in the second half of the night.

    Adults usually move through five or six sleep cycles in a night, and it is entirely normal to wake up briefly at the end of each one. That means we might be waking up five times during the night. This can increase with older age and still be healthy. If you’re not remembering these awakenings that’s OK – they can be quite brief.

    What does getting a ‘good’ sleep actually mean?

    You’ll often hear that adults need between seven and nine hours of sleep per night. But good sleep is about more than the number of hours – it’s also about the quality.

    For most people, sleeping well means being able to fall asleep soon after getting into bed (within around 30 minutes), sleeping without waking up for long periods, and waking feeling rested and ready for the day.

    You shouldn’t be feeling excessively sleepy during the day, especially if you’re regularly getting at least seven hours of refreshing sleep a night (this is a rough rule of thumb).

    But are you noticing you’re feeling physically tired, needing to nap regularly and still not feeling refreshed? It may be worthwhile touching base with your general practitioner, as there a range of possible reasons.

    Common issues

    Sleep disorders are common. Up to 25% of adults have insomnia, a sleep disorder where it may be hard to fall or stay asleep, or you may wake earlier in the morning than you’d like.

    Rates of common sleep disorders such as insomnia and sleep apnoea – where your breathing can partially or completely stop many times during the night – also increase with age, affecting 20% of early adults and 40% of people in middle age. There are effective treatments, so asking for help is important.

    Beyond sleep disorders, our sleep can also be disrupted by chronic health conditions – such as pain – and by certain medications.

    There can also be other reasons we’re not sleeping well. Some of us are woken by children, pets or traffic noise during the night. These “forced awakenings” mean we may find it harder to get up in the morning, take longer to leave bed and feel less satisfied with our sleep. For some people, night awakenings may have no clear cause.

    A good way to tell if these awakenings are a problem for you is by thinking about how they affect you. When they cause feelings of frustration or worry, or are impacting how we feel and function during the day, it might be a sign to seek some help.

    We also may struggle to get up in the morning. This could be for a range of reasons, including not sleeping long enough, going to bed or waking up at irregular times – or even your own internal clock, which can influence the time your body prefers to sleep.

    If you’re regularly struggling to get up for work or family needs, it can be an indication you may need to seek help. Some of these factors can be explored with a sleep psychologist if they are causing concern.

    Can my smart watch help?

    It is important to remember sleep-tracking devices can vary in accuracy for looking at the different sleep stages. While they can give a rough estimate, they are not a perfect measure.

    In-laboratory polysomnography, or PSG, is the best standard measure to examine your sleep stages. A PSG examines breathing, oxygen saturation, brain waves and heart rate during sleep.

    Rather than closely examining nightly data (including sleep stages) from a sleep tracker, it may be more helpful to look at the patterns of your sleep (bed and wake times) over time.

    Understanding your sleep patterns may help identify and adjust behaviours that negatively impact your sleep, such as your bedtime routine and sleeping environment.

    And if you find viewing your sleep data is making you feel worried about your sleep, this may not be useful for you. Most importantly, if you are concerned it is important to discuss it with your GP who can refer you to the appropriate specialist sleep health provider.
