Chronic kidney disease is often called a silent killer, because 90% of people don’t notice they have it until the disease has progressed to an extreme level.
While none of these signs or symptoms are guaranteed to appear, especially in the early phases, if they do show up then they are cause for getting a check-up done:
Watch out for…
These should serve as alarm bells:
- Foamy urine: persistent dense foam (like beer head) in urine suggests protein (albumin) leakage due to kidney filter damage
- Swelling (pitting edema): especially in the legs, feet, or around the eyes, caused by low blood albumin leading to fluid leakage into tissues
- Nocturia (peeing at night): frequent nighttime urination due to kidneys losing the ability to concentrate urine
- Half-and-half nails: nails with a distinct brownish band on the distal half, linked to chronic kidney disease
- Calcinosis cutis: hard white-yellow skin bumps from calcium phosphate deposits due to high blood phosphate
- Artery calcification: hardened arteries visible on X-ray caused by phosphate-induced bone-like deposits in blood vessel walls
- Muscle cramps: especially at night, due to low calcium, low magnesium, or high blood acidity from impaired kidney function
- Osteoporosis: weak, brittle bones from calcium being leached out due to disrupted calcium regulation—may cause height loss or fractures
- Itchy skin: intense, often nighttime itching caused by uremic toxins irritating nerves or accumulating in skin
- Restless legs syndrome: irresistible urge to move legs at night due to iron deficiency from chronic inflammation and hepcidin overproduction
- Metallic taste in mouth: due to urea breakdown in saliva causing ammonia and other metallic-tasting compounds
- Loss of appetite: also, potentially, nausea and vomiting triggered by toxins activating brain regions that sense food poisoning
- Easy bruising: from reduced platelet stickiness, leading to frequent unexplained bruises, gum bleeding, or nosebleeds
- Uremic frost: white crystalline powder on the skin in advanced kidney failure due to urea excreted through sweat
- Pericarditis: inflammation of the sac around the heart causing chest pain and a scratchy sound due to uremic toxins
- Fatigue (anemia): low red blood cell count from reduced erythropoietin production by kidneys, leading to extreme tiredness
Attentive readers will have noticed two things here:
- Many of these could indicate a lot of other things (e.g. fatigue can be almost anything, osteoporosis isn’t something one sees unless one checks for it, loss of appetite can be many things, etc), which helps mask kidney disease.
- Dr. Deshauer says “17 signs” in her title, so where’s the 17th? The answer is that she listed in 17th place “no symptoms”, because many people have no noticeable symptoms until the disease reaches moderate or advanced stages.
Both of those factors contribute to kidney disease’s “silent killer” status, but with good vigilance, we can stay as healthy as possible.

People presenting at emergency with mental health concerns are experiencing the longest wait times in Australia for admission to a ward, according to a new report from the Australasian College of Emergency Medicine.
But with half of New South Wales’ public psychiatrists set to resign next week after ongoing pay disputes – and amid national shortages in the mental health workforce – Australians who rely on psychiatry support may be wondering where else to go.
If you can’t get in to see a psychiatrist and you need help, there are some other options. However in an emergency, you should call 000.
Why do people see a psychiatrist?
Psychiatrists are doctors who specialise in mental health and can prescribe medication.
People seek or require psychiatry support for many reasons. These may include:
- severe depression, including suicidal thoughts or behaviours
- severe anxiety, panic attacks or phobias
- post-traumatic stress disorder (PTSD)
- eating disorders, such as anorexia or bulimia
- attention deficit hyperactivity disorder (ADHD).
Psychiatrists complement other mental health clinicians by prescribing certain medications and making decisions about hospital admission. But when psychiatry support is not available a range of team members can contribute to a person’s mental health care.
Can my GP help?
Depending on your mental health concerns, your GP may be able to offer alternatives while you await formal psychiatry care.
GPs provide support for a range of mental health concerns, regardless of formal diagnosis. They can help address the causes and impact of issues including mental distress, changes in sleep, thinking, mood or behaviour.
The GP Psychiatry Support Line also provides doctors advice on care, prescription medication and how support can work.
It’s a good idea to book a long consult and consider taking a trusted person. Be explicit about how you’ve been feeling and what previous supports or medication you’ve accessed.
What about psychologists, counsellors or community services?
Your GP should also be aware of supports available locally and online.
For example, Head to Health is a government initiative, including information, a nationwide phone line, and in-person clinics in Victoria. It aims to improve mental health advice, assessment and access to treatment.
Medicare Mental Health Centres provide in-person care and are expanding across Australia.
There are also virtual care services in some areas. This includes advice on individualised assessment including whether to go to hospital.
Some community groups are led by peers rather than clinicians, such as Alternatives to Suicide.
How about if I’m rural or regional?
Accessing support in rural or regional areas is particularly tough.
Beyond helplines and formal supports, other options include local Suicide Prevention Networks and community initiatives such as ifarmwell and Men’s sheds.
Should I go to emergency?
As the new report shows, people who present at hospital emergency departments for mental health should expect long wait times before being admitted to a ward.
But going to a hospital emergency department will be essential for some who are experiencing a physical or mental health crisis.
Managing suicide-related distress
With the mass resignation of NSW psychiatrists looming, and amid shortages and blown-out emergency waiting times, people in suicide-related distress must receive the best available care and support.
Roughly nine Australians die by suicide each day. One in six have had thoughts of suicide at some point in their lives.
Suicidal thoughts can pass. There are evidence-based strategies people can immediately turn to when distressed and in need of ongoing care.
Safety planning is a popular suicide prevention strategy to help you stay safe.
What is a safety plan?
This is a personalised, step-by-step plan to remain safe during the onset or worsening of suicidal urges.
You can develop a safety plan collaboratively with a clinician and/or peer worker, or with loved ones. You can also make one on your own – many people like to use the Beyond Now app.
Safety plans usually include:
- recognising personal warning signs of a crisis (for example, feeling like a burden)
- identifying and using internal coping strategies (such as distracting yourself by listening to favourite music)
- seeking social supports for distraction (for example, visiting your local library)
- letting trusted family or friends know how you’re feeling – ideally, they should know they’re in your safety plan
- knowing contact details of specific mental health services (your GP, mental health supports, local hospital)
- making the environment safer by removing or limiting access to lethal means
- identifying specific and personalised reasons for living.
Our research shows safety planning is linked to reduced suicidal thoughts and behaviour, as well as feelings of depression and hopelessness, among adults.
Evidence from people with lived experience shows safety planning helps people to understand their warning signs and practice coping strategies.

There are people ready to listen, by phone or online chat, Australia-wide. You can try any of the following (most are available 24 hours a day, seven days a week):

Additionally, each state and territory will have its own list of mental health resources.
With uncertain access to services, it’s helpful to remember that there are people who care. You don’t have to go it alone.

First, what this is not: it’s not a productivity book.
What is rather: a book of better wellbeing.
There is a little overlap, insofar as getting “time smart” in the ways that Dr. Whillans recommends will give you more ability to also be more productive—if that’s what you want.
She talks us through time traps and the “time poverty epidemic”, as well as steps to finding time and funding time. Perhaps most critical idea-wise is the chapter on building a “time-affluence habit”, making decisions that prioritize your time-freedom where you can—which in turn will allow you to build yet more. Kind of like compound interest really, but for time.
The writing style is a conversational tone, but peppered with bullet-point lists and charts and the like from time to time, and often with citations to back up claims. It makes for a very readable book, and yet one that’s also inspiring of the confidence that it’s more than just one person’s opinion.
Bottom line: if you sometimes feel like you could do everything you want to if you could just find the time, this book can help you get there.

Slashed Federal Funding Cancels Vaccine Clinics Amid Measles Surge

More than a dozen vaccination clinics were canceled in Pima County, Arizona.
So was a media blitz to bring low-income children in Washoe County, Nevada, up to date on their shots.
Planned clinics were also scuttled in Texas, Minnesota, and Washington, among other places.
Immunization efforts across the country were upended after the federal Centers for Disease Control and Prevention abruptly canceled $11.4 billion in covid-related funds for state and local health departments in late March.
A federal judge temporarily blocked the cuts last week, but many of the organizations that receive the funds said they must proceed as though they’re gone, raising concerns amid a resurgence of measles, a rise in vaccine hesitancy, and growing distrust of public health agencies.
“I’m particularly concerned about the accessibility of vaccines for vulnerable populations,” former U.S. surgeon general Jerome Adams told KFF Health News. Adams served in President Donald Trump’s first administration. “Without high vaccination rates, we are setting those populations and communities up for preventable harm.”
The Department of Health and Human Services, which houses the CDC, does not comment on ongoing litigation, spokesperson Vianca Rodriguez Feliciano said. But she sent a statement on the original action, saying that HHS made the cuts because the covid-19 pandemic is over: “HHS will no longer waste billions of taxpayer dollars responding to a non-existent pandemic that Americans moved on from years ago.”
Still, clinics have also used the money to address other preventable diseases such as flu, mpox, and measles. More than 500 cases of measles so far in a Texas outbreak have led to 57 hospitalizations and the deaths of two school-age children.
In Pima County, Arizona, officials learned that one of its vaccination programs would have to end early because the federal government took away its remaining $1 million in grant money. The county had to cancel about 20 vaccine events offering covid and flu shots that it had already scheduled, said Theresa Cullen, director of the county health department. And it isn’t able to plan any more, she said.
The county is home to Tucson, the second-largest city in Arizona. But it also has sprawling rural areas, including part of the Tohono O’odham Nation, that are far from many health clinics and pharmacies, she said.
The county used the federal grant to offer free vaccines in mostly rural areas, usually on the weekends or after usual work hours on weekdays, Cullen said. The programs are held at community organizations, during fairs and other events, or inside buses turned into mobile health clinics.
Canceling vaccine-related grants has an impact beyond immunization rates, Cullen said. Vaccination events are also a chance to offer health education, connect people with other resources they may need, and build trust between communities and public health systems, she said.
County leaders knew the funding would run out at the end of June, but Cullen said the health department had been in talks with local communities to find a way to continue the events. Now “we’ve said, ‘Sorry, we had a commitment to you and we’re not able to honor it,’” she said.
Cullen said the health department won’t restart the events even though a judge temporarily blocked the funding cuts.
“The vaccine equity grant is a grant that goes from the CDC to the state to us,” she said. “The state is who gave us a stop work order.”
The full effect of the CDC cuts is not yet clear in many places. California Department of Public Health officials estimated that grant terminations would result in at least $840 million in federal funding losses for its state, including $330 million used for virus monitoring, testing, childhood vaccines, and addressing health disparities.
“We are working to evaluate the impact of these actions,” said California Department of Public Health Director Erica Pan.
In Washoe County, Nevada, the surprise cuts in federal funding mean the loss of two contract staffers who set up and advertise vaccination events, including state-mandated back-to-school immunizations for illnesses such as measles.
“Our core team can’t be in two places at once,” said Lisa Lottritz, division director for community and clinical health services at Northern Nevada Public Health.
She expected to retain the contractors through June, when the grants were scheduled to sunset. The health district scrambled to find money to keep the two workers for a few more weeks. They found enough to pay them only through May.
Lottritz immediately canceled a publicity blitz focused on getting children on government insurance up to date on their shots. Vaccine events at the public health clinic will go on, but are “very scaled back” with fewer staff members, she said. Nurses offering shots out and about at churches, senior centers, and food banks will stop in May, when the money to pay the workers runs out.
“The staff have other responsibilities. They do compliance visits, they’re running our clinic, so I won’t have the resources to put on events like that,” Lottritz said.
The effect of the cancellations will reverberate for a long time, said Chad Kingsley, district health officer for Northern Nevada Public Health, and it might take years for the full scope of decreasing vaccinations to be felt.
“Our society doesn’t have a collective knowledge of those diseases and what they did,” he said.
Measles is top of mind in Missouri, where a conference on strengthening immunization efforts statewide was abruptly canceled due to the cuts.
The Missouri Immunization Coalition, which organized the event for April 24-25, also had to lay off half its staff, according to board president Lynelle Phillips. The coalition, which coordinates immunization advocacy and education across the state, must now find alternative funding to stay open.
“It’s just cruel and unthinkably wrong to do this in the midst of a measles resurgence in the country,” Phillips said.
Dana Eby, of the health department in New Madrid County, Missouri, had planned to share tips about building trust for vaccines in rural communities at the conference, including using school nurses and the Vaccines for Children program, funded by the CDC.
New Madrid has one of the highest childhood vaccination rates in the state, despite being part of the largely rural “Bootheel” region that is often noted for its poor health outcomes. Over 98% of kindergartners in the county received the vaccine for measles, mumps, and rubella in 2023-24 compared with the state average of about 91%, and rates in some other counties as low as 61%.
“I will say I think measles will be a problem before I retire,” Eby, 42, said.
Also slated to speak at the Missouri event was former surgeon general Adams, who said he had planned to emphasize the need for community collaboration and the importance of vaccinations in protecting public health and reducing preventable diseases. He said the timing was especially pertinent given the explosion in measles cases in Texas and the rise in whooping cough cases and deaths in Louisiana.
“We can’t make America healthy again by going backwards on our historically high U.S. vaccination rates,” Adams said. “You can’t die from chronic diseases when you’re 50 if you’ve already died from measles or polio or whooping cough when you’re 5.”

To address the elephant in the room, this is not a rehash of Rick Warren’s best-selling “The Purpose-Driven Life”. Instead, this book is (in this reviewer’s opinion) a lot better. - “she raised that community around her”
- “they did so much for us”
- “finding my place in that community changed my life”
- …and so forth. Isn’t that something worth doing?

Lyssavirus is rare, but deadly. What should you do if a bat bites you?

A man in his 50s has died from lyssavirus in New South Wales after being bitten by a bat several months ago.
This is Australia’s fourth human case of bat lyssavirus and the first confirmed case in NSW since the virus was first identified in 1996 in a black flying fox in Queensland.
So what is lyssavirus? And how can you protect yourself if you come into contact with a bat?

Australian bat lyssavirus belongs to the Rhabdoviridae family, the same group of viruses that causes rabies.
It primarily infects bats. Active monitoring suggests fewer than 1% of healthy bats carry the virus, though prevalence rises to 5–10% in sick or injured bats.
In bats, the virus often causes no obvious symptoms, though some show neurological signs such as disorientation, aggression, muscle spasms and paralysis. Some will die.
The virus has been confirmed in all four mainland flying fox species (Pteropus alecto, P. poliocephalus, P. scapulatus and P. conspicillatus) as well as the yellow-bellied sheathtail bat (Saccolaimus flaviventris), a species of microbat.
However, serological evidence – where scientists test for antibodies in bats’ blood – suggests other microbats could be susceptible too. So we should be cautious with all Australian bat species when it comes to lyssavirus.
Rare, but potentially deadly
Unlike rabies, which causes roughly 59,000 human deaths annually, predominantly in Africa and Asia, human infection with bat lyssavirus is extremely rare.
Australian bat lyssavirus, as the name suggests, is unique to Australia. But other bat lyssaviruses, such as European bat lyssavirus, have similarly caused rare human infections.
Human infection with bat lyssavirus occurs through direct contact with infected bat saliva via bites, scratches or open skin. It can also occur if our mucous membranes (eyes, nose, mouth) are exposed to bat saliva.
There’s no risk associated with bat faeces, urine, blood, or casual proximity to roosts.
If someone has been exposed, there’s an incubation period which can range from weeks to more than two years. During this time the virus slowly moves through the body’s nerves to the brain, staying hidden and symptom-free.
Treating the virus during the incubation period can prevent the illness. But if it’s not treated, symptoms are serious and it’s invariably fatal.
The nature of the illness in humans mirrors rabies, beginning with flu-like symptoms (fever, headache, fatigue), then quickly progressing to severe neurological disease, including paralysis, delirium, convulsions, and loss of consciousness. Death generally occurs within 1–2 weeks of symptom onset.
All four recorded human cases in Australia – three in Queensland (in 1996, 1998 and 2013) and the recent NSW case – have been fatal.
There’s no effective treatment once symptoms develop
If someone is potentially exposed to bat lyssavirus and seeks medical attention, they can be treated with post-exposure prophylaxis, consisting of rabies antibodies and the rabies vaccine.
This intervention is highly effective if initiated promptly – preferably within 48 hours, and no later than seven days post-exposure – before the virus enters the central nervous system.
But no effective treatment exists for Australian bat lyssavirus once symptoms develop. Emerging research on monoclonal antibodies offers potential future therapies, however these are not yet available.
So what’s the best protection? And what if a bat bites you?
Pre-exposure rabies vaccination, involving three doses over one month, is recommended for high-risk groups. This includes veterinarians, animal handlers, wildlife rehabilitators, and laboratory workers handling lyssaviruses.
It’s important for members of the public to avoid all direct contact with bats. Only vaccinated, trained professionals, such as wildlife carers or veterinarians, should handle bats.
Public education campaigns are essential to reduce risky interactions, especially in bat-populated areas.
If you get bitten or scratched by a bat, it’s vital to act immediately. Wash the wound thoroughly with soap and water for at least 15 minutes, apply an antiseptic (such as betadine), and seek urgent medical attention.
This tragic case in NSW underscores that while extremely rare, bat lyssavirus is an important public health threat. We need to see enhanced public awareness and ensure vaccination for high-risk groups, alongside ongoing bat monitoring and research into new treatments.

Let’s imagine that instead of being healthily fulfilled in life, you wanted to spend your days as miserable as possible. What should you do?
Here are a few pointers:
Stay still
Avoid physical activity and/or outdoor exposure, to avoid any mood-lifting neurochemicals. In fact, remain indoors as much as possible, preferably in the same room.
If you want to absolutely maximize your misery, make your bedroom the sole space for all activities that it’s possible to do there.
Disrupt your sleep
Keep an irregular sleep schedule by varying your bedtime and wake-up times frequently. Sleep in as much as possible, and make up for it by staying up late to ensure ongoing exhaustion.
Maximize screentime
Use digital entertainment as much as possible to distract you from meaningful activities and rest—as a bonus, this will also help you to avoid self-reflection.
Begin and end your day with a device in hand.
Fuel negative emotions
If you’re going to focus on something, focus on problems you cannot control, to stoke the fires of anger and angst.
A good way of doing this is by staying informed about distressing events, while avoiding meaningful actions to address them. Contribute only in token gestures, and then lament the lack of change.
Follow your impulses
Act on short-term desires without considering long-term consequences, while avoiding behaviors that you know might improve your mood or wellbeing.
Trust that doing the same things that have not previously resulted in happiness, will continue to reliably deliver unhappiness.
Set goals to miss
It’s important that your goals should be vague, and overly ambitious in their scope and/or deliverability. Ideally you should also disregard any preparatory work that a person would normally do before embarking on such a project.
Bonus tip: you can further sabotage any chances of progress, by waiting for motivation to strike before you take any action.
Pursue happiness
Focus on chasing happiness itself, instead of improving your situation or skills. Treat happiness as an end goal, instead of a by-product of worthwhile activities.
