When it comes to advertising, we know that companies will often be as misleading as they can get away with. But just how misleading is it?

Vani Hari, of “Food Babe” fame, is here to unravel it all.

The book covers many areas of food and drink advertising and marketing, and gives particular attention to:

  • Sodas (with and without sugar), and how deleterious they are to the health—as well as not even helping people lose weight, but actively hindering
  • Nutritionally fortified foods, and what we may or may not actually get from them by the time the processing is done
  • Organic food, and what that may or may not mean

She also covers a lot of what happens outside of supermarkets, way back in universities and corporate boardrooms. In short, who is crossing whose palms with silver for a seal of approval… And what that means for us as consumers.

A strength of this book that sets it apart from many of its genre, by the way, is that while being deeply critical of certain institutions’ practices, it doesn‘t digress into tinfoil-hat pseudoscientific scaremongering, either. Here at 10almonds we love actual science, so that was good to see too.

Bottom line: is you’d like to know “can they say that and get away with it if it’s not true?” and make decisions based on the actual nutritional value of things, this is a great book for you.

    So, no question/request too big or small

    ❝I tried to use your calculator for heart health, and was unable to enter in my height or weight. Is there another way to calculate? Why will that field not populate?❞

    (this is in reference to yesterday’s main feature “How Are You, Really? And How Old Is Your Heart?“)

    How strange! We tested it in several desktop browsers and several mobile browsers, and were unable to find any version that didn’t work. That includes switching between metric and imperial units, per preference; both appear to work fine. Do be aware that it’ll only take numerical imput, though.

    ❝I may have missed it, but how much black pepper provides benefits?❞

    So, for any new subscribers joining us today, this is about two recent main features:

    As for a daily dosage of black pepper, it varies depending on the benefit you’re looking for, but:

    • 5–20mg of piperine is the dosage range used in most scientific studies we looked at
    • 10mg is a very common dosage found in many popular supplements
    • That’s the mass of piperine though, so if taking it as actual black pepper rather than as an extract, ½ teaspoon is considered sufficient to enjoy benefits.

    ❝I loved the health benefits of pepper. I do not like pepper. Where can I get it as a supplement?❞

    You can simply buy whole black peppercorns and take a few with water as though they were tablets. Your stomach acid will do the rest. Black pepper is also good for digestion, so taking it with a meal is best.

    You can buy piperine (black pepper extract) by itself as a supplement in powder form, but if you don’t like black pepper, you will probably not like this powder either. We couldn’t find it readily in capsule form.

    You can buy piperine (black pepper extract) as an adjunct to other supplements, with perhaps the most common/popular being turmeric capsules that also contain 10mg (or more) piperine per capsule. Shop around if you like, but here’s one that has 15mg piperine* per capsule, for example.

    *They call it “Bioperine®” but that is literally just piperine. Same goes if you see “Absorbagen™”, it’s still just piperine.

    ❝What do you mean when you say that something is adaptogenic?❞

    Simple version: it means it helps the body adapt to stress, by adjusting the body’s natural responses. Thus, adaptogenic supplements can be contrasted with tranquilizing drugs that mask stress by brute force, for example.

    Technical version: adaptogenic activity refers to improving physiological stress resilience, such as by moderating and modulating hypothalamic–pituitary–adrenal axis signaling, and/or by regulating levels of endogenic compounds involved in the cellular stress response.

  • Millet vs Buckwheat – Which is Healthier?

    Our Verdict

    When comparing millet to buckwheat, we picked the buckwheat.

    Why?

    Both of these naturally gluten-free grains* have their merits, but we say buckwheat comes out on top for most people (we’ll discuss the exception later).

    *actually buckwheat is a flowering pseudocereal, but in culinary terms, we’ll call it a grain, much like we call tomato a vegetable.

    Considering the macros first of all, millet has slightly more carbs while buckwheat has more than 2x the fiber. An easy win for buckwheat (they’re about equal on protein, by the way).

    In the category of vitamins, millet has more of vitamins B1, B2, B3, B6, and B9, while buckwheat has more of vitamins B5, E, K, and choline. Superficially that’s a 5:4 win for millet, though buckwheat’s margins of difference are notably greater, so the overall vitamin coverage could arguably be considered a tie.

    When it comes to minerals, millet has more phosphorus and zinc, while buckwheat has more calcium, copper, iron, magnesium, manganese, potassium, and selenium. For most of them, buckwheat’s margins of difference are again greater. An easy win for buckwheat, in any case.

    This all adds up to a clear win for buckwheat, but as promised, there is an exception: if you have issues with your kidneys that mean you are avoiding oxalates, then millet becomes the healthier choice, as buckwheat is rather high in oxalates while millet is low in same.

    For everyone else: enjoy both! Diversity is good. But if you’re going to pick one, buckwheat’s the winner.

  • Blue Cheese vs Brunost – Which is Healthier?

    Our Verdict

    When comparing blue cheese to brunost, we picked the brunost.

    Why?

    First, for the unfamiliar, as brunost isn’t necessarily as popular as blue cheese in N. America where most of our readers are:

    Brunost, literally “brown cheese” is a traditional Norwegian affair made from aggressively boiling milk, cream, and whey in an iron cauldron. Whereas the blue in blue cheese comes from mold, the brown in brown cheese comes from caramelizing the milk sugars in the cauldron. When we say “cauldron”, yes, there is nowadays mass-produced brunost that is no longer made in something that could be mistaken for a witch’s brew, but the use of cast iron is actually important to the process, and has been the subject of regulatory controversy in Norway; first the cast iron was abandoned, then because that changed the cheese they fortified the product with added iron supplementation, then that was banned, then they reversed it because it affected iron levels in the general population. Nowadays, it is usually made with iron, one way or another.

    Ok, so let’s see how they stack up against each other:

    In terms of macronutrients, the two cheeses are comparable in fat, but brunost has more carbs—because whereas bacteria (and to a lesser extent, the mold) ate nearly all the carbs in the blue cheese, the caramelization of the milk sugars in brunost meant the result stayed higher in carbs. Both are considered “low GI” foods, but this category is still at least a moderate win for blue cheese.

    When it comes to vitamins, brunost is higher in vitamins A, B1, B2, B3, B5, B6, and B12, while blue cheese is higher in vitamin B9. In other words, a clear and easy win for brunost.

    In the category of minerals, brunost has more copper, iron, magnesium, manganese, phosphorus, and potassium. Meanwhile, blue cheese contains more zinc, although we can also mention that blue cheese has about 2x the sodium, which is generally not considered a benefit. The two cheeses are about equal in calcium and selenium. Adding these up makes for another clear and easy win for brunost.

    In short, unless you are strongly avoiding [even low-GI foods’] carbs for some reason, brunost wins the day by virtue of its overwhelmingly better vitamin and mineral content.

    Still, like most fermented dairy products, both cheeses can be enjoyed in moderation as part of a healthy diet (assuming you don’t have an allergy/intolerance).

    When comparing oats to pearl barley, we picked the oats.

    Why?

    In terms of macronutrients first, pearl barley has about three times the carbs for only the same amount of protein and fiber—if it had been regular barley rather than pearl parley, it’d have about twice the fiber, but pearl barley has had the fibrous husk removed.

    Vitamins really set the two part, though: oats have a lot more (60x more) vitamin A, and notably more of vitamins B1, B2, B3, B5, B6, and B9, as well as 6x more vitamin E. In contrast, pearl barley has a little more vitamin K and choline. An easy win for oats in this section.

    In the category of minerals, oats have over 6x more calcium, 3x more iron, and a little more magnesium, manganese, and phosphorus. Meanwhile, pearl barley boats a little more copper, potassium, selenium, and zinc. So, a more moderate win for oats in this category.

    They are both very good for the gut, unless you have a gluten intolerance/allergy, in which case, oats are the only answer here since pearl barley, as per barley in general, has gluten as its main protein (oats, meanwhile, do not contain gluten, unless by cross-contamination).

    Adding up all the sections, this one’s a clear win for oats.

  • How Does One Test Acupuncture Against Placebo Anyway?

    Pinpointing The Usefulness Of Acupuncture

    We asked you for your opinions on acupuncture, and got the above-depicted, below-described, set of answers:

    • A little under half of all respondents voted for “It’s well-backed by modern science, per neurology, cardiology, immunology, etc”
    • Slightly fewer respondents voted for “We don’t understand how it works, but it works!”
    • A little under a fifth of respondents voted for “It may have some limited clinical applications beyond placebo”
    • One (1) respondent voted for for “It’s placebo at best”

    When we did a main feature about homeopathy, a couple of subscribers wrote to say that they were confused as to what homeopathy was, so this time, we’ll start with a quick definition first.

    First, what is acupuncture? For the convenience of a quick definition so that we can move on to the science, let’s borrow from Wikipedia:

    ❝Acupuncture is a form of alternative medicine and a component of traditional Chinese medicine in which thin needles are inserted into the body.

    Acupuncture is a pseudoscience; the theories and practices of TCM are not based on scientific knowledge, and it has been characterized as quackery.❞

    Now, that’s not a promising start, but we will not be deterred! We will instead examine the science itself, rather than relying on tertiary sources like Wikipedia.

    It’s worth noting before we move on, however, that there is vigorous debate behind the scenes of that article. The gist of the argument is:

    • On one side: “Acupuncture is not pseudoscience/quackery! This has long been disproved and there are peer-reviewed research papers on the subject.”
    • On the other: “Yes, but only in disreputable quack journals created specifically for that purpose”

    The latter counterclaim is a) potentially a “no true Scotsman” rhetorical ploy b) potentially true regardless

    Some counterclaims exhibit specific sinophobia, per “if the source is Chinese, don’t believe it”. That’s not helpful either.

    Well, the waters sure are muddy. Where to begin? Let’s start with a relatively easy one:

    It may have some clinical applications beyond placebo: True or False?

    True! Admittedly, “may” is doing some of the heavy lifting here, but we’ll take what we can get to get us going.

    One of the least controversial uses of acupuncture is to alleviate chronic pain. Dr. Vickers et al, in a study published under the auspices of JAMA (a very respectable journal, and based in the US, not China), found:

    ❝Acupuncture is effective for the treatment of chronic pain and is therefore a reasonable referral option. Significant differences between true and sham acupuncture indicate that acupuncture is more than a placebo.

    However, these differences are relatively modest, suggesting that factors in addition to the specific effects of needling are important contributors to the therapeutic effects of acupuncture❞

    If you’re feeling sharp today, you may be wondering how the differences are described as “significant” and “relatively modest” in the same text. That’s because these words have different meanings in academic literature:

    • Significant = p<0.05, where p is the probability of the achieved results occurring randomly
    • Modest = the differences between the test group and the control group were small

    In other words, “significant modest differences” means “the sample sizes were large, and the test group reliably got slightly better results than placebo”

    We don’t understand how it works, but it works: True or False

    Broadly False. When it works, we generally have an idea how.

    Placebo is, of course, the main explanation. And even in examples such as the above, how is placebo acupuncture given?

    By inserting acupuncture needles off-target rather than in accord with established meridians and points (the lines and dots that, per Traditional Chinese Medicine, indicate the flow of qi, our body’s vital energy, and welling-points of such).

    So, if a patient feels that needles are being inserted randomly, they may no longer have the same confidence that they aren’t in the control group receiving placebo, which could explain the “modest” difference, without there being anything “to” acupuncture beyond placebo. After all, placebo works less well if you believe you are only receiving placebo!

    Indeed, a (Korean, for the record) group of researchers wrote about this—and how this confounding factor cuts both ways:

    ❝Given the current research evidence that sham acupuncture can exert not only the originally expected non-specific effects but also sham acupuncture-specific effects, it would be misleading to simply regard sham acupuncture as the same as placebo.

    Therefore, researchers should be cautious when using the term sham acupuncture in clinical investigations.❞

    It’s well-backed by modern science, per neurology, cardiology, immunology, etc: True or False?

    False, for the most part.

    While yes, the meridians and points of acupuncture charts broadly correspond to nerves and vasculature, there is no evidence that inserting needles into those points does anything for one’s qi, itself a concept that has not made it into Western science—as a unified concept, anyway…

    Note that our bodies are indeed full of energy. Electrical energy in our nerves, chemical energy in every living cell, kinetic energy in all our moving parts. Even, to stretch the point a bit, gravitational potential energy based on our mass.

    All of these things could broadly be described as qi, if we so wish. Indeed, the ki in the Japanese martial art of aikido is the latter kinds; kinetic energy and gravitational potential energy based on our mass. Same goes, therefore for the ki in kiatsu, a kind of Japanese massage, while the ki in reiki, a Japanese spiritual healing practice, is rather more mystical.

    The qi in Chinese qigong is mostly about oxygen, thus indirectly chemical energy, and the electrical energy of the nerves that are receiving oxygenated blood at higher or lower levels.

    On the other hand, the efficacy of the use of acupuncture for various kinds of pain is well-enough evidenced. Indeed, even the UK’s famously thrifty NHS (that certainly would not spend money on something it did not find to work) offers it as a complementary therapy for some kinds of pain:

    ❝Western medical acupuncture (dry needling) is the use of acupuncture following a medical diagnosis. It involves stimulating sensory nerves under the skin and in the muscles.

    This results in the body producing natural substances, such as pain-relieving endorphins. It’s likely that these naturally released substances are responsible for the beneficial effects experienced with acupuncture.❞

    Meanwhile, the NIH’s National Cancer Institute recommends it… But not as a cancer treatment.

    Rather, they recommend it as a complementary therapy for pain management, and also against nausea, for which there is also evidence that it can help.

    Frustratingly, while they mention that there is lots of evidence for this, they don’t actually link the studies they’re citing, or give enough information to find them. Instead, they say things like “seven randomized clinical trials found that…” and provide links that look reassuring until one finds, upon clicking on them, that it’s just a link to the definition of “randomized clinical trial”:

    However, doing our own searches finds many studies (mostly in specialized, potentially biased, journals such as the Journal of Acupuncture and Meridian Studies) finding significant modest outperformance of [what passes for] placebo.

    Sometimes, the existence of papers with promising titles, and statements of how acupuncture might work for things other than relief of pain and nausea, hides the fact that the papers themselves do not, in fact, contain any evidence to support the hypothesis. Here’s an example:

    ❝The underlying mechanisms behind the benefits of acupuncture may be linked with the regulation of the hypothalamic-pituitary-gonadal (adrenal) axis and activation of the Wnt/β-catenin and OPG/RANKL/RANK signaling pathways.

    In summary, strong evidence may still come from prospective and well-designed clinical trials to shed light on the potential role of acupuncture in preserving bone loss❞

    So, here they offered a very sciencey hypothesis, and to support that hypothesis, “strong evidence may still come”.

    “We must keep faith” is not usually considered evidence worthy of inclusion in a paper!

    PS: the above link is just to the abstract, because the “Full Text” link offered in that abstract leads to a completely unrelated article about HIV/AIDS-related cryptococcosis, in a completely different journal, nothing to do with acupuncture or osteoporosis).

    Again, this is not the kind of professionalism we expect from peer-reviewed academic journals.

    Bottom line:

    Acupuncture reliably performs slightly better than sham acupuncture for the management of pain, and may also help against nausea.

    Beyond placebo and the stimulation of endorphin release, there is no consistently reliable evidence that is has any other discernible medical effect by any mechanism known to Western science—though there are plenty of hypotheses.

    That said, absence of evidence is not evidence of absence, and the logistical difficulty of testing acupuncture against placebo makes for slow research. Maybe one day we’ll know more.

    For now:

    • If you find it helps you: great! Enjoy
    • If you think it might help you: try it! By a licensed professional with a good reputation, please.
    • If you are not inclined to having needles put in you unnecessarily: skip it! Extant science suggests that at worst, you’ll be missing out on slight relief of pain/nausea.

    The titular “two-second advantage” can in some cases be literal (imagine you got a two-second head-start in a boxing match!), in other cases can refer to being just a little ahead of things in a way that can confer a great advantage, often cumulatively—as anyone who’s played Monopoly can certainly attest.

    Vivek Ranadivé and Kevin Maney give us lots of examples from business, sports, politics, economics, and more, in a way that seeks to cultivate a habit of asking the right questions in order to anticipate the future and not just be ahead of the competition—some areas of life don’t have competition for most people, like health, for example—but to generally have things “in hand”.

    When it comes to personal finances, health, personal projects, and the like, those tiny initial advantages that lead to incremental further improvements, can be the difference between continually (and frantically) playing catch-up, or making the jump past breaking even to going from strength to strength.
