Migraine is more than just a headache. A neurologist explains the 4 stages

A migraine attack is not just a “bad headache”.

Migraine is a debilitating neurological condition which can cause nausea, vomiting, and sensitivity to light or sound, in addition to severe headaches.

Migraine affects roughly five million Australians, but few people understand the different stages of a migraine attack.

Knowing the four distinct phases can help you recognise the symptoms and manage pain at each stage.

Phase 1: Premonitory

The first phase of migraine development is the “premonitory” or “prodrome” phase. It functions like a warning period which begins 24 to 48 hours before a migraine attack fully sets in.

The premonitory phase has a lot to do with the hypothalamus. The hypothalamus is the part of the brain which regulates key functions such as body temperature, appetite, mood and sleep.

When a person experiences a migraine attack, their hypothalamus becomes abnormally activated. The hypothalamus is connected to other parts of the brain with different functions, so this abnormal activation can also disrupt how those parts function.

This can lead to symptoms such as poor concentration, food cravings, irritability and insomnia. If you notice these early signs, you’re more likely to “catch” the start of a migraine attack and be able to treat it early.

Phase 2: Aura

The second phase of a migraine attack is called “aura”. Aura refers to various neurological symptoms which affect your vision, speech or ability to feel sensations. Visual auras, which mainly affect your vision, are the most common kind.

Visual aura symptoms can include seeing flashing lights, swirling shapes or blind spots. A sensory aura can lead to numbness or tingling in your face or limbs. In severe cases, people may even have trouble speaking.

Research suggests a process called cortical spreading depression contributes to aura symptoms. During this process, a wave of electrical activity spreads very slowly through the brain and can impact how certain brain regions function.

Only 30% of people experience migraine with aura.

Phase 3: Headache

The third phase of a migraine attack is the headache. This is when people typically experience a throbbing or pulsating headache, alongside other symptoms like nausea and sensitivity to light and sound.

This phase usually lasts between four and 72 hours if untreated.

When different brain networks become activated during a migraine attack, other symptoms can develop in addition to headache.

When the medulla or “vomit centre” of the brain is abnormally activated, it can lead to nausea and vomiting.

The trigeminal nerve, the nerve which allows you to feel sensations on your face, can also become abnormally activated. This causes the release of chemicals which may be perceived by the brain as pain.

One of these chemicals is a protein called calcitonin gene-related peptide (CGRP). Some injectable types of migraine medication block this protein to reduce pain.

Phase 4: Postdrome

The fourth and final phase is the “postdrome”. It is also known as the “migraine hangover”.

During this recovery phase, your brain is working hard to return to its normal functioning. That is why you may feel even more fatigued or have difficulty concentrating after a migraine attack.

So, how can I manage a migraine attack?

It helps to know the symptoms and stages of migraine development.

If you have predictable symptoms, particularly during the premonitory phase, it’s best to carry pain medications or anti-nausea tablets with you. That way you can treat early symptoms as soon as they arise. It can also be a sign to rest, ideally before the headache phase sets in.

In the aura phase, taking migraine-specific pain medications such as triptans, aspirin or anti-inflammatory pain killers may stop the headache phase from starting.

If you have more than four migraine attacks each month, you may also consider taking preventive medications. These are usually daily tablets which help control the baseline level of head pain you experience. Injectable options are also available.

Finally, don’t ignore the postdrome phase. If you push yourself too hard during this recovery period, you may experience overlapping migraine attacks. This is when one migraine attack starts before the last one resolves itself. Overlapping migraine attacks are much harder to treat.

You may also experience other symptoms related to the migraine attack. These can include dizziness, neck pain, or ringing in the ears. If you have any of these additional symptoms, you should consult your neurologist to check they are not caused by a more serious underlying condition.

And if you are a woman who experiences migraine with aura, speak to your doctor before starting hormone-based contraception. This is because you may need different treatment than someone who does not experience aura symptoms.

By understanding the different phases and symptoms of migraine, you will be better equipped to tackle any future attacks that come.

    When comparing banana to papaya, we picked the banana.

    Why?

    In terms of macros, the banana boasts nearly 2x the fiber, a little over 2x the carbs, and more than 2x the protein, making it the nutrient-dense option in the macros category.

    In the category of vitamins, bananas have more of vitamins B1, B2, B3, B5, B6, B7, and choline, while papaya has more of vitamins A, B9, C, E, and K, making for a marginal win for bananas this time.

    When it comes to minerals, bananas have more copper, iron, magnesium, manganese, phosphorus, potassium, selenium, and zinc, while papaya has more calcium. A clear win for bananas here.

    Adding up the sections makes for a clear overall win for bananas, but by all means enjoy either or both, as both have their merits!

    Uric Acid’s Extensive Health Impact (And How To Lower It)

    This is Dr. David Perlmutter. He’s a medical doctor, and a Fellow of the American College of Nutrition. He’s a member of the Editorial Board for the Journal of Alzheimer’s Disease, and has been widely published in many other peer-reviewed journals.

    What does he want us to know?

    He wants us to know about the health risks of uric acid (not something popularly talked about so much!), and how to reduce it.

    First: what is it? Uric acid is a substance we make in our own body. However, unlike most substances we make in our body, we have negligible use for it—it’s largely a waste product, usually excreted in urine.

    However, if we get too much, it can build up (and crystallize), becoming such things as kidney stones, or causing painful inflammation if it shows up in the joints, as in gout.

    More seriously (unpleasant as kidney stones and gout may be), this inflammation can have a knock-on effect triggering (or worsening) other inflammatory conditions, ranging from non-alcoholic fatty liver disease, to arthritis, to dementia, and even heart problems. See for example:

    How can we reduce our uric acid levels?

    Uric acid is produced when we metabolize purine nucleotides, which are found in many kinds of food. We can therefore reduce our uric acid levels by reducing our purine intake, as well as things that mess up our liver’s ability to detoxify things. Offsetting the values for confounding variables (such as fiber content, or phytochemicals that mitigate the harm), the worst offenders include…

    Liver-debilitating things:

    • Alcohol (especially beer)
    • High-fructose corn syrup (and other fructose-containing things that aren’t actual fruit)
    • Other refined sugars
    • Wheat / white flour products (this is why beer is worse than wine, for example; it’s a double-vector hit)

    Purine-rich things:

    • Red meats and game
    • Organ meats
    • Oily fish, and seafood (great for some things; not great for this)

    Some beans and legumes are also high in purines, but much like real fruit has a neutral or positive effect on blood sugar health despite its fructose content, the beans and legumes that are high in purines, also contain phytochemicals that help lower uric acid levels, so have a beneficial effect.

    Eggs (consumed in moderation) and tart cherries have a uric-acid lowering effect.

    Water is important for all aspects of health, and doubly important for this.

    Hydrate well!

    Lifestyle matters beyond diet

    The main key here is metabolic health, so Dr. Perlmutter advises the uncontroversial lifestyle choices of moderate exercise and good sleep, as well as (more critically) intermittent fasting. We wrote previously on other things that can benefit liver health:

    …in this case, that means the liver gets a break to recuperate (something it’s very good at, but does need to get a chance to do), which means that while you’re not giving it something new to do, it can quickly catch up on any backlog, and then tackle any new things fresh, next time you start eating.

  • How To Control Cravings, Hunger & Emotional Eating

    Firstly, it’s a tale of two hormones: ghrelin (“the hunger hormone”) which increases with calorie restriction, stress, or sleep deprivation, and CCK (cholecystokinin) which reduces hunger when the stomach detects certain nutrients, including omega-3, amino acids (protein), and CLA (conjugated linoleic acid).

    So, how to use that information?

    Five Pillars Of Craving Control

    Laura’s tips are:

    1. Satisfy fullness-signal receptors early: eat 40g of protein per meal, and include sources of omega-3s and CLA (or take supplements). Taking these nutrients early in meals can activate CCK and reduce overeating.
    2. Consider using glutamine: this is an amino acid that helps promote the release of CCK and also reduces sweet cravings. You can get it from foods, or as a supplement (5g powder in water before meals or during cravings). Here’s an example product on Amazon 😎
    3. Avoid emulsifiers in processed foods: they can cause gut receptors to retract, preventing hunger signals from being regulated. Common emulsifiers include ingredients ending in -ate, -at, or gum.
    4. Meal order matters: eat protein and vegetables before carbohydrates! This promotes CCK release and reduces blood sugar spikes, lowering overall food intake. Eating carbs first, on the other hand, can spike hunger and promote overeating.
    5. Understand deeper roots of eating habits: appetite and cravings can be influenced by many factors. Her own binge-eating was linked to undiagnosed ADHD and thus low dopamine (so, she was eating not for nutrients, but for dopamine). ADHD meds helped her overcome that. Yours might be for the same or entirely different reasons, but either way, it’s worth exploring.

  • Feeding your baby butter won’t help them sleep through the night, whatever TikTok says

    Sleep is the holy grail for new parents. So no wonder many tired parents are looking for something to help their babies sleep.

    A TikTok trend claims giving your baby a tablespoon or two of butter in the evening will help them sleep more at night.

    As we’ll see, butter is just the latest food that promises to help babies sleep at night. But no single food can do this.

    So if you’re a new parent and desperate for a good night’s sleep, here’s what to try instead.

    Is my baby’s sleep normal?

    Babies need help to fall asleep, through feeding, movement (like rocking) or touch (like a cuddle or massage).

    Newborn babies also do not know night from day. Melatonin in breastmilk helps babies sleep more at night until they start to make this sleep-inducing hormone themselves. Bottlefed newborn babies do not have access to this melatonin. Regardless of how you feed your baby, it can take several months for them to develop a sleep pattern with longer stretches at night.

    Babies also sleep lighter than older children and adults. Light sleep helps ensure they continue breathing, protecting them from SIDS (sudden infant death syndrome). It also means they wake easily and often.

    The idea that babies should sleep deeply, alone and for long stretches, goes against their physiology. So “sleeping like a baby” usually means waking quite a lot at night.

    Yet, many parents have been asked whether their baby is sleeping through the night and is a “good baby”. The perception is that if a baby doesn’t sleep for long stretches at night, it must be “bad”.

    This may lead parents to say their babies sleep longer than they really do, setting unrealistic expectations for other new parents.

    Could feeding butter do any harm?

    The social pressure around baby sleep can add stress and anxiety for new parents. So the Tiktok trend about feeding babies butter may seem tempting.

    But giving babies any solid food before they are around six months old is not recommended. Babies’ digestive systems are not ready for solid food until they are around six months and feeding them before this can cause constipation or make them more likely to catch an illness. For this reason alone, you should not give your young baby butter.

    From about six months old, babies should be offered nutritious, iron-rich solid foods. Butter doesn’t fit this bill because it is almost all saturated fat. If butter replaces more nutritious foods, babies may not get the vitamins and minerals they need.

    Butter is the latest in a long line of beliefs about certain foods making babies sleep longer at night. It was once thought that adding cereal or crushed arrowroot biscuits in bottle of milk before bedtime would make them sleep longer. Research found this did not increase sleep at all.

    Similarly, there is no evidence that giving babies butter before bed makes them sleep longer.

    In fact, research shows the foods babies eat make no difference to night waking.

    What else can I try?

    Waking overnight doesn’t necessarily mean a baby is hungry. And stopping breastfeeds or bottle feeds overnight doesn’t necessarily reduce night waking.

    Your baby could be too hot or cold, or need a nappy change. But some babies continue to wake at night even without an obvious problem.

    The good news is, sleeping is a skill babies develop naturally as they grow.

    Behavioural sleep interventions, known as “sleep training”, are not very effective in increasing overnight sleep. In one study, sleep training did not reduce the number of night wakes and only increased the length of the longest sleep by about 16 minutes. Sleep training is especially not recommended for babies under six months.

    Look after yourself

    If you’re missing out on sleep at night, try to have small naps during the day while your baby sleeps. Ask friends and family to do some chores to allow you to nap.

    If your baby is crying and you find yourself getting overwhelmed it is OK to put your baby down somewhere safe (like a cot or baby mat) and take some time to settle yourself.

    If your baby’s sleep pattern changes significantly or they haven’t slept at all for more than a day, or if your baby seems to have pain or a fever see your doctor, or family and child health nurse, as soon as possible.

    If you think your baby is not sleeping well because of a breastfeeding problem, the Australian Breastfeeding Association has a national helpline. The association can also advise on co-sleeping.

    The charity Little Sparklers provides peer support for parents, including someone to chat to, about baby sleep. It also has helpful resources.

    UNICEF has resources about caring for your baby at night. And the UK-based Baby Sleep Info Source (Basis) provides evidence-based information about babies and sleep.

  • Is TikTok right? Are there health benefits to eating sea moss?

    Sea moss is the latest “superfood” wellness influencers are swearing by. They claim sea moss products – usually in gel form – have multiple health benefits. These include supporting brain and immune function, or protecting against viruses and other microbes.

    But do these health claims stack up? Let’s take a look.

    What is sea moss?

    Sea moss is produced using a kind of seaweed – particularly red algae – that grow in various locations all around the world. Three main species are used in sea moss products:

    • Chondrus crispus (known as Irish moss or carrageenan moss)
    • Eucheuma cottonii (sea moss or seabird’s nest)
    • Gracilaria (Irish moss or ogonori).

    Some products also contain the brown algae Fucus vesiculosus (commonly known as bladderwrack, black tang, rockweed, sea grapes, bladder fucus, sea oak, cut weed, dyers fucus, red fucus or rock wrack).

    Most sea moss products are sold as a gel that can be added to recipes, used in smoothies, frozen into ice cubes or eaten on its own. The products also come in capsule form or can be purchased “raw” and used to make your own gels at home.

    What’s the evidence?

    Sea moss products claim a host of potential health benefits, from supporting immunity, to promoting skin health and enhancing mood and focus, among many others.

    But is there any evidence supporting these claims?

    Recent studies have reviewed the biological properties of the main sea moss species (Chondrus crispus, Eucheuma cottonii, Gracilaria and Fucus vesiculosus).

    They suggest these species may have anti-inflammatory, antioxidant, anticancer, antidiabetic and probiotic properties.

    However, the vast majority of research relating to Chondrus crispus, Gracilaria and Fucus vesiculosus – and all of the research on Eucheuma cottonii – comes from studies done in test tubes or using cell and animal models. We should not assume the health effects seen would be the same in humans.

    In cell and animal studies, researchers usually administer algae in a laboratory and use specific extracts rich in bioactive compounds rather than commercially available sea moss products.

    They also use very different – often relatively larger – amounts compared to what someone would typically consume when they eat sea moss products.

    This means the existing studies can’t tell us about the human body’s processes when eating and digesting sea moss.

    Sea moss may have similar effects in humans. But so far there is very little evidence people who consume sea moss will experience any of the claimed health benefits.

    Nutritional value

    Eating sea moss does not replace the need for a balanced diet, including a variety of fruits and vegetables.

    Chondrus crispus, Eucheuma cottonii and Gracilaria, like many seaweeds, are rich sources of nutrients such as fatty acids, amino acids, vitamin C and minerals. These nutrients are also likely to be present in sea moss, although some may be lost during the preparation of the product (for example, soaking may reduce vitamin C content), and those that remain could be present in relatively low quantities.

    There are claims that sea moss may be harmful for people with thyroid problems. This relates to the relationship between thyroid function and iodine. The algae used to make sea moss are notable sources of iodine and excess iodine intake can contribute to thyroid problems, particularly for people with pre-existing conditions. That is why these products often carry disclaimers related to iodine sensitivity or thyroid health.

    Is it worth it?

    So you may be wondering if it’s worth trying sea moss. Here are a few things to consider before you decide whether to start scooping sea moss into your smoothies.

    A 375mL jar costs around $A25–$30 and lasts about seven to ten days, if you follow the recommended serving suggestion of two tablespoons per day. This makes it a relatively expensive source of nutrients.

    Sea moss is often hyped for containing 92 different minerals. While there may be 92 minerals present, the amount of minerals in the algae will vary depending on growing location and conditions.

    The efficiency with which minerals from algae can be absorbed and used by the body also varies for different minerals. For example, sodium is absorbed well, while only about 50% of iodine is absorbed.

    But sea moss has also been shown to contain lead, mercury and other heavy metals – as well as radioactive elements (such as radon) that can be harmful to humans. Seaweeds are known for their ability to accumulate minerals from their environment, regardless of whether these are beneficial or harmful for human nutrition. Remember, more doesn’t always mean better.

    What else am I eating?

    While you won’t get a full nutritional breakdown on the jar, it is always wise to check what other ingredients you may be eating. Sea moss products can contain a range of other ingredients, such as lime, monk fruit powder, spirulina and ginger, among many others.

    These ingredients differ between brands and products, so be aware of your needs and always check.

    Despite their health claims, most sea moss products also carry disclaimers indicating that the products are not intended to diagnose, treat, cure or prevent any disease.

    If you have concerns about your health, always speak to a health professional for accurate and personalised medical advice.

    From calorie-counting to cholesterol content, from fish to bottled water, to why of all the people who self-report having an allergy, only around half turn out to actually have one when tested, Dr. Spector sets the record straight.

    The style is as very down-to-earth and not at all self-aggrandizing; the author acknowledges his own mistakes and limitations along the way. In terms of pushing any particular agenda, his only agenda is clear: inform the public about bad science, so that we demand better science going forwards. Along the way, he gives us lots of information that can inform our personal health choices based on better science than indiscriminate headlines wildly (and sometimes intentionally) misinterpreting results.

    Read this book, and you may find yourself clicking through to read the studies for yourself, next time you see a bold headline.

    Bottom line: this book looks at a lot of what’s wrong with what a lot of people believe about healthy eating. Regular 10almonds readers might not find a lot that’s new here, but it could be a great gift for a would-be health-conscious friend or relative
