What Heat Waves Do To Chronic Pain

As the world heats up, there’s been a lot in the news not just about the record-breaking temperatures each year, and there’s also been a fair bit written about the effects of extreme heat, and especially prolonged extreme heat, on the human body.

But what’s this about heat waves and chronic pain? Isn’t it supposed to be cold damp weather that does that?

Too hot to handle

First of all, the “cold damp weather” thing is contested: No, your aches and pains don’t get worse in the cold. So why do we think they do?

Hot weather, on the other hand? Now you’re definitely cooking.

More prosaically, researchers (Dr. Feino Sun & Dr. Kai Zhang) followed 35,152 U.S. adults aged 50+ from 1998–2022, linking chronic pain reports with local temperature data.

There were some interesting and superficially conflicting results, which make sense once we examine them:

  • Extreme heat overall: sustained extreme heat was not significantly associated with high-impact chronic pain reporting across the overall population.
  • Rural residents in particular: cumulative extreme-heat exposure was associated with a higher incidence of high-impact chronic pain among rural adults.
  • Lower-wealth adults, anywhere: older adults in the lowest wealth quartile also had a higher incidence of high-impact chronic pain with cumulative extreme-heat exposure.
  • Higher-wealth adults, anywhere: among the wealthiest participants, extreme heat was associated with lower pain incidence than for the general population and greater recovery.

In other words: money can buy improved access to air conditioning, health care, and ways to avoid heat.

One last thing about this study: remember when we said the “cold damp weather” thing is contested? Well, this study does support extreme cold being a problem too, with 4% increases in chronic pain reporting—but that’s with long-term exposure to many extreme cold days, not “I went out for an hour and it was a bit cold today”.

In a similar vein, while heatstroke can kill quickly (see: Stay Safe From Heat Exhaustion & Heatstroke!), again the biggest issue for chronic pain (and other adverse chronic health conditions that may or may not be associated with chronic pain) is usually not so much the temperatures in the moment, unless exceeding a certain level, but rather when there are long periods of the nighttime temperatures not dropping below the 20s (Celsius) or 70s (Fahrenheit).     Hold the celery juice

    Let’s get right down to it: post-holiday guilt and the promise of “purifying” your body are powerful selling points for a multitrillion-dollar wellness industry that benefits from framing indulgence as moral failure. However, in reality, a January detox is unnecessary, unsupported by science, and largely driven by marketing alone.

    People also are often a little hazy about what a “toxin” is. Consider the following items, and see if you can spot what’s safe and what’s toxic:

    • Sugar
    • Alcohol
    • Water
    • Botulinum toxin

    Did you get it right? Sugar, alcohol, water, and Botulinum toxin (Botox) are all dangerously toxic above a certain dose, and safe beneath that dose.

    Yes, the World Health Organization has said that “the only safe amount of alcohol is zero“, but that’s based on number of drinks. If you use hand sanitizer (or estradiol gel!) then some alcohol will enter your bloodstream via transdermal infusion, but the amount of alcohol entering your bloodstream is so tiny (most of the alcohol evaporates off your skin), that you’d have to bathe in it to get toxic effects.

    So the moral of the story here is: the dose makes the poison.

    In the case of sugar, completely “banning” sugar often increases cravings and binge behavior, whereas reducing added sugar—especially sugary drinks—while allowing flexibility is more realistic and sustainable.

    Writer’s example: I consider myself someone who, broadly speaking, “doesn’t eat sugar”. But I’ll use some teriyaki sauce in cooking (contains sugar), or even disregard the small amount of unavoidable added sugar in my annual healthy tiramisu.

    Back to Dr. Machin, she also wants us to know that broadly speaking, your liver and kidneys do not need cleansing. They already detoxify themselves efficiently, regenerate when damaged (especially the liver, which is exceptionally good at this; kidneys are much slow to regain function if damaged), and generally function well if just left to do their thing and not actively sabotaged.

    On which note, you may be wondering what actually supports detox organs: staying well hydrated, limiting alcohol, avoiding excess abdominal fat*, and eating a balanced diet do far more for liver and kidney health than any cleanse.

    *which is also, happily, a consequence of looking after your liver in particular—the healthier your liver is, the less abdominal fat you will tend to accumulate, unless there is something unusual going on (i.e. you have some uncommon health condition unrelated your liver).

    Juice cleanses miss the point so badly that it’s basically a “friendly fire” situation: your body already removes waste through your bowel, lungs, liver, and kidneys, and eating whole fruits and vegetables provides fiber and micronutrients which help with this, but juice? That provides mostly sugar with practically no fiber, and actively sabotages your metabolic health, including putting a lot of pressure on your liver, and potentially (due to the osmotic gradient) doing more harm than gut for your kidneys.

    Also very bad are detoxes as crash diets: many cleanses are very low-calorie diets that cause mostly water loss, reduce muscle mass, slow metabolism, increase hunger hormones, and almost always lead to weight regain. So, we recommend to skip those.

    One last warning: coffee enemas are dangerous! They can cause burns, intestinal perforation, infections, and microbiome disruption, and have sometimes caused death. Coffee is great for your gut, iff you drink it. Please don’t put it up your butt.

    And finally, what works instead: establish (or return to) normal healthy routines, eat nutritious foods at least most of the time, move plenty daily, and prioritize adequate sleep—even though these approaches are boring and not marketable, they do have the benefit of actually working.

    We’ll not keep them a mystery; they are:

    1. Perform movements that target the range of motion in stiff joints, especially in knees and hips, to prevent them from being stuck in limited positions overnight.
    2. Use relaxation techniques like a hot shower, heating pad, or light reading before bed to reduce muscle tension and stiffness upon waking.
    3. Manage joint swelling during the day through gentle movement, compression sleeves, and self-massage .
    4. Maintain a balanced level of activity throughout the day to avoid excessive stiffness from either overactivity or, on the flipside, prolonged inactivity.
    5. Use pillows to support joints, such as placing one between your knees for hip and knee arthritis, and ensure you have a comfortable pillow for neck support.
    6. Eat anti-inflammatory foods prioritizing fruits and vegetables to reduce joint stiffness, and avoid foods high in added sugar, trans-fats, and saturated fats.
    7. Perform simple morning exercises targeting stiff areas to quickly relieve stiffness and ease into your daily routine.
    8. Engage in strength training exercises 2–3 times per week to build stronger muscles around the joints, which can reduce stiffness and pain.
    9. Ensure you get 7–8 hours of restful sleep, as poor sleep can increase stiffness and pain sensitivity the next day. 10almonds note: we realize there’s a degree of “catch 22” here, but we’re simply reporting her advice. Of course, do what you can to prioritize being able to get the best quality sleep you can.
    10. Perform gentle movements or stretches before bed to keep joints limber, focusing on exercises that feel comfortable and soothing.

  • How Useful Is Hydrotherapy?

    Hydrotherapy is a very broad term, and refers to any (external) use of water as part of a physical therapy. Today we’re going to look at some of the top ways this can be beneficial—maybe you’ll know them all already, but maybe there’s something you hadn’t thought about or done decently; let’s find out!

    Notwithstanding the vague nature of the umbrella term, some brave researchers have done a lot of work to bring us lots of information about what works and what doesn’t, so we’ll be using this to guide us today. For example:

    Scientific Evidence-Based Effects of Hydrotherapy on Various Systems of the Body

    Swimming (and similar)

    An obvious one, this can for most people be a very good full-body exercise, that’s exactly as strenuous (or not) as you want/need it to be.

    It can be cardio, it can be resistance, it can be endurance, it can be high-intensity interval training, it can be mobility work, it can be just support for an aching body that gets to enjoy being in the closest to zero-gravity we can get without being in freefall or in space.

    Depending on what’s available for you locally (pool with a shallow area, for example), it can also be a place to do some exercises normally performed on land, but with your weight being partially supported (and as a counterpoint, a little resistance added to movement), and no meaningful risk of falling.

    Tip: check out your local facilities, to see if they offer water aerobics classes; because the water necessitates slow movement, this can look a lot like tai chi to watch, but it’s great for mobility and balance.

    Water circuit therapy

    This isn’t circuit training! Rather, it’s a mixture of thermo- and cryotherapy, that is to say, alternating warm and cold water immersion. This can also be interspersed with the use of a sauna, of course.

    this last one is about thermal shock-mediated hormesis, which sounds drastic, but it’s what we’re doing here with the hot and cold, and it’s good for most people!

    Pain relief

    Most of the research for this has to do with childbirth pain rather than, for example, back pain, but the science is promising:

    Post-exercise recovery

    It can be tempting to sink into a hot bath, or at least enjoy a good hot shower, after strenuous exercise. But does it help recovery too? The answer is probably yes:

  • Clicking Hips: Why It Happens & How To Fix It

    …is not as scary as it sounds!

    Clicking hips during movement are common and usually not harmful, especially if there’s no pain. The most common kind, “internal snapping”, usually occurs when the iliopsoas tendon slides over bony structures like the femoral head.

    In other words, it’s the hip equivalent of cracking knuckles.

    Nevertheless, clicking is more likely when exercises are performed quickly or with poor form, particularly if the lower back arches, so here are some exercises to minimize that, by improving hip alignment, core strength, and controlled movement:

    • Lunge stretch: stretches tight hip flexors and quads by lunging forward with core engaged and pelvis tucked, optionally adding a quad stretch by pulling the back foot toward the glutes.
    • Ab-strengthening leg extensions: lying flat with the lower back pressed to the floor, legs extend slowly while maintaining core control to target the lower abs and prevent hip misalignment.
    • Back extensions: strengthens the back by lifting the upper body slowly while lying face down, helping balance core strength and support proper hip positioning.
    • Alternating leg lifts: performed slowly with a tight core, one leg lowers toward the ground and returns; modified with bent knees if snapping occurs to strengthen the hip flexors without discomfort.
    • Leg circles: slowly circling the legs with strong core engagement, adjusting the range of motion or bending knees to reduce hip clicking and build strength in controlled movement.

    Thank you! And, no, sorry. Any salt that is sodium chloride has the exact same effect because it’s chemically the same substance, even if impurities (however pretty) make it look different.

    If you want a lower-sodium salt, we recommend the kind that says “low sodium” or “reduced sodium” or similar. Check the ingredients, it’ll probably be sodium chloride cut with potassium chloride. Potassium chloride is not only not a source of sodium, but also, it’s a source of potassium, which (unlike sodium) most of us could stand to get a little more of.

    Basic exercise:

    • Imagine stepping over an electric fence side to side.
    • Lift each leg high but slowly to engage hip muscles.
    • Adjust the height and speed based on ease/difficulty.

    Variations:

    • Step over an imaginary side fence.
    • Step over an imaginary front fence.
    • Step sideways in the opposite direction.
    • Step backward to complete a square.
    • Ensure both legs are worked evenly.

    As a bonus, it also improves balance!
