
Why Does Sciatica Improve When You Stretch One Day—But Not The Next?

Dr. Amy Konvalin explains:
Stretching a nerve rarely helps
…because nerves are not particularly stretchy! Which means that stretching the sciatic nerve can aggravate an already irritated nerve because irritated nerves generally don’t respond well to being stretched.
There can be some temporary relief, because stretching can initially make sciatic pain feel better briefly because the nerve responds to the sudden change in stimulation. However, the pain can return after that temporary relief, often worse than before, encouraging repeated stretching and potentially creating a cycle of increasing irritation.
If the relief is enduring, it’s generally because it wasn’t actually sciatica, and was more likely phantom sciatica, a pain caused by fascial tightness that can follow the same general path as the sciatic nerve without actually involving the nerve itself. This type of pain is described as deep, achy, and annoying rather than sharp, shooting, or burning, and hamstring or hip stretching can provide both short- and long-term relief.
A quick test to see if it’s that: walking for five minutes before stretching can help distinguish between the two patterns, with worsening or spreading pain suggesting true sciatica and feeling looser or better suggesting phantom sciatica.
And if it’s sciatica: if the pain is so severe that walking for five minutes or even walking to the bathroom is extremely painful, stretching is discouraged and an ice pack can be used temporarily to calm the sciatic nerve. Dr. Konvalin describes this as a short-term measure that can make movement possible, rather than something that heals the irritated nerve itself.
