Fun fact: aging makes alcohol harder to process because liver enzymes become less efficient, while loss of muscle and gain of fat reduce body water, meaning alcohol is less diluted and has a greater effect.
Menopause can exacerbate this because alcohol can worsen already-disrupted sleep and may increase symptoms such as hot flushes.
Nevertheless, it’s especially around this time of life that an evening glass of wine can develop into a regular habit, and of course alcohol is often used as an unhealthy attempt to cope with stress or anxiety, and/or can be used the intention of increasing confidence in social situations.
As for the risks, they include:
- Liver damage: prolonged excessive alcohol consumption can progress from fatty liver, which is reversible, to liver inflammation and eventually permanent scarring and cirrhosis.
- Aging and weight: alcohol is associated with shorter telomeres, which are protective DNA sequences at the ends of chromosomes and a marker of biological aging. Alcohol is also calorie-dense without providing nutritional value, with a standard 12% wine serving containing around 155 calories, making it particularly unhelpful when abdominal fat tends to increase around menopause.
- Mental health: alcohol is a depressant, and regular drinking can worsen anxiety and depression, which can already become more common during menopause.
- Cancer risk: alcohol is classified as a group 1 carcinogen by the World Health Organization, and alcohol consumption further increases the risk of several cancers, including mouth, throat, esophageal, stomach, colorectal, liver, and breast cancer. A 2026 meta-analysis found that breast cancer risk increased with any alcohol consumption and rose proportionally with the amount consumed.
- Heart disease: the former idea that moderate alcohol consumption, particularly red wine, protects the heart came largely from observational studies with flawed comparisons between drinkers and non-drinkers. When researchers accounted for former heavy drinkers and people who stopped drinking because they were already ill, the apparent cardiovascular and mortality benefits disappeared.
In short: quitting is good. Failing that, less is better than more.

Ear Candling: Is It Safe & Does It Work?

Does This Practice Really Hold A Candle To Evidence-Based Medicine?
In Tuesday’s newsletter, we asked you your opinion of ear candling, and got the above-depicted, below-described set of responses:
- Exactly 50% said “Under no circumstances should you put things in your ear and set fire to them”
- About 38% said “It is a safe, drug-free way to keep the ears free from earwax and pathogens”
- About 13% said “Done correctly, thermal-auricular therapy is harmless and potentially beneficial”
This means that if we add the two positive-to-candling answers together, it’s a perfect 50:50 split between “do it” and “don’t do it”.
(Yes, 38%+13%=51%, but that’s because we round to the nearest integer in these reports, and more precisely it was 37.5% and 12.5%)
So, with the vote split, what does the science say?
First, a quick bit of background: nobody seems keen to admit to having invented this. One of the major manufacturers of ear candles refers to them as “Hopi” candles, which the actual Hopi tribe has spent a long time asking them not to do, as it is not and never has been used by the Hopi people. Other proposed origins offered by advocates of ear candling include Traditional Chinese Medicine (not used), Ancient Egypt (no evidence of such whatsoever), and Atlantis:

It is a safe, drug-free way to keep the ears free from earwax and pathogens: True or False?
False! In a lot of cases of alternative therapy claims, there’s an absence of evidence that doesn’t necessarily disprove the treatment. In this case, however, it’s not even an open matter; its claims have been actively disproven by experimentation:
- It doesn’t remove earwax; on the contrary, experimentation “showed no removal of cerumen from the external auditory canal. Candle wax was actually deposited in some“
- It doesn’t remove pathogens, and the proposed mechanism of action for removing pathogens, that of the “chimney effect”: the idea that the burning candle creates a vacuum that draws wax out of the ear along with debris and bacteria, simply does not work; on the contrary, “Tympanometric measurements in an ear canal model demonstrated that ear candles do not produce negative pressure”.
- It isn’t safe; on the contrary, “Ear candles have no benefit in the management of cerumen and may result in serious injury”
In a medium-sized survey (n=122), the following injuries were reported:
- 13 x burns
- 7 x occlusion of the ear canal
- 6 x temporary hearing loss
- 3 x otitis externa (this also called “swimmer’s ear”, and is an inflammation of the ear, accompanied by pain and swelling)
- 1 x tympanic membrane perforation
Indeed, authors of one paper concluded:
❝Ear candling appears to be popular and is heavily advertised with claims that could seem scientific to lay people. However, its claimed mechanism of action has not been verified, no positive clinical effect has been reliably recorded, and it is associated with considerable risk.
No evidence suggests that ear candling is an effective treatment for any condition. On this basis, we believe it can do more harm than good and we recommend that GPs discourage its use❞

Under no circumstances should you put things in your ear and set fire to them: True or False?
True! It’s generally considered good advice to not put objects in general in your ears.
Inserting flaming objects is a definite no-no. Please leave that for the Cirque du Soleil.
You may be thinking, “but I have done this and suffered no ill effects”, which seems reasonable, but is an example of survivorship bias in action—it doesn’t make the thing in question any safer, it just means you were one of the one of the ones who got away unscathed.
If you’re wondering what to do instead… Ear oils can help with the removal of earwax (if you don’t want to go get it sucked out at a clinic—the industry standard is to use a suction device, which actually does what ear candles claim to do). Can I take antihistamines everyday? More than the recommended dose? What if I’m pregnant? Here’s what the research says

Allergies happen when your immune system overreacts to a normally harmless substance like dust or pollen. Hay fever, hives and anaphylaxis are all types of allergic reactions.
Many of those affected reach quickly for antihistamines to treat mild to moderate allergies (though adrenaline, not antihistamines, should always be used to treat anaphylaxis).
If you’re using oral antihistamines very often, you might have wondered if it’s OK to keep relying on antihistamines to control symptoms of allergies. The good news is there’s no research evidence to suggest regular, long-term use of modern antihistamines is a problem.
But while they’re good at targeting the early symptoms of a mild to moderate allergic reaction (sneezing, for example), oral antihistamines aren’t as effective as steroid nose sprays for managing hay fever. This is because nasal steroid sprays target the underlying inflammation of hay fever, not just the symptoms.
Here are the top six antihistamines myths – busted.
Andrea Piacquadio/Pexels Myth 1. Oral antihistamines are the best way to control hay fever symptoms
Wrong. In fact, the recommended first line medical treatment for most patients with moderate to severe hay fever is intranasal steroids. This might include steroid nose sprays (ask your doctor or pharmacist if you’d like to know more).
Studies have shown intranasal steroids relieve hay fever symptoms better than antihistamine tablets or syrups.
To be effective, nasal steroids need to be used regularly, and importantly, with the correct technique.
In Australia, you can buy intranasal steroids without a doctor’s script at your pharmacy. They work well to relieve a blocked nose and itchy, watery eyes, as well as improve chronic nasal blockage (however, antihistamine tablets or syrups do not improve chronic nasal blockage).
Some newer nose sprays contain both steroids and antihistamines. These can provide more rapid and comprehensive relief from hay fever symptoms than just oral antihistamines or intranasal steroids alone. But patients need to keep using them regularly for between two and four weeks to yield the maximum effect.
For people with seasonal allergic rhinitis (hayfever), it may be best to start using intranasal steroids a few weeks before the pollen season in your regions hits. Taking an antihistamine tablet as well can help.
Antihistamine eye drops work better than oral antihistamines to relieve acutely itchy eyes (allergic conjunctivitis).
Myth 2. My body will ‘get used to’ antihistamines
Some believe this myth so strongly they may switch antihistamines. But there’s no scientific reason to swap antihistamines if the one you’re using is working for you. Studies show antihistamines continue to work even after six months of sustained use.
Myth 3. Long-term antihistamine use is dangerous
There are two main types of antihistamines – first-generation and second-generation.
First-generation antihistamines, such as chlorphenamine or promethazine, are short-acting. Side effects include drowsiness, dry mouth and blurred vision. You shouldn’t drive or operate machinery if you are taking them, or mix them with alcohol or other medications.
Most doctors no longer recommend first-generation antihistamines. The risks outweigh the benefits.
The newer second-generation antihistamines, such as cetirizine, fexofenadine, or loratadine, have been extensively studied in clinical trials. They are generally non-sedating and have very few side effects. Interactions with other medications appear to be uncommon and they don’t interact badly with alcohol. They are longer acting, so can be taken once a day.
Although rare, some side effects (such as photosensitivity or stomach upset) can happen. At higher doses, cetirizine can make some people feel drowsy. However, research conducted over a period of six months showed taking second-generation antihistamines is safe and effective. Talk to your doctor or pharmacist if you’re concerned.

As long as it’s the second-generation antihistamine, it’s fine. You can buy child versions of second-generation antihistamines as syrups for kids under 12.
Though still used, some studies have shown certain first-generation antihistamines can impair childrens’ ability to learn and retain information.
Studies on second-generation antihistamines for children have found them to be safer and better than the first-generation drugs. They may even improve academic performance (perhaps by allowing kids who would otherwise be distracted by their allergy symptoms to focus). There’s no good evidence they stop working in children, even after long-term use.
For all these reasons, doctors say it’s better for children to use second-generation than first-generation antihistimines.
What about using antihistimines while you’re pregnant? One meta analysis of combined study data including over 200,000 women found no increase in fetal abnormalities.
Many doctors recommend the second-generation antihistamines loratadine or cetirizine for pregnant people. They have not been associated with any adverse pregnancy outcomes. Both can be used during breastfeeding, too.
Myth 5. It is unsafe to use higher than the recommended dose of antihistamines
Higher than standard doses of antihistamines can be safely used over extended periods of time for adults, if required.
But speak to your doctor first. These higher doses are generally recommended for a skin condition called chronic urticaria (a kind of chronic hives).
Myth 6. You can use antihistamines instead of adrenaline for anaphylaxis
No. Adrenaline (delivered via an epipen, for example) is always the first choice. Antihistamines don’t work fast enough, nor address all the problems caused by anaphylaxis.
Antihistamines may be used later on to calm any hives and itching, once the very serious and acute phase of anaphylaxis has been resolved.
In general, oral antihistamines are not the best treatment to control hay fever – you’re better off with steroid nose sprays. That said, second-generation oral antihistamines can be used to treat mild to moderate allergy symptoms safely on a regular basis over the long term.

Our Verdict
When comparing mango to pineapple, we picked the pineapple.
Why?
It was close! Both of these tropical fruits have almost identical macros, and when it comes to vitamins and minerals, mango has slightly more vitamins while pineapple has slightly more minerals, so that balances out too. Their glycemic loads are 11 and 13 respectively, so: very low, and very similar.

In terms of what sets them apart:
Mango has a lot of vitamin A, to the point that it can interfere with blood-thinners if you take those.
Pineapple has bromelain, an enzyme with unique anti-inflammatory properties that we must devote a Research Review Monday to one of these days, because there’s a lot to say, but the short version is, it’s very powerful.
Since bromelain is found only in pineapples, whereas vitamin A is easy to find in abundance in many foods, we went with the pineapple.

This One Bed Exercise Strengthens Your Legs Faster Than Walking

No, not that one bed exercise. Though that can be great too!
But this isn’t quite that:
Put your feet up!
Walking is great in many ways, but it doesn’t provide enough resistance to preserve or build the leg, hip, and core strength needed for stability as we age.
So instead, the exercise presented is a progression of bridge variations designed to increase lower-body strength and single-leg stability, both of which are really important.
Single-leg stability may seem like a strange one, “when am I ever having to perform feats of strength while balancing on one leg?”, but actually, while walking (and especially while going up and down stairs, and getting in and out of seats, including car seats), we’re often relying 70% or so on one leg at a time, and if at any point something requires us to have to rebalance suddenly, then very often it’s 100% on one leg and sometimes at an inconvenient angle, to avoid injuring ourselves.
So, it’s important.
Here’s what to do. We’re going to present it here in 4 steps, but it’s one exercise to be done start to finish:
- Foundation bridge: lie on your back with your heels close to your buttocks and your hands by your sides. Flatten your lower back into the bed or mat, push through your heels, lift your hips into a straight line, squeeze your buttocks, and lower yourself back down. If you cannot do more than 12 comfortably, stick with this part until it becomes easier, before progressing to the next steps.
- Stability boost: hold the bridge position, shift your weight onto one leg, briefly lift the other foot, put it back down, and alternate sides while keeping your hips elevated. The movement is intended to challenge stability in a way that resembles the weight shift that occurs during walking.
- Single-leg bridge: lift one leg from the bridge and straighten it, hold for a couple of seconds, then lower yourself back down using the supporting leg. Alternate sides. Do six repetitions on each leg while keeping your pelvis completely level.
- Straight-leg raise: from the bridge position, straighten one leg and lower that heel toward the bed or mat without allowing your pelvis to drop. Bring the leg back up, do six repetitions, then switch sides and repeat. Lower yourself slowly back down after finishing.

Gut Health 101
We have so many microorganisms inside us, that by cell count, their cells outnumber ours more than ten-to-one. By gene count, we have 23,000 and they have more than 3,000,000. In effect, we are more microbe than we are human. And, importantly: they form a critical part of what keeps our overall organism ticking on.

Our trillions of tiny friends keep us alive, so it really really pays to return the favor.
But how?
Probiotics and fermented foods
You probably guessed this one, but it’d be remiss not to mention it first. It’s no surprise that probiotics help; the clue is in the name. In short, they help add diversity to your microbiome (that’s a good thing).

As for fermented foods, not every fermented food will boost your microbiota, but great options include…
- Fermented vegetables (sauerkraut, pickles, etc)
- You’ll often hear kimchi mentioned; that is also pickled vegetables, usually mostly cabbage. It’s just the culinary experience that differs. Unlike sauerkraut, kimchi is usually spiced, for example.
- Kombucha (a fermented sweet tea)
- Miso & tempeh (different preparations of fermented soy)
The health benefits vary based on the individual strains of bacteria involved in the fermentation, so don’t get too caught up on which is best.
The best one is the one you enjoy, because then you’ll have it regularly!
Feed them plenty of prebiotic fibers
Those probiotics you took? The bacteria in them eat the fiber that you can’t digest without them. So, feed them those sorts of fibers.
Great options include:
- Bananas
- Garlic
- Onions
- Whole grains

Don’t feed them sugar and sweeteners
Sugar and (and, counterintuitively, aspartame) can cause unfortunate gut microbe imbalances. Put simply, they kill some of your friends and feed some of your enemies. For example…
Candida, which we all have in us to some degree, feeds on sugar (including the sugar formed from breaking down alcohol, by the way) and refined carbs. Then it grows, and puts its roots through your intestinal walls, linking with your neural system. Then it makes you crave the very things that will feed it and allow it to put bigger holes in your intestinal walls.
Do not feed the Candida.

(That’s scientist-speak for “Candida puts holes in your intestines, and stuff can then go through those holes”)
And as for how that comes about, it’s like we said:

The usual five things
- Good diet (Mediterranean Diet is good; plant-based version of it is by far the best for this)
- Good exercise (yes, really)
- Good sleep (helps them, and they’ll help you get better sleep in return)
- Limit or eliminate alcohol consumption (what a shocker)
- Don’t smoke (it’s bad for everything, including gut health)
One last thing you should know:
If you’re used to having animal products in your diet, and make a sudden change to all plants, your gut will object very strongly. This is because your gut microbiome is used to animal products, and a plant-based diet will cause many helpful microbes to flourish in great abundance, and many less helpful microbes will starve and die. And they will make it officially Not Fun™ for you.
So, you have two options to consider:
- Do it anyway, and sit it out (and believe us, you’ll be sitting), get the change over with quickly, and enjoy the benefits and much happier gut that follows.
- Make the change gradual. Reduce portions of animal products slowly, have “Meatless Mondays” etc, and slowly make the change over. This—for most people—is pretty comfortable, easy, and effective.
And remember: the effects of these things we’ve talked about today compound when you do more than one of them, but if you don’t want to take probiotics or really hate kombucha or absolutely won’t consider a plant-based diet or struggle to give up sugar or alcohol, etc… Just do what you can do, and you’ll still have a net improvement!
