When comparing artichoke to carrots, we picked the artichoke.

Why?

Both are great, but still, it wasn’t close:

In terms of macros, the artichoke has nearly 2x the fiber and more than 3x the protein, for the same carbs; a clear first-round win for artichoke.

In the category of vitamins, artichoke has more of vitamins B1, B2, B3, B5, B7, B9, C, and K, while carrots have more of vitamins A, B6, and E, giving artichoke an 8:3 win in this second round.

Looking at minerals, artichoke has more calcium, copper, iron, magnesium, manganese, phosphorus, potassium, selenium, and zinc, while carrots are not higher in any minerals; a complete win for artichoke here.

Adding up the sections makes for a clear overall win for artichoke, but by all means enjoy either or both, as diversity is best!

    Looking at minerals, artichokes have more copper, iron, magnesium, manganese, potassium, and zinc, while celeriac has more phosphorus and selenium, meaning a convincing 6:2 win for artichokes in this round.

    In other considerations, artichokes also have a much higher polyphenol content, so that’s another point in their favor.

  • What’s the difference between osteoarthritis and rheumatoid arthritis?

    Arthritis – an umbrella term for around 100 conditions that damage the joints – affects 4.1 million Australians. This is expected to rise by 31% to 5.4 million by 2040 and cost the Australian health-care system an estimated $12 billion each year.

    The two most common types, osteoarthritis and rheumatoid arthritis, can both cause joint pain, swelling and stiffness. Both are more common in women. Neither can be cured.

    But their causes, risk factors and treatments are different – here’s what you need to know.

    What is osteoarthritis?

    Osteoarthritis is the most common form of arthritis. It affects 2.1 million Australians, mostly older people. About a third of Australians aged 75 and older have the condition.

    It can affect any joint but is most common in the knees, hips, fingers, thumbs and big toes.

    The main symptom is pain, especially during movement. Other symptoms may include swelling, stiffness and changes to the shape of joints.

    The main risk factors are ageing and obesity, as well as previous injuries or surgery. For osteoarthritis in the hands, genetics also play a big role.

    Signs of osteoarthritis can appear on knee scans from around age 45 and become more common with age.

    However, this type of arthritis not simply the “wear and tear” of ageing. Osteoarthritis is a complex disease that affects the whole joint. This includes the cartilage (“shock-absorbing” connective tissue protecting your bones), bones, ligaments (connective tissue holding bones and body parts in place) and joint lining.

    How is it diagnosed?

    Diagnosis is based on symptoms (such as pain and restricted movement) and a physical exam.

    The disease generally worsens over time and cannot be reversed. But the severity of damage does not always correlate with pain levels.

    For this reason, x-rays and MRI scans are usually unhelpful. Some people with early osteoarthritis experience severe pain, but the damage won’t show up on a scan. Others with advanced and visible osteoarthritis may have few symptoms or none at all.

    What about rheumatoid arthritis?

    Unlike osteoarthritis, rheumatoid arthritis is an autoimmune disease. This means the immune system attacks the joint lining, causing inflammation and damage.

    Common symptoms include pain, joint swelling and stiffness, especially in the morning.

    Rheumatoid arthritis is less common than osteoarthritis, affecting around 514,000 Australians. It mostly impacts the wrists and small joints in the hands and feet, though larger joints such as the elbows, shoulders, knees and ankles can also be involved.

    It can also affect other organs, including the skin, lungs, eyes, heart and blood vessels. Fortunately, disease outside the joint has become less common in recent years, likely due to better and earlier treatment.

    Rheumatoid arthritis often develops earlier than osteoarthritis but can occur at any age. Onset is most frequent in those aged 35–64. Smoking increases your risk.

    How is it diagnosed?

    As with osteoarthritis, your doctor will diagnose rheumatoid arthritis based on your symptoms and a physical exam.

    Some other tests can be useful. Blood tests may pick up specific antibodies that indicate rheumatoid arthritis, although you can still have the condition with negative results.

    X-rays may also reveal joint damage if the disease is advanced. If there is uncertainty, an ultrasound or MRI can help detect inflammation.

    How is osteoarthritis treated?

    No treatment can stop osteoarthritis progressing. However many people manage their symptoms well with advice from their doctor and self-care. Exercise, weight management and pain medicines can help.

    Exercise has been shown to be safe for osteoarthritis of the knee, hip and hand. Many types of exercise are effective at reducing pain, so you can choose what suits you best.

    For knee osteoarthritis, managing weight through diet and/or exercise is strongly recommended. This may be because it reduces pressure on the joint or because losing weight can reduce inflammation. Anti-obesity medicines may also reduce pain.

    Topical and oral anti-inflammatories are usually recommended to manage pain. However, opioids (such as tramadol or oxycodone) are not, due to their risks and limited evidence they help.

    In some cases antidepressants such as duloxetine may also be considered as a treatment for pain though, again, evidence they help is limited.

    What about rheumatoid arthritis?

    Treatments for rheumatoid arthritis focus on preventing joint damage and reducing inflammation.

    It’s essential to get an early referral to a rheumatologist, so that treatment with medication – called “disease-modifying anti-rheumatic drugs” – can begin quickly.

    These medicines suppress the immune system to stop inflammation and prevent damage to the joint.

    With no cure, the overall goal is to achieve remission (where the disease is inactive) or get symptoms under control.

    Advances in treatment

    There is an increasing interest in prevention for both types of arthritis.

    A large international clinical trial is currently investigating whether a diet and exercise program can prevent knee osteoarthritis in those with higher risk – in this case, women who are overweight and obese.

    For those already affected, new medicines in early-stage clinical trials show promise in reducing pain and improving function.

    There is also hope for rheumatoid arthritis with Australian researchers developing a new immunotherapy. This treatment aims to reprogram the immune system, similar to a vaccine, to help people achieve long-term remission without lifelong treatment.

    Hit the reset

    If you can’t get all the way into these, don’t worry—so long as you can feel the stretch, it’s doing you good and getting you where you need to be:

    • Seated knee-open stretch: sit tall with your legs straight, bend one knee upwards, open it outwards, then hinge from your hips and lean forwards while keeping your spine straight; feel a stretch through your adductors and your hamstrings.
    • Seated 90-90 side-flex stretch: sit in a 90-90 position, lean forwards over your front leg, then walk your hands to the outside of your bent leg and let your ribs move sideways towards the floor; allow your body to relax into the side-bend.
    • Pancake lift-activation: sit in a wide-leg pancake, keep your spine neutral, walk your hands forwards, then lift one knee at a time while keeping your heel on the floor; move slowly to wake up your deep hip and adductor muscles.
    • Deep lunge hip reset: move into a deep lunge with your back knee down, push your front knee forwards while raising your arms and lightly extending your back, then shift your hips backwards onto your glutes with your front leg straight; alternate between these two positions with good, steady control.
    • diagonal adductor lean: from kneeling, step one foot out on a diagonal, keep your heel down and your knee aligned with your foot, then lean your torso towards the angle of your front leg without letting your hips rotate; feel the stretch deep in your hip and adductors.

    As with most exercises of this kind, little and often is key. Or you can do a lot and often if you want! Don’t let us stop you. But the point is that it’s better to do little and often, than a lot 3x per month for a month and a half and a bit.

    When comparing apple to pear, we picked the pear.

    Why?

    Both are great! But there’s a category that puts pears ahead of apples…

    Looking at their macros first, pears contain more carbs but also more fiber. Both are low glycemic index foods, though.

    In the category of vitamins, things are moderately even: apples contain more of vitamins A, B1, B6, and E, while pears contain more of vitamins B3, B9, K, and choline. That’s a 4:4 split, and the two fruits are about equal in the other vitamins they both contain.

    When it comes to minerals, pears contain more calcium, copper, iron, magnesium, manganese, phosphorus, potassium, selenium, and zinc. A resounding victory for pears, as apples are not higher in any mineral.

    In short, if an apple a day keeps the doctor away, a pear should keep the doctor away for about a day and a half, based on the extra nutrients ← this is slightly facetious as medicine doesn’t work like that, but you get the idea: pears simply have more to offer. Apples are still great though! Enjoy both! Diversity is good.

  • Your Heart In Their Hands: Surgeon Preferences & Survival Rates

    Unless you are paying entirely out-of-pocket for a heart surgery, you will not usually get final say over which surgeon you get.

    The surgeon, however, will have final say over what they actually do when they open you up.

    And their preferences, it seems, can make all the difference:

    MAG vs SAG

    When doing coronary artery bypass grafting, (CABG), surgeons may prefer to do multi-arterial grafting (MAG) or single-arterial grafting (SAG).

    Recently, there was a study analysing more than a million Americans who underwent CABG on Medicare over an 18-year period, looking at outcomes for MAG vs SAG.

    The superficial news: those who received MAG had much better long-term survival chances than those who received SAG.

    However: this may be less to do with the relative merits of the procedures themselves, and more to do with the preferences of the surgeon.

    The “eyeball test”

    If surgeons look at a patient and think they will not have many years to live after surgery, they may opt for the SAG, as the long-term benefits of the MAG will only manifest in the long-term.

    This may seem a little self-defeating (indeed, maybe you won’t live to see the long-term if you don’t get the surgery type with the longer-term survival chances), there can be other factors involved, that may make surgeons more interested in your short-term survival chances.

    Or you might just not have enough donor artery tissue available to pick and choose; after all, a person having a coronary artery bypass quite possibly won’t have great arteries in their arm or leg, either.

    Or a person could be missing limbs (a common complication, given the comorbidities of both peripheral artery disease, and diabetes).

    Why it might be ok that things are like this

    When factoring in surgeon preference for MAG or SAG as an instrumental variable, no significant difference in long-term survival was observed. This may explain inconsistencies with randomized controlled trials like the Arterial Revascularization Trial (ART), which also found no survival benefit of MAG over SAG.

    Also, MAG recipients were generally younger, healthier, and from more resourceful areas, which likely had a further impact on MAG-giving decisions, and/but at the same time, may also have increased survival chances for reasons other than that they got MAG rather than SAG.

    …or your loved one, of course. Now, having a coronary artery bypass surgery of any kind is not a fun activity; it will be dangerous, it’ll be stressful before and after, and the recovery will often not be an easy time either. However, it is possible to learn more about what is going on / what will happen, ask the right questions, and get the best options for you (which may not always be the same as the best options for someone else).
